The major factor to influence localization of parathyroid adenomas is tumor size. P-glycoprotein (P-gp) expression in parathyroid adenomas has been considered to be an another possible factor to influence localization of parathyroid adenomas because false-negative studies have been reported with large tumors and true-positives reported with very small tumors in previous studies. The aim of this study was to characterize Tc-99m MIBI uptake and retention by parathyroid adenomas and to correlate this with cell surface expression of P-gp. Sixteen patients with parathyroid adenoma (larger than 1.5 gm) underwent dual-phase (10min and 2hr) Tc-99m MIBI parathyroid image immediately before parathyroid exploration. Tissues were obtained from normal and abnormal parathyroid glands and from the thyroid gland. Immunohistochemistry (IHC) was obtained with monoclonal antibodies to identify P-gp expression in all tissues. All of the 16 parathyroid adenomas and 32 normal control specimens (16 normal parathyroid and 16 normal thyroid specimens) were submitted for P-gp detection by IHC. The dual-phase Tc-99m MIBI parathyroid image accurately localized 14 parathyroid adenomas, but not the remaining 2 adenomas. The 14 parathyroid adenomas with significant Tc-99m MIBI uptake in delayed 2hr images revealed negative P-gp expression, but the 2 adenomas without significant Tc-99m MIBI uptake, as well as normal parathyroid and normal thyroid specimens, revealed positive P-gp expression when evaluated by IHC. Not only the size of parathyroid adenomas, but also significant P-gp expression limited the sensitivity of dual-phase Tc-99m MIBI parathyroid image to localize parathyroid adenomas before operation.  相似文献

BACKGROUND: It has been demonstrated that during +Gz exposure cerebral blood flow is significantly reduced, resulting in brain ischemia. In pilots, such conditions could recur several times during centrifuge training and combat maneuvers and could possibly cause reversible change in brain energy metabolism. HYPOTHESIS: In rats there is an association between +10 Gz exposure and the decreased brain metabolism, as indicated by decreased adenosine triphosphate (ATP) and ATPase activity, and increased adenosine diphosphate (ADP) and lactate, etc. The aim of the present study was to examine the time course and recovery of brain energy metabolism, lactate, ATPase activity, Na+, K+, Ca2+ and water contents after three +10 Gz exposures in rats. METHODS: There were 64 male Sprague-Dawley rats that were restrained and placed on an animal centrifuge. They were divided into groups of 16. Control rats were exposed to +1 Gz and experimental rats were exposed to +10 Gz three times each for 3 min at 30-min intervals. After being euthanized, rat brains were removed 0 h, 1 h, or 6 h after the last centrifuge run. Brain samples were analyzed for energy metabolism, lactate, Na+-K+-ATPase activity, water and electrolytes contents. RESULTS: The cortical ATP content, Na+-K+-ATPase and lactate dehydrogenase (LDH) activities decreased significantly, whereas the cortical ADP, adenosine monophosphate (AMP) and lactate contents increased significantly 0 h after three +10 Gz exposures, as compared with those of control. The ATP, ADP, and AMP contents returned to their control levels 1 h after the +10 Gz exposures, however, lactate content, Na+-K+-ATPase and LDH activities delayed recovery 6 h after +10 Gz exposures. The cortical K+ content increased significantly 0 h and 1 h after +10 Gz exposures, and returned to the control level 6 h after +Gz exposures. Na+ and water contents increased significantly 1 h and 6 h after the +10 Gz exposures. There was no significant change in Ca2+ content after +Gz exposures. CONCLUSIONS: Three +10 Gz (3 min each) exposures were associated with transient depression of brain metabolism as indicated by a decrease in ATP, Na+-K+-ATPase activity, and an accumulation of lactate, and disturbance of ion homeostasis. It is suggested that a causal relationship might exist between repeated high +Gz exposures and brain metabolism.  相似文献

目的 探讨舌海绵状血管瘤的治疗方法。 方法 舌海绵状血管瘤患者36例。Nd∶YAG激光功率10~20 W,导光纤维直径400~600 μm,行激光表面凝固治疗12例,采用光导纤维插入瘤体凝固治疗者14例,先行表面凝固后插入凝固治疗10例。分析影响疗效的因素并探讨预防并发症的措施。 结果 随访1~7年,痊愈10例,痊愈率27.8%;显效18例,显效率50.0%;有效8例,有效率22.2%。总有效率100%。舌部主要功能保持良好,无严重的并发症发生。 结论 Nd∶YAG激光治疗舌海绵状血管瘤,疗效可靠,操作方便,并发症少,是一种有效的治疗方法。  相似文献

Objective

To determine the usefulness of perfusion MR imaging in assessing the histologic grade of cerebral gliomas.

