伊贝沙坦与雷米普利对充血性心力衰竭疗效比较

目的比较伊贝沙坦与雷米普利对充血性心力衰竭(CHF)的临床疗效及安全性。方法72例CHF患者在常规治疗基础上随机分为2组。伊贝沙坦组(36例)口服伊贝沙坦150 mg.d-1;雷米普利组(36例)口服雷米普利2.5 mg.d-1。治疗前与治疗6 mo后分别用超声心动图测量左室射血分数(LVEF)、左室短轴缩短分数(LVFS)、每搏输出量(SV)、心排量(CO)、心脏指数(CI)评估心功能。结果治疗6 mo后,2组心功能较治疗前非常明显改善(P<0.01),2组之间比较差异无统计学意义(P>0.05)。结论伊贝沙坦与雷米普利治疗CHF均有效,且疗效相当。     

比较4种影像技术对早期股骨头缺血性坏死的诊断敏感性

目的评价CR,CT,ECT和MRI在早期股骨头缺血性坏死(ANFH)中的诊断价值。方法回顾性分析16例23髋经手术、病理证实为早期(符合国际骨循环研究会(ARCO)分期标准Ⅰ期-Ⅱ期)股骨头缺血性坏死患者的CR,CT,ECT和MRI的影像学特征及其检查结果。结果CR诊断早期ANFH11髋,阳性率为47.8%,CT诊断早期ANFH14髋,阳性率为60.9%,ECT诊断诊断早期ANFH21髋,阳性率为91.3%,MRI诊断早期ANFH22髋,阳性率为95.7%。经统计学处理,X2=19.38,P<0.01,CR,CT,ECT和MRI四者敏感性的差异具有显著性意义。结论对早期股骨头缺血性坏死影像学诊断而言,CR诊断敏感性较低,CT可提供较多的诊断信息,ECT和MRI的敏感性高于CT及CR,MRI可准确地诊断早期股骨头缺血性坏死。

同类药物因原料来源、附加成分和生产厂家不同而商品名称各异，有些名称相似的药物其性能却完全不同。临床实习护生常反映课本讲授的药物知识与临床用药相差甚远，不知如何记住所有的药物。为此，作者在临床教学中培养学生养成阅读药物说明书的习惯，收到良好效果。     

Structural shielding design for a gamma ray stereotactic body radiotherapy system.

An OUR-QGD gamma ray stereotactic body radiotherapy system (body knife), made in China, is described. According to its structure and the principle of gamma radiation revolved on a focus, the energy distribution of scattered radiation in its treatment room is calculated. The structural shielding of the wall, roof, and door for a certain treatment room is calculated according to the local radiation protection law.

Dies wird anhand von 2 ausgew?hlten Praxisbeispielen dargestellt. Die Handlungskompetenz von Supervisandinnen und Supervisanden kann durch einfache psychodramatische Interventionen erweitert werden. Handlungshemmung wird überwunden, Spontaneit?t und Kreativit?t kommen ins Laufen und er?ffnen Perspektiven.     

The expression of emotion through nonverbal behavior in medical visits

Relationship-centered care reflects both knowing and feeling: the knowledge that physician and patient bring from their respective domains of expertise, and the physician’s and patient’s experience, expression, and perception of emotions during the medical encounter. These processes are conveyed and reciprocated in the care process through verbal and nonverbal communication. We suggest that the emotional context of care is especially related to nonverbal communication and that emotion-related communication skills, including sending and receiving nonverbal messages and emotional self-awareness, are critical elements of high-quality care. Although nonverbal behavior has received far less study than other care processes, the current review argues that it holds significance for the therapeutic relationship and influences important outcomes including satisfaction, adherence, and clinical outcomes of care.
