Internal fixation of fractures of both bones forearm: Comparison of locked compression and limited contact dynamic compression plate

Background:

The locking compression plate (LCP) with combination holes is a newer device in fracture fixation. We undertook a study comparing the LCP with limited contact dynamic compression plate (LC-DCP) in the treatment of diaphyseal fractures of both bones of the forearm.

Materials and Methods:

This is a prospective comparative study, 36 patients (18 in each group) with fractures of both the forearm bones (72 fractures) were treated with one of the two devices. The average age of the patients was 30.5 years (range 16–60 years) with mean followup of 2.1 years (range 1.5–2.8 years). The patients were assessed for fracture union and function and complications and by Disabilities of the Arm, Shoulder and Hand (DASH) score for patient related outcome at the latest followup.

Results:

There was no significant difference in two groups with respect to the range of movements or grip strength. One case had delayed union (LC-DCP group) and another had synostosis (LCP group). Plate removal was done in four cases within the study period with no refracture till the presentation of this report.

Conclusion:

LC plating is an effective treatment option for fractures of both bones of forearm. The present study could not prove its superiority over LC-DCP.     

Referrals among cancer services organizations serving underserved cancer patients in an urban area

Significant racial, socioeconomic, and geographic disparities exist nationwide in cancer screenings, treatments, and outcomes. Differences in health and social service provision and utilization may contribute to or exacerbate these disparities. We evaluated the composition and structure of a referral network of organizations providing services to underserved cancer patients in an urban area in 2007. We observed a need for increased awareness building among provider organizations, broader geographic coverage among organizations, and increased utilization of tobacco cessation and financial assistance services.     

Attachment behavior and mother-child conversations as predictors of attachment representations in middle childhood: a longitudinal study

This study examines longitudinal links between mother-child conversations and attachment patterns in early childhood and later attachment representations. It also tests the role of conversations as mediators in the association between behavioral security and attachment representations. Mother-child conversations (snack-time) and attachment behaviors (Separation-Reunion procedure) were assessed for 83 5.5-year-olds while attachment representations (attachment narratives) were measured at 8.5 years of age. Results showed correspondence between attachment behaviors and representations for secure-confident, ambivalent-preoccupied, and disorganized/controlling-frightened groups. Affective quality of mother-child conversations predicted both child attachment behaviors and representations. Secure and confident children showed greater integration of affective information, ambivalent and preoccupied children more affect exaggeration, and disorganized/controlling and frightened children more chaotic conversations. Avoidant children tended to show more affect minimization in conversations. Finally, mother-child conversations centered on the sharing of emotions and thoughts mediated the relation between behavioral and representational attachment security, which underscores the importance of mother-child conversations in the development of attachment representations in childhood.
