Background  Indocyanine green (ICG) lymphangiography is being increasingly employed to assess the severity of lymphedema, locate the areas of patent linear lymphatics and dermal backflow and plan treatment. This study suggests a novel method of reporting ICG findings in extremities to enable easy understanding among surgeons and physiotherapists and avoid repeat testing when a patient visits a disparate lymphedema center or clinician. Methods  A reporting protocol was developed in the lymphedema clinic of the plastic surgery department, and patients were asked to bring along the report in every subsequent review. The ICG findings were recorded on the fluorescence imaging system as well. The report was prepared by one and analyzed by two different clinicians without repeating the test on 10 consecutive patients. Results  The interrater reliability of findings in the report was found to be 98.7% among the three clinicians. Foot ulcers in diabetics are prone to polymicrobial infection by bacteria as well as fungi. Infection retards healing of ulcers leading to gangrene and ultimately, amputation of the extremity. The choice of empiric antibiotic therapy is extrapolated from results of studies from other regions, western literature or infection at other sites which may be inappropriate. Thus, a study was conducted to determine microbial aetiology of diabetic foot ulcers and their susceptibility patterns. Pus aspirates or wound swabs were collected from foot ulcers of 70 diabetics at the time of admission and processed for isolation of bacteria (aerobic and anaerobic) and fungi according to standard protocols. Organisms were identified upto species level. Antimicrobial susceptibility testing was performed by Kirby Bauer disk diffusion method as per CLSI standards. One hundred forty-seven organisms were isolated from 70 specimens. Seventy-three percent showed polymicrobial growth. The proportion of Gram negative bacilli, Gram positive cocci, anaerobes and fungi were 57 %, 27 %, 4 % and 12 % respectively. Enterobacteriaceae were the predominant Gram negative bacilli and S.aureus, the predominant Gram positive cocci. Clostridium and Candida species were the commonest among anaerobes and fungi respectively. Twenty-three percent of S.aureus were MRSA and 23 % of Enterobacteriaceae were ESBL producers. Ps.aeruginosa strains demonstrated good susceptibility to antipseudomonal antimicrobials and 16.5 % strains were meropenem resistant. Due to the associated risk of increased morbidity and mortality in infected diabetic ulcers, the wide spectrum of organisms associated with it and their differing susceptibility profile, treatment should be individualised based on microbial culture and antimicrobial susceptibility results.  相似文献

Myoclonus is a brief, rapid, involuntary muscle jerk originating in the central nervous system that can be physiological or a symptom of disease. We report a group of five children with excessive myoclonic jerks, only during sleep, and abnormal EEG during the events. Although only one third of the events had EEG epileptiform correlate, the presence of myoclonus without epileptiform EEG correlate has been described in patients with benign myoclonic epilepsy of infancy. We hypothesize that these findings may represent a variant of benign myoclonic epilepsy of infancy.  相似文献

Medullary carcinoma of thyroid is a relatively uncommon malignancy, which can be sporadic and syndromic in nature. It commonly spreads to regional lymph nodes followed by spreading to distant sites. Breast is an uncommon site of metastasis of this malignancy. Our case is a 38‐year‐old woman married parous woman presenting to the outpatient department with complaints of lump in both the breasts. Fine‐needle aspiration (FNA) was attempted, which revealed a malignancy more suggestive of a metastasis, which was confirmed on CT scan. A detailed history revealed that the patient is a known case of medullary carcinoma of thyroid. The report was given as metastatic medullary carcinoma to the breast after confirming with a calcitonin immunostain. Given the versatility of primary lesions in the breast, minimally invasive FNA cytology (FNAC) technique with adequate sampling helps in identifying metastatic lesions. Differentiating primary from metastatic lesions changes the course of management to the patient. Metastatic lesions should always be kept in mind in the occurrence of known malignancies, however rare the site of occurrence may be. Morphological clues and immunohistochemical work up aid in arriving at correct diagnosis.  相似文献

The causative agent of tuberculosis (TB), Mycobacterium tuberculosis and more recently totally drug-resistant strains of M. tuberculosis, display unique mechanisms to survive in the host. A four-drug treatment regimen was introduced 40 years ago but the emergence of multidrug-resistance and more recently TDR necessitates the identification of new targets and drugs for the cure of M. tuberculosis infection. The current efforts in the drug development process are insufficient to completely eradicate the TB epidemic. For almost five decades the TB drug development process remained stagnant. The last 10 years have made sudden progress giving some new and highly promising drugs including bedaquiline, delamanid, and pretomanid. Many of the candidates are repurposed compounds, which were developed to treat other infections but later, exhibited anti-TB properties also. Each class of drug has a specific target and a definite mode of action. These targets are either involved in cell wall biosynthesis, protein synthesis, DNA/RNA synthesis, or metabolism. This review discusses recent progress in the discovery of newly developed and Food and Drug Administration approved drugs as well as repurposed drugs, their targets, mode of action, drug-target interactions, and their structure-activity relationship.  相似文献
