Thoracic duct cyst in supraclavicular region.

A 28-year-old female attended an outpatient clinic in October, 1989, because of a tumor in the left supraclavicular fossa, detected in a health examination. Following exploratory puncture of the tumor which yielded milky-white fluid, suggesting a cyst in the thoracic duct, she was admitted to our department. The cyst was unilocular measuring about 6 cm in diameter, and the fluid content was chyle-rich in lipids. Lymphography demonstrated a lymphatic structure adjacent to the lesion and scattered lymph vessels on the cyst surface. On November 16 the cyst was resected. A restiform structure was observed between the cyst and the thoracic duct, but the presence or absence of communication was unclear. The histological diagnosis was thoracic duct cyst. Thoracic duct cyst occurring in the cervical region is very rare. Our case may provide useful information as to its pathogenesis and the mode of retention of cyst fluid.     

Fine needle aspiration cytology of gastric carcinoma.

Four patients between 58 and 81 years of age undergoing investigation and endoscopic biopsy for gastric carcinoma also were subjected to direct-vision fine needle aspiration cytology of their mucosal lesions which yielded malignant cells. The relevance of this technique is discussed regarding both intrinsic and extrinsic lesions of the gastrointestinal tract.

应用免疫磁珠法分离脐血ＣＤ３４＾＋细胞，以比较ＣＤ３４＾＋细胞亚群及各系相关抗原的变化。经过分离，ＣＤ３４＾＋细胞由１．４７±０．７７％升至５７．３９±１６．１７％，分离后ＣＤ３４＾＋ＣＤ３８＾－细胞占ＣＤ３４＾＋细胞的比率为７．８２±４．７９％ＣＤ３４＾＋。细胞纯化同时可减少ＣＤ３＾＋细胞达１４．１２±９．１８倍，ＣＤ１５＾＋细胞减少１６．４２±２１．４７倍；Ｂ系（ＣＤ１９）、巨核系（ＣＤ４１）     

Research review: DNA polymerases as molecular markers of the regenerating capacity of hepatocytes

Hepatocyte regeneration has been widely investigated, with the mitotic index and the incorporation of [³H]thymidine being used as regeneration markers. We focused on the induction of DNA replication enzymes, particularly DNA polymerases (pol) α, δ, and ε. Using rat models, we have shown that the activity of pol α in crude liver extract well represents the regenerating capacity of hepatocytes. Using pol α as an indicator, we analyzed liver regeneration in rat models under various conditions: obstructive jaundice, external or internal biliary drainage, and the obstruction of portal vein branches. It has been revealed that the ligation of the common bile duct alone induces a certain amount of hepatocyte proliferation. It was striking that external biliary drainage suppressed regeneration capacity in cholestatic rat liver after partial hepatectomy. The strong regeneration in nonligated lobes induced by portal branch ligation was similar to the liver regeneration seen after partial hepatectomy with respect to the induction of DNA polymerases. Taken together, the aspects of DNA replication, particularly the induction of DNA polymerases, may contribute to shedding new light on the regeneration of human liver. 50岁以上肾移植的临床分析

自１９８９年１月～１９９５年１２月共施行同种异体尸肾移植术５５６例次，其中５０岁以上患者１８２例（３２７％），对这些患者的临床资料进行比较分析。结果显示：５０岁以上组１、３、５年的人／肾存活率分别为８３４％／７９３％、７７１％／７１１％和５４５％／４５５％，与４９岁以下组比较差异无显著性（Ｐ＞００５），急性排斥发生率并不比４９岁以下组高，并发症发生率尤其是感染发生率及致死率均高于４９岁以下组（Ｐ＜００５）。本资料说明，年龄不是肾移植的主要影响因素，感染和心血管并发症是主要的死亡原因和影响长期存活的因素。适应证的选择和术前充分透析，术后合理应用免疫抑制剂，定期复查及心、肝、肺功能的监护，是提高高龄肾移植患者长期存活的主要因素。     

膀胱移行细胞癌中P16及Rb基因表达的关系

应用免疫组织化学方法对５９例膀胱移行细胞癌中Ｐ１６及Ｒｂ基因的表达进行研究。结果显示：５９例中Ｐ１６及Ｒｂ基因阳性表达率分别为４７４６％、７１１９％，其中１１例肿瘤组织（１８６４％）Ｐ１６及Ｒｂ基因均阳性表达，未发现同一肿瘤组织中同时存在Ｐ１６及Ｒｂ基因的表达缺失。Ｐ１６及Ｒｂ基因在膀胱移行细胞癌中的阳性表达率在肿瘤病理分级及临床分期的差异中均有显著性（Ｐ＜００５）。结果提示：Ｐ１６及Ｒｂ基因的异常表达在膀胱肿瘤的发生、发展过程中起重要作用，膀胱肿瘤中Ｐ１６及Ｒｂ基因同时表达缺失是少见现象     

射精管异位开口于苗勒管囊肿

报告２例射精管异位开口于苗勒管囊肿。均以血精、血尿就诊，通过经皮输精管穿刺造影及染色尿道镜检查确诊。经抗炎及窥视下纵行剪开苗勒管囊肿前壁。１例治愈，１例好转。对本病的病因、临床表现、诊断、治疗及预后进行了讨论。     

Required duration of combined annual ivermectin treatment and vector control in the Onchocerciasis Control Programme in west Africa.

In the extension areas of the Onchocerciasis Control Programme in West Africa, aerial larviciding is supplemented with annual ivermectin treatment, mainly to achieve better control of morbidity. The purpose of this study is to determine whether and to what extent the addition of annual ivermectin treatment permits earlier cessation of vector control than originally recommended. The effectiveness of combined ivermectin distribution and vector control was assessed using an epidemiological model. Model predictions suggest that, dependent on the pre-control endemicity of the area and the proportion of persons treated during each ivermectin round, large-scale annual treatment permits a considerable reduction in the duration of vector control. Taking into account uncertainty about the efficacy of ivermectin, our results indicate that, provided treatment coverage is at least 65% and there is no importation of infection from elsewhere, 12 years of combined control will be sufficient to reduce the risk of recrudescence to below 1% in even the most afflicted areas.     

脊髓Ⅱ号对大鼠损伤脊髓轴浆运输的影响

选取24只Wistar大白鼠，将其中18只制成胸髓右半侧横断损伤的大鼠模型，分别以脊髓Ⅱ号方剂、激素、生理盐水灌喂治疗。未造模的6只大鼠作为正常组，常规喂养。一个月后，注入辣根过氧化物酶（HRP），检测路经损伤区的上下行神经传导束始发核团中的标记细胞。发现脊髓Ⅱ号组HRP标记细胞数明显多于另两组，而与正常大鼠相比没有统计学上的显著差异。说明脊髓Ⅱ号方剂在恢复损伤传导来神经纤维的正常连续性，恢复神经元轴浆运输，促进神经细胞修复再生方面，有显著的效应。
