The findings in the Wilson precordial leads and in the standard and Goldberger limb leads have been analyzed and correlated with the pathologic findings in fifty-two cases of coexistent infarction of the anterior and posterior walls of the left ventricle. Autopsy revealed a recent infarct involving the apical one-third or more of the anterior wall and continuing through the septum and around the tip of the left ventricle into the apical one-third or more of the posterior wall in twenty cases, and an old healed infarct of similar distribution in sixteen cases. The pathologic findings were the result of the confluence of independent anterior and posterior infarcts in at least three of these thirty-six cases, and were attributed to simultaneous anteroposterior infarction in the other thirty-three cases because of the anatomical continuity of the lesions and the uniformity in age. Five additional cases were observed with pathologic evidence of simultaneous anteroposterior infarction, complicated by a separate posterobasal lesion. The eleven remaining cases had anatomically separate anterior and posterior lesions.  相似文献

Introduction: Pheochromocytoma may present with ECG abnormalities as one of the few clues to the diagnosis.
Methods and Results: A 30-year-old woman presented with chest pain and a QTc of 525 ms. Four weeks later following a syncopal episode, her QTc was 660 ms. Short runs of monomorphic ventricular tachycardia were recorded. Investigations revealed an adrenal pheochromocytoma. Her QTc normalized after excision of the tumor. Comprehensive QT gene screening of KCNQ1, KCNH2, SCN5A, KCNE1, and KCNE2 was negative.
Conclusion: Pheochromocytoma may induce monomorphic VT and QT prolongation. The interaction of different catecholamines may have a compounding effect on cardiac repolarization.  相似文献

Clinical Efficacy and Safety of the New Cardioverter Defibrillator Systems   总被引:1,自引:0,他引:1  
Clinical efficacy and safety of two new third-generation implantable cardioverter defibrillators (ICD) were studied in 38 patients with ventricular tachycardia (VT) or fibrillation (VF). There were 31 patients with coronary disease, three patients with right ventricular dysplasia, one patient with dilated cardiomyopathy, and three patients with valvular disease. Twenty-four patients (group I) received an ICD with monophasic (Ventak PRx 1700, CPI) and 14 patients (group II) with biphasic shocks (Cadence V 100, Ventritex). Intraoperatively, the mean defibrillation threshold was significantly lower in group II than in group I, both in patients with induced VT (group I 11.0 ± 6.3 joules: group N 5.8 ± 1.3 joules) (P < O.01) and induced VF (group I 17.5 ± 4.6 joules; group II 9.6 ± 5.2 joules) (P < O.O1). During the mean follow-up of 12 ± 7 months four patients (11%) died. 865 arrhythmia events (AE) occurred and were terminated by ATP (671 VTs, 78%). Acceleration of VTs was observed in 28 AE (3%) and ATP was unable to interrupt 58 AE (7%). ICD shocks were delivered as a first therapy in 108 AE (13%).  相似文献

The influence of myocardial ischemia on defibrillation success was studied using two different lead orientations in halothane-anesthetized pigs. Ischemia was induced by ligating the left anterior descending artery in its distal third. Controls had loosely tied ligatures placed around the artery at the same site. Ventricular fibrillation was induced by electrical stimulation 30 minutes after coronary artery ligation. Defibrillation used a single truncated pulse of approximately 6 ms duration passed to either: a transvenous electrode catheter (Medtronic, 6880) with the cathode in the apex of the right ventricle and the anode in the superior vena cava-atrial junction region, or the cathode in the apex of the right ventricle and a mesh plaque on the epicardium of the basal lateral left ventricle as anode. Ten seconds after the onset of ventricular fibrillation, defibrillation was attempted with increasing incremental energies until defibrillation was achieved. Fibrillation episodes were repeated at 15-minute intervals until the minimum first shock was successful in defibrillating the animal (i.e., defibrillation threshold). The number of animals successfully defibrillated with a minimum energy above or below 30 J was not different between normal and ischemic animals for either electrode configuration (i.e., 3 out of 20 vs 1 out of 13 for the catheter and 5 out of 6 vs 6 out of 7 for the epicardial plaque, respectively). Also, the cumulative percent success as a function of defibrillation energy was similar in both the normal and ischemic groups. There was a significant reduction in the minimum energy necessary for defibrillation when passing current between the right ventricular apex and the left ventricular epicardial plaque. The present results indicate that, despite differences in lead orientations, acute ischemia in the anesthetized pig does not appear to influence defibrillation success.  相似文献

