We proposed a novel surface modification for an artificial hip joint stem from the viewpoint of maintenance and establishment of appropriate bone function and microstructure, represented by the preferred alignment of biological apatite (BAp) and collagen (Col). Oriented grooves were introduced into the proximal medial region of the femoral stem to control the principal stress applied to the bone inside the grooves, which is a dominant factor contributing to the promotion of Col/BAp alignment. The groove angle and the stem material were optimized based on the stress inside the grooves through a finite element analysis (FEA). Only the groove oriented proximally by 60° from the normal direction of the stem surface generated the healthy maximum principal stress distribution. The magnitude of the maximum principal stress inside the groove decreased with increasing the stem Young's modulus, while the direction of the stress did not largely changed. An in vivo implantation experiment showed that this groove was effective in inducing the new bone with preferential Col/BAp alignment along the groove depth direction which corresponded to the direction of maximum principal stress inside the groove. The anisotropic principal stress distribution and the oriented microstructure inside the groove are similar to those found in the femoral trabeculae; therefore, the creation of the oriented groove is a potent surface modification for optimizing implant design for a long-term fixation. 相似文献

Gastrointestinal stromal tumors (GISTs) have been reported to occasionally occur in patients with neurofibromatosis type 1 (NF-1), and many cases have had multiple lesions predominantly involving the small intestine. We report herein a case of multiple GISTs associated with NF-1 from whom laparoscopic surgery was beneficial. In a 79-year-old female admitted with anemia and melena, the abdominal computed tomography revealed a tumor arising from the small intestine. Laparoscopic surgery was performed, and another small tumor was revealed during laparoscopic observation. Extracorporeal partial and wedge resection of the small intestine were undertaken. Both lesions were diagnosed as typical GISTs of low risk. Laparoscopic surgery would be useful for examination and a minimally invasive approach to tumors of the small intestine, especially on cases with the possibility of multiple tumors. 相似文献

Osteoarthritis (OA) is a group of common, chronic, and painful inflammatory joint diseases. One important finding in OA patients is a remarkable decrease in the molecular weight of hyaluronic acid (HA) in the synovial fluid of affected joints. Therapeutic HA is available to patients in most parts of the world as a viscosupplementation product for the treatment of OA. Previous clinical reports show that high molecular weight HA (HMWHA) more effectively relieves pain than low molecular weight HA (LMWHA). However, the mechanism behind this finding remains unclear. In this study, we investigated whether a LMWHA (Low‐0.9 MDa) and two types of HMWHA (High‐1.9 MDa and 6 MDa) differentially affected chondroregulatory action. We tested this using ATDC5 cell, a murine chondrocytic cell line widely used in culture systems to study chondrogenic differentiation. We found that HMWHA, especially hylan G‐F 20 (High‐6 MDa), significantly induced aggrecan and proteoglycan accumulation, nodule formation, and mRNA expression of chondrogenic differentiation markers in a time‐ and dose‐dependent manner. In addition, we showed that HMWHA prevented TNF‐α induced inhibition of chondrogenic differentiation, with no effect on cell proliferation or viability. These results reveal that HMWHA significantly promotes chondrogenic differentiation of ATDC5 cells in vitro, and suggest that HMWHA plays a significant chondroregulatory role in vivo. © 2014 Orthopaedic Research Society. A 7-year-old boy with juvenile erythroleukemia is described, whose red cells demonstrated a high content of Hb F with fetal structure and yet contained carbonic anhydrase at adult red cell level. The findings seem to exemplify the occurrence of uncoordinated expression of fetal markers and consequently the incomplete reversion to fetal-type erythropoiesis in a hematologic malignancy. 相似文献
