To prevent malignant diseases, various strategies are reasonable: screening, avoidance of risk factors, detection and knowledge of preinvasive lesions, screening of special populations with risk factors, and application of protective factors. Screening asymptomatic women for endometrial cancer is generally not warranted. Oral combined contraceptives show significant protective effects; however, their use for preventive measures is limited. For these reasons, understanding of risk factors (hyperestrogenism, polycystic ovary syndrome, chronic anovulation, tamoxifen, obesity, hereditary nonpolyposis colorectal cancer) is important. Awareness of early symptoms (bleeding disturbances, postmenopausal bleeding), diagnosis, and treatment of endometrial hyperplasia is important for preventive reasons and is discussed in detail.

Autotransplantation – eine vergessene Option?

Renal autotransplantation should be considered as an individual treatment in a number of renal or ureteral pathologies, such as ureteral strictures or transitional cell carcinomas of the upper urinary tract. Pyelovesicostomy offers the possibility of a complete ureteral resection and provides the opportunity for a follow-up cystopyeloscopy. In cases of careful patient selection and experience in vascular surgery autotransplantation should not become a forgotten option.     

Knorpelzelltransplantation — eine Alternative zur Endoprothese?

Zusammenfassung Knorpelschäden sind aufgrund Ihrer Häufigkeit, den damit verbundenen Schmerzen und Funktionsstörungen sowie erheblichen Folgekosten eine sozialmedizinisch ausgesprochen bedeutsame Erkrankung. Bei isolierten Grad-4-Knorpelschäden ab 4 cm² ist die Indikation zur Knorpelzelltransplantation gegeben. In Studien werden gute Langzeitergebnisse berichtet. In den letzten Jahren wurde die Kultivierung modifiziert. Durch die Knorpelzelltransplantation wird hyalinartiger Knorpel gebildet. Das Vorliegen von gegenüberliegenden 3- bis 4-gradigen Knorpelschäden bei jungen Patienten stellt ein therapeutisches Dilemma dar. Als Ultima Ratio kann hier zurzeit nur eine unikondyläre oder bei Vorliegen von Knorpelschäden im medialen und lateralen Kompartiment nur die Implantation einer bikondylären Prothese empfohlen werden. Gute Langzeitergebnisse können dadurch erreicht werden. Die Knorpelzelltransplantation wird derzeit nur bei Patienten bis zum 50. Lebensjahr empfohlen. Sie stellt zurzeit keine Alternative zur Endoprothetik dar.