Background: Prevalence of patent foramen ovale (PFO) with detectable right‐to‐left shunt is higher in young adults with transient ischemic attack (TIA) and stroke compared to the general population. So far, published series included different occluder systems, various indications and regimens of postprocedural anticoagulation. In our experience, occluder systems may be associated with an increased prevalence of thrombus formation, which has also reported by other groups. The aim of the present study was to evaluate the follow‐up results after implantation of the Amplatzer® occluder in patients with PFO using a consistent anticoagulation regimen. Methods and Results: One‐hundred and fourteen patients with PFO (60 men; age: 47 ± 13 years) and ≥1 thromboembolic event were included. Other causes for embolism were excluded. PFO‐closure was successful in all patients. All patients were treated with aspirin (100 mg/day) and clopidogrel (75 mg/day) for 6 months. TEE was repeated at a mean of 10.3 months. Mean clinical follow‐up period was 18 ± 9 months. After a mean of 10 months, no patient had either a significant residual shunt nor a suspected thrombus formation on the occluder. During follow‐up, 5 patients suffered from neurological events (1 stroke, 2 TIAs, 2 epileptic seizures), though complete closure of the PFO was documented by TEE. One patient suffered from bleeding complications (upper GI‐bleeding). Conclusion: Percutaneous closure of PFO in symptomatic patients by Amplatzer® occluder represents an effective therapy with a low incidence of peri‐interventional complications and recurrent thromboembolism. The most effective antitachycardia pacing (ATP) mode is still a matter of debate. Randomized prospective testing of 3 different ATP modes was performed in B5 patients (pts) prior to and after cardioverter Defibrillator implantation (Ventak PHx 36 pts, Cadence V 100 29 pts). All 3 ATP modes included 4 stimulation attempts with 4–7 adaptive scanning burst pulses. Adaptive burst coupling interval was 75% in mode A, 81% in mode B and 69% in mode C. Autodecremental scanning within bursts was 8 msec in all, decremenial scanning between bursts was 8 msec in modes B and C. Each ATP mode had to be tested 3 times; all 3 ATP modes were randomly applied to each pt. During preoperative testing 91 of 133 VT episodes (68%) could be terminated by ATP. Termination was achieved in 68% with mode A, 68% with mode B and 73% with mode C, During predischarge testing, 251 VTs were induced and ATP was successful in 147 VTs (59%). Termination rate was 56% with mode A, 68% with mode B and 50% with mode C. During the mean follow-up of 12 months, 2301 arrhythmia episodes (AE) occurred. ATP was performed in 2097 AE (91%) and successful in 1920 AE (92%). Acceleration of VT occurred in 65 AE (3%) and unsuccessful ATP was observed in 112 AE (5%). It is concluded that ATP in general is highly effective in pts with sustained VT. None of the tested ATP modes, however, proved to be superior to the other.  相似文献

Background: Besides implantation of an implantable cardioverter-defibrillator (ICD), a proportion of patients with left ventricular (LV) dysfunction due to ischemic cardiomyopathy are potential candidates for surgical LV reconstruction (Dor procedure), which changes LV ejection fraction (LVEF) considerably. In these patients, LVEF as selection criterium for ICD implantation may be difficult. This study aimed to determine the value of LVEF as criterium for ICD implantation in heart failure patients undergoing surgical LV reconstruction.
Methods: Consecutive patients with end-stage heart failure who underwent ICD implantation and LV reconstruction were evaluated. During admission, two-dimensional (2D) echocardiography (LV volumes and LVEF) was performed before surgery and was repeated at 3 months after surgery. Over a median follow-up of 18 months, the incidence of ICD therapy was evaluated.
Results: The study population consisted of 37 patients (59 ± 11 years). At baseline, mean LVEF was 23 ± 5%. Mean left ventricular end-systolic volume (LVESV) and left ventricular end-diastolic volume (LVEDV) were 175 ± 73 mL and 225 ± 88 mL, respectively. At 3-month follow-up, mean LVEF was 41 ± 9% (P < 0.0001 vs. baseline), and mean LVESV and LVEDV were 108 ± 65 mL and 176 ± 73 mL, respectively (P < 0.0001 vs. baseline). During 18-month follow-up, 12 (32%) patients had ventricular arrhythmias, resulting in appropriate ICD therapy. No significant relations existed between baseline LVEF (P = 0.77), LVEF at 3-month follow-up (P = 0.34), change in LVEF from baseline to 3-month follow-up (P = 0.28), and the occurrence of ICD therapy during 18-month follow-up.
Conclusion: LVEF before and after surgical LV reconstruction is of limited use as criterium for ICD implantation in patients with end-stage heart failure.  相似文献
