BACKGROUND: Cow's milk is the most important food antigen in infancy and may lead to acute cutaneous symptoms and atopic dermatitis (AD). The role of circulating allergen-specific T cells in the pathogenesis of food-allergic skin symptoms is still under investigation. OBJECTIVE: This study was designed to analyze the cow's milk protein (CMP)-specific T-cell response at the clonal level in infants with AD and cow's milk allergy (CMA) in comparison with infants with AD without CMA. METHODS: We used an antigen-specific culturing system with autologous B cells as antigen-presenting cells to establish CMP-specific T-cell clones derived from PBMCs in infants with AD. T-cell reactivity, measured by using a lymphocyte stimulation test, and cytokine production, measured by using ELISA, was compared between infants with AD with and without CMA. RESULTS: Both infants with and without allergy to cow's milk had a CMP-specific T helper cell response directed against the major proteins in milk. Analysis of antigen-specific cytokine production showed that this response was T(H)2 skewed in infants with CMA, with production of high levels of IL-4, IL-5, and IL-13. In contrast, infants without CMA had a T(H)1-skewed response, with high levels of IFN-gamma and low levels of IL-4, IL-5, and IL-13. CONCLUSION: These data confirm for the first time at the clonal level that food allergy in infants with AD is associated with production of T(H)2 cytokines by circulating antigen-specific CD4(+) T cells, whereas tolerance to food antigens is associated with low levels of these cytokines. This suggests a key role for the T helper cell-derived T(H)2 cytokines in food allergy-related skin symptoms.  相似文献

The intrinsic nonuniformities in the transmit radiofrequency field from standard quadrature volume resonators at high field are particularly problematic for localized MRS in areas such as the temporal lobe, where a low signal‐to‐noise ratio and poor metabolite quantification result from destructive B field interference, in addition to line broadening and signal loss from strong susceptibility gradients. MRS of the temporal lobe has been performed in a number of neurodegenerative diseases at clinical fields, but a relatively low signal‐to‐noise ratio has prevented the reliable quantification of, for example, glutamate and glutamine, which are thought to play a key role in disease progression. Using a recently developed high‐dielectric‐constant material placed around the head, localized MRS of the medial temporal lobe using the stimulated echo acquisition mode sequence was acquired at 7 T. The presence of the material increased the signal‐to‐noise ratio of MRS by a factor of two without significantly reducing the sensitivity in other areas of the brain, as shown by the measured B maps. An increase in the receive sensitivity B was also measured close to the pads. The spectral linewidth of the unsuppressed water peak within the voxel of interest was reduced slightly by the introduction of the dielectric pads (although not to a statistically significant degree), a result confirmed by using a pad composed of lipid. Using LCmodel for quantitative analysis of metabolite concentrations, the increase in signal‐to‐noise ratio and the slight decrease in spectral linewidth contributed to statistically significant reductions in the Cramer–Rao lower bounds (CRLBs), also allowing the levels of glutamate and glutamine to be quantified with CRLBs below 20%. Teeth seldom fracture under normal functional loading. This indicates that the natural tooth design is optimized for the distribution of regular masticatory forces by means of its properties and structure. When a tooth is restored with an intracoronal restoration, however, the incidence of tooth fracture increases. Since remaining tissues do not change, the restorative actions apparently alter the original stress distributions. In this study, the effect of different restoration types (unbonded amalgam and bonded composite restorations) were compared with the original stress conditions of the intact tooth, using finite element analysis. It was shown that an unbonded amalgam restoration did not restore the original stress conditions but led to much higher stresses in the buccal and lingual enamel and to higher tensile stresses in the cavity floor. The unbonded amalgam thus filled the cavity but did not restore the tooth. In contrast, a bonded composite restoration restored the original stress pattern in the tooth if there was no polymerization shrinkage. Polymerization shrinkage causes residual tensile stresses in the dentin around the cavity and in the buccal and lingual enamel. Residual tensile stresses in the buccal and lingual enamel are momentary compensated by compressive stress components during occlusal loading. It was concluded that bonding and elimination of residual stresses are prerequisites for restoring the original tooth integrity.  相似文献

Introduction

It is not clear whether increasing the ultrasonic intensity would enhance the cleaning efficacy of passive ultrasonic irrigation (PUI) inside a root canal. The aim of this study was to evaluate the effect of the ultrasonic intensity on PUI to remove dentin debris and whether there is any lateral effect beyond the ultrasonic tip.

