Borrelia burgdorferi may be the causal agent of sarcoidosis]

Serum antibody to Borrelia burgdorferi was measured in 33 patients with sarcoidosis who were confirmed clinically and pathologically. The results showed that 81.8% of the patients were positive. In addition, a strain of Borrelia burgdorferi was isolated from a patient's blood. Fourteen patients received ceftriaxone 2 g per day and/or penicillin 12 million per day and a patient received lincomycin 1.2 g per day. The antibody titer of the patients turned to normal level, their SACE turned to normal range, and chest X-ray findings were markedly improved in 3 cases after the treatment. According to the facts mentioned above, we consider that Borrelia burgdorferi may be the causal agent of sarcoidosis and sarcoidosis might be a special type of Lyme disease.     

喉癌的颈清扫术(附5例报告)

本文报告5例喉癌颈清扫术,对其适应症、手术方式等进行了初步分析。认为CN_(1,2),无论病理LN是否阳性均应行ND。CN_3者ND要慎重考虑。CN_0一般不施行ND,除声门上癌或声门癌侵及声门上区时例外。术式以FND为宜。     

蛛网膜下腔注射神经降压素对大鼠血压和心率的影响

本文研究了蛛网膜下腔注射神经降压素（ｎｅｕｒｏｔｅｎｓｉｎ，ＮＴ）对大鼠血压和心率的影响，结果表明：注射ＮＴ后血压立即下降，继而有所回升，但不及正常水平，然后再次降低，持续６０ｍｉｎ；心率无明显改变。Ｈ１受体阻断剂苯海拉明能部分阻断ＮＴ的降血压效应，Ｈ２受体阻断剂甲氰咪胍无翻转作用。注射组胺释放剂Ｃｏｍｐｏｕｎｄ４８／８０使血压下降，待血压恢复正常水平后，再注射ＮＴ，其降压效应减弱。结果提示：蛛网     

小儿内脏器官的形态学研究(五)——新生儿盲肠和阑尾的形态观察

测量30例新生儿盲肠和阑尾的形态和位置。回盲口的形状多数(80.00±7.30％)呈不规则形。阑尾根部体表投影在脐水平线下方1.51±8.95mm和腹股沟韧带中点垂线外侧1.26±10.90mm处。     

Multiple sclerosis: myelin basic protein induced mRNA expression of proinflammatory cytokines in mononuclear cells is suppressed by interferon-β 1b in vitro

Beta-interferon (IFN-β) is a promising treatment in multiple sclerosis (MS), reducing the exacerbation rate and MRI lesion burden, as well as the disease progression in relapsing-remitting MS. IFN-β was originally defined by its antiviral effects, but the interest has recently been focused on its immunomodulatory properties. Myelin basic protein (MBP) is one of several autoantigens considered to be the target for autoaggressive immune responses, which eventually might lead to the development of MS. To study in-vitro effects of IFN-β1b on MBP induced cytokine expression, mRNA for the Th1 cytokines IFN-γ and TNF-α, the Th2 related IL-4 and IL-6, the cytolytic perforin and the immune response downregulating TGF-β was measured with in situ hybridization after culture of blood mononuclear cells (MNC) in the presence and absence of MBP. Numbers of cells expressing IFN-γ, TNF-α, perforin and IL-4 mRNA were significantly suppressed after culture with 10 U/ml IFN-β1b. No such effect was seen on MBP induced IL-6 or TGF-β mRNA expression. These observations suggest that one of the major effects of IFN-β1b is the induction of a shift in the cytokine mRNA profile towards a more immunosuppressive pattern. In parallel in vitro tests, the control substance dexametasone (40 μg/ml) reduced the numbers of cells expressing mRNA for all cytokines under study with the exception of TGF-β, to an extent equal to or even more pronounced than IFN-β1b.     

替尼泊苷和卡莫司汀对脑胶质瘤的治疗作用及耐药的研究

目的研究联合应用替尼泊苷、卡莫司汀对脑胶质瘤细胞的杀伤作用及联合用药的耐药机制.方法测定单独或联合应用替尼泊苷、卡莫司汀两种药物对20例胶质瘤细胞的体外敏感性,检测各标本中多药耐药基因(mdr-1)表达的情况.结果①替尼泊苷、卡莫司汀的敏感性为20%、15%,联合用药为45%,经统计学分析联合应用明显优于单独用药(P<0.05);②联合用药的耐药情况(8/20)与mdr-1基因表达(7/20)符合良好(P<0.05).结论亚硝脲类和鬼臼毒衍生物联合应用对胶质瘤有较好的治疗作用,联合用药的耐药性与mdr-1的表达有关.     

感染性心内膜炎手术时机的初步探讨

本文报道了15例感染性心内膜炎（IE）的外科治疗结果。左心IE9例，其中3例合并风湿性心脏病。右心IE6例，均合并先天性室间隔缺损（VSD）。活动性IE13例，静止期IE2例。右心IE的手术方法是修补VSD，剥除三尖瓣上的赘生物并涂以0.5％活力碘，环缩三尖瓣环。左心IE的处理方法为瓣膜置换。手术死亡2例。本文着重对手术时机进行了讨论。     

病理性损伤因素对肾小管上皮细胞表型转化的影响

目的 :观察低血清、高糖、高蛋白刺激下肾小管上皮细胞向间叶性细胞的表型转化 ,探讨损伤的肾小管上皮细胞在肾小管间质纤维化中的作用。方法 :以体外培养的人近端肾小管上皮细胞为对象 ,分别用含低血清(0 .2mol·L-1小牛血清 )、高葡萄糖 (4.5 g·L-1)、高白蛋白 (15 g·L-1)的培养液培养 7d ,透射电镜检测细胞形态变化 ;免疫组化和Westernblot检测细胞表型改变 ,包括CK、Vimentin、α SMA、Ⅰ和Ⅲ型胶原 ,原位杂交检测Ⅰ型胶原mRNA表达 ,同时检测细胞TGFβ1蛋白表达。 结果 :低血清、高糖、高蛋白直接刺激下 ,肾小管上皮细胞出现明显形态学改变 ,包括细胞变为长梭形 ;透射电镜下细胞内线粒体明显减少 ,粗面内质网明显增加 ,并出现actin样微丝。免疫组化染色和Westernblot显示CK表达明显减弱 ,Vimentin、Ⅰ、Ⅲ型胶原、α SMA和TGFβ1表达增强。原位杂交显示Ⅰ型胶原mRNA表达增强。结论 :低血清、高糖、高蛋白可直接引起人近端肾小管上皮细胞TGFβ1表达增加 ,并发生向间叶性细胞的表型转化     

中国人血管抑素基因的克隆及其序列分析

目的：克隆中国人血管抑素(angiostatin，ANG)基因全长并进行序列分析。方法：应用RT-PCR，将5例健康人肝脏组织ANG基因克隆到pSP71载体上，用Sanger法进行基因测序分析。结果：通过RT-PCR获得一个1141bp的扩增产物，测序发现与已报道的ANG比较，国人ANG上有3个碱基突变位点：分别为第825位C→T；第927位T→G和第1078位G→A。前两个碱基突变氨基酸未发生变化，第3个碱基突变结果使缬氨酸(Val342)改变成为蛋氨酸(Met342)。结论：克隆得到中国人ANG基因片段，其碱基和氨基酸的变化可能是人群中该基因的遗传多态现象。