Materials and Methods

In order to determine relative cerebral blood volume (rCBV), 22 patients with pathologically proven gliomas (9 glioblastomas, 9 anaplastic gliomas and 4 low-grade gliomas) underwent dynamic contrast-enhanced T2*-weighted and conventional T1- and T2-weighted imaging. rCBV maps were obtained by fitting a gamma-variate function to the contrast material concentration versus time curve. rCBV ratios between tumor and normal white matter (maximum rCBV of tumor / rCBV of contralateral white matter) were calculated and compared between glioblastomas, anaplastic gliomas and low-grade gliomas.

Results

Mean rCBV ratios were 4.90°±1.01 for glioblastomas, 3.97°±0.56 for anaplastic gliomas and 1.75°±1.51 for low-grade gliomas, and were thus significantly different; p < .05 between glioblastomas and anaplastic gliomas, p < .05 between anaplastic gliomas and low-grade gliomas, p < .01 between glioblastomas and low-grade gliomas. The rCBV ratio cutoff value which permitted discrimination between high-grade (glioblastomas and anaplastic gliomas) and low-grade gliomas was 2.60, and the sensitivity and specificity of this value were 100% and 75%, respectively.

Conclusion

Perfusion MR imaging is a useful and reliable technique for estimating the histologic grade of gliomas.  相似文献

Objective: To examine the rate of Gleason sum upgrading (GSU) from a sum of 6 to a Gleason sum of ≥7 in patients undergoing radical prostatectomy (RP), who fulfilled the recommendations for low dose rate brachytherapy (Gleason sum 6, prostate‐specific antigen ≤10 ng/mL, clinical stage ≤T2a and prostate volume ≤50 mL), and to test the performance of an existing nomogram for prediction of GSU in this specific cohort of patients. Methods: The analysis focused on 414 patients, who fulfilled the European Society for Therapeutic Radiation and Oncology and American Brachytherapy Society criteria for low dose rate brachytherapy (LD‐BT) and underwent a 10‐core prostate biopsy followed by RP. The rate of GSU was tabulated and the ability of available clinical and pathological parameters for predicting GSU was tested. Finally, the performance of an existing GSU nomogram was explored. Results: The overall rate of GSU was 35.5%. When applied to LD‐BT candidates, the existing nomogram was 65.8% accurate versus 70.8% for the new nomogram. In decision curve analysis tests, the new nomogram fared substantially better than the assumption that no patient is upgraded and better than the existing nomogram. Conclusions: GSU represents an important issue in LD‐BT candidates. The new nomogram might improve patient selection for LD‐BT and cancer control outcome by excluding patients with an elevated probability of GSU.  相似文献

ContextVitamin D receptor (VDR) gene polymorphisms have been strongly associated with bone mineral density in some studies. However, in a recent meta-analysis, no relationship of the VDR BsmI or TaqI polymorphism and fracture risk was found in the meta-analysis of published data.Objective and designOur meta-analysis studied whether a relationship exists between BsmI, TaqI, ApaI and FokI polymorphisms in the VDR gene and risk of fracture.Data sourcesRelevant studies were identified from the following electronic databases: MEDLINE, EMBASE and Current Contents before January 2010.Data synthesisThis meta-analysis included 17 studies with a total of 21 eligible comparisons, which included 2112 fracture cases and 4521 controls. All of these studies reported on Caucasians. The combined results based on all studies showed that fracture cases had a significantly lower frequency of bb genotype of BsmI [odds ratio (OR) = 0.87, 95% confidence interval (CI) = 0.76, 0.98]. When stratifying by fracture type, we found that (1) hip fracture cases had a significantly lower frequency of bb genotype of BsmI (OR = 0.82, 95% CI = 0.70, 0.97); (2) hip fracture cases had a significantly lower frequency of Tt genotype of TaqI (OR = 0.65, 95% CI = 0.43, 0.97); (3) hip fracture cases had a significantly higher frequency of tt genotype of TaqI (OR = 1.74, 95% CI = 1.05, 2.91); (4) vertebral fracture cases had a significantly higher frequency of Aa genotype of ApaI (OR = 1.63, 95% CI = 1.03, 2.59). No significant difference was found in any genotype of FokI.ConclusionOur meta-analysis suggests that there is a modest but statistically significant association between the BsmI bb genotypes and fracture.  相似文献

目的通过2477例不育男性的染色体检查分析,探讨遗传性因素在男性不育症中的重要性。方法采取2477例不育男性的外周血进行染色体核型分析。结果检出染色体异常465例,阳性检出率为18.7%,其中明显异常348例(14.0%),多态等微细变异117例(4.7%)。结论染色体异常是男性不育的重要病因,染色体检查不但有助于明确诊断而且可避免盲目治疗。  相似文献