Methods

Each of 15 in vitro root canal models with four standard depressions in the apical part of one canal wall were filled with dentin debris and received PUI repeatedly. The most apical depression was localized apically from the ultrasonic tip. The highest intensity was applied in group 1, the lowest intensity was applied in group 3, and syringe irrigation was performed in group 4 as a control. Before and after irrigation, images of the canal wall with depressions were taken and compared. The removal of dentin debris in the depressions was categorized as clean or not clean. The data were analyzed by means of the chi-square test. The oscillation amplitude of the ultrasonic file at each intensity was recorded in vitro using time-resolved high-speed imaging.

Results

Group 1 (highest intensity) exhibited significantly better cleaning than all the other groups (P < .05); no significant difference was found between the four levels of the depressions within any of the four groups. High-speed imaging showed that the amplitude of the oscillating file increased as the intensity went up, which leads to a higher velocity of the irrigant around the file.

Conclusions

Higher ultrasonic intensity resulted in a higher amplitude of the oscillating file and, consequently, enhanced the cleaning efficacy of PUI.  相似文献

In this experimental study, the nonspherical and translational behavior of individual coated microbubbles of different sizes, in contact with a 20-μm thickness cellulose wall, are observed and categorized systematically. Images from two orthogonally positioned microscopes are merged and then recorded with an ultra-fast framing camera. Large nonspherical deformations were found with 2.25 MHz frequency ultrasound pulses having driving pressures from 80 to 325 kPa. A parametric model combining potential flow theory with a viscous boundary layer at the wall is developed and used to calculate stresses from the optically recorded microbubble oscillations. Peak shear stress of up to 300 kPa and normal stresses of up to 1 MPa are estimated when microbubbles are insonifed with a 2.25 MHz pulse at 325 kPa. The clinical relevance of these results is discussed.  相似文献

Radially oscillating microbubbles can deform when in contact with a wall. These nonspherical shapes have a preferential orientation perpendicular to the wall. Conventional microscope setups for microbubble studies have their optical axis perpendicular to the wall (top view); consequently they have a limited view of the deformation of the bubble. We developed a method to image the bubble in a side view by integrating a mirror in the microscope setup. The image was recorded at 14.5 million frames per second by a high-speed camera. When insonified by a 1-MHz, 140-kPa ultrasound pulse, a 9-microm diameter coated bubble appeared spherical in the top view, but strongly nonspherical in the side view. Its shape was alternatively oblate and prolate, with maximum second order spherical harmonic amplitude equal to the radius.  相似文献

The occurrence of nonspherical oscillations (or surface modes) of coated microbubbles, used as ultrasound contrast agents in medical imaging, is investigated using ultra-high-speed optical imaging. Optical tweezers designed to micromanipulate single bubbles in 3-D are used to trap the bubbles far from any boundary, enabling a controlled study of the nonspherical oscillations of free-floating bubbles. Nonspherical oscillations appear as a parametric instability and display subharmonic behavior: they oscillate at half the forcing frequency, which was fixed at 1.7 MHz in this study. Surface modes are shown to preferentially develop for a bubble radius near the resonance of radial oscillations. In the studied range of acoustic pressures, the growth of surface modes saturates at a level far below bubble breakage. With the definition of a single, dimensionless deformation parameter, the amplitude of nonspherical deformation is quantified as a function of the bubble radius (between 1.5 and 5 mum) and of the acoustic pressure (up to 200 kPa). Increased iron in subcortical structures in patients with Huntington's Disease (HD) has been suggested as a causal factor of neuronal degeneration. The present study examines iron accumulation, measured using magnetic resonance imaging (MRI), in premanifest gene carriers and in early HD patients as compared to healthy controls. In total 27 early HD patients, 22 premanifest gene carriers and 25 healthy controls, from the Leiden site of the TRACK-HD study, underwent 3T MRI including high resolution 3D T(1)- and T(2)-weighted and asymmetric spin echo (ASE) sequences. Magnetic Field Correlation (MFC) maps of iron levels were constructed to assess magnetic field inhomogeneities and compared between groups in the caudate nucleus, putamen, globus pallidus, hippocampus, amygdala, accumbens nucleus, and thalamus. Subsequently the relationship of MFC value to volumetric data and disease state was examined. Higher MFC values were found in the caudate nucleus (p<0.05) and putamen (p<0.005) of early HD compared to controls and premanifest gene carriers. No differences in MFC were found between premanifest gene carriers and controls. MFC in the caudate nucleus and putamen is a predictor of disease state in HD. No correlation was found between the MFC value and volume of these subcortical structures. We conclude that Huntington's disease patients in the early stages of the disease, but not premanifest gene carriers, have higher iron concentrations in the caudate nucleus and putamen. We have demonstrated that the iron content of these structures relates to disease state in gene carriers, independently of the measured volume of these structures.  相似文献
