Massed aversion treatment of sexual deviance

Four patients exhibiting deviant sexual patterns were treated with massed electrical aversion therapy in a single day. Seen at follow-up, they reported no further sexually deviant behaviors.     

Possession trance in a semi-urban community in Sri Lanka.

A survey of possession trance states in a semi-urban population of 7653 identified 37 subjects. The manifestations of the trance, and its course and outcome are outlined. The characteristics of the subjects, including presence of psychiatric disorder, are compared with that of the total population, and significant differences are noted. Four psychodynamic syndromes observed are described with illustrative case vignettes. The functions subserved by possession behaviour are reviewed, and comparisons are drawn between personal possession, ritual possession, and altered states of consciousness in Western society.     

The Use of Impedance Index in the Surveillance of PTFE Femorodistal Grafts

BACKGROUND: Impedance is the equivalent in pulsatile flow of resistance in steady flow. The impedance index has been used successfully in the surveillance of vein grafts, but its use has not been reported in the context of PTFE femorodistal grafts. METHODS: Twenty-eight patients (median age 68 years (IQR 59-73 years) and 20 men) undergoing 28 PTFE femorodistal grafts with a vein cuff were evaluated prospectively comparing the impedance index with standard duplex graft surveillance. All grafts were performed for critical ischaemia. At risk grafts were identified and treated appropriately after angiography. RESULTS: The primary patencies at 1 and 2 years were 82% and 50% respectively. Duplex identified 11 at risk grafts of which 9 had an identifiable correctable lesion. Impedance analysis overpredicted at risk status when compared with duplex in the immediate postoperative phase and was unsuccessful in detecting inflow disease or low flow relating to cardiac failure. Using a threshold index of 0.5, impedance analysis has a sensitivity of 87%, specificity of 88%, with positive and negative predictive values of 76% and 94% respectively. CONCLUSIONS: Impedance index is a non-invasive method of graft surveillance which is applicable to PTFE femorodistal bypasses and may be a useful alternative to duplex although formal validation studies will be required.     

Characterization of 13 novel band 3 gene defects in hereditary spherocytosis with band 3 deficiency

Hereditary spherocytosis (HS) is a common hemolytic anemia of variable clinical expression. Pathogenesis of HS has been associated with defects of several red cell membrane proteins including erythroid band 3. We have studied erythrocyte membrane proteins in 166 families with autosomal dominant HS. We have detected relative deficiency of band 3 in 38 kindred (23%). Band 3 deficiency was invariably associated with mild autosomal dominant spherocytosis and with the presence of pincered red cells in the peripheral blood smears of unsplenectomized patients. We hypothesized that this phenotype is caused by band 3 gene defects. Therefore, we screened band 3 DNA from these 38 kindred for single strand conformational polymorphisms (SSCP). In addition to five mutations detected previously by SSCP screening of cDNA, we detected 13 new band 3 gene mutations in 14 kindred coinherited with HS. These novel mutations consisted of two distinct subsets. The first subset included seven nonsense and frameshift mutations that were all associated with the absence of the mutant mRNA allele from reticulocyte RNA, implicating decreased production and/or stability of mutant mRNA as the cause of decreased band 3 synthesis. The second group included five substitutions of highly conserved amino acids and one in-frame deletion. These six mutations were associated with the presence of comparable levels of normal and mutant band 3 mRNA. We suggest that these mutations interfere with band 3 biosynthesis leading thus to the decreased accumulation of the mutant band 3 allele in the plasma membrane.
