Ambient air pollution and cardiovascular emergency department visits   总被引:3,自引:0,他引:3  
BACKGROUND: Despite evidence supporting an association between ambient air pollutants and cardiovascular disease (CVD), the roles of the physicochemical components of particulate matter (PM) and copollutants are not fully understood. This time-series study examined the relation between ambient air pollution and cardiovascular conditions using ambient air quality data and emergency department visit data in Atlanta, Georgia, from January 1, 1993, to August 31, 2000. METHODS: Outcome data on 4,407,535 emergency department visits were compiled from 31 hospitals in Atlanta. The air quality data included measurements of criteria pollutants for the entire study period, as well as detailed measurements of mass concentrations for the fine and coarse fractions of PM and several physical and chemical characteristics of PM for the final 25 months of the study. Emergency department visits for CVD and for cardiovascular subgroups were assessed in relation to daily measures of air pollutants using Poisson generalized linear models controlling for long-term temporal trends and meteorologic conditions with cubic splines. RESULTS: Using an a priori 3-day moving average in single-pollutant models, CVD visits were associated with NO2, CO, PM2.5, organic carbon, elemental carbon, and oxygenated hydrocarbons. Secondary analyses suggested that these associations tended to be strongest with same-day pollution levels. CONCLUSIONS: These findings provide evidence for an association between CVD visits and several correlated pollutants, including gases, PM2.5, and PM2.5 components.  相似文献

In understanding the brain's response to extensive practice and development of high-level, expert skill, a key question is whether the same brain structures remain involved throughout the different stages of learning and a form of adaptation occurs, or a new functional circuit is formed with some structures dropping off and others joining. After training subjects on a set of complex motor tasks (tying knots), we utilized fMRI to observe that in subjects who learned the task well new regional activity emerged in posterior medial structures, i.e. the posterior cingulate gyrus. Activation associated with weak learning of the knots involved areas that mediate visual spatial computations. Brain activity associated with no substantive learning indicated involvement of areas dedicated to the declarative aspects learning such as the anterior cingulate and prefrontal cortex. The new activation for the pattern of strong learning has alternate interpretations involving either retrieval during episodic memory or a shift toward non-executive cognitive control of the task. While these interpretations are not resolved, the study makes clear that single time-point images of motor skill can be misleading because the brain structures that implement action can change following practice.  相似文献

BACKGROUND AND PURPOSE: The incidence of blunt traumatic vertebral artery dissection/thrombosis varies widely in published trauma series and is associated with spinal trauma. The purpose of this study was to determine the frequency of traumatic vertebral artery thrombosis (VAT) in cervically injured patients by using routine MR angiography (MRA) and MR imaging and identify associations with the severity of neurologic injury. METHODS: A retrospective review of 1283 patients with nonpenetrating cervical spine fractures with or without an associated spinal cord injury (SCI) was performed. Imaging consisted of routine cervical MR imaging and 2D time-of-flight MRA of the neck. The cervical injury level, neurologic level of injury, and American Spinal Injury Association (ASIA) grade were recorded. RESULTS: In this study, 632 patients met the inclusion criteria, 83 (13%) of whom had VAT on the admission MR imaging/MRA. Fifty-nine percent (49/83) of VAT patients had an associated SCI. VAT was significantly more common in motor-complete patients (ASIA A and B, 20%) than in neurologically intact (ASIA E, 11%) cervical spine-injured patients (P = .019). VAT incidence was not significantly different between motor-incomplete (ASIA C and D, 10%) and neurologically intact (ASIA E, 11%) cervical spine-injured patients (P = .840). CONCLUSION: The absence of neurologic symptoms in a patient with cervical spine fracture does not preclude VAT. VAT associated with cervical spinal injury occurs with similar frequency in both neurologically intact (ASIA E) and motor-incomplete patients (ASIA C and D) but is significantly more common in motor-complete SCI (ASIA A and B).  相似文献

Six adult patients with growth hormone receptor deficiency (GHRD) (2 men, 4 women) with an identical defect in the growth hormone receptor (GHR) gene, were treated with recombinant human insulin-like growth factor I (IGF-I), 40 μgikg S.C. twice daily, for 7 days. Serum concentrations of IGF peptide and IGF binding protein-3 (IGFBP-3) were measured by specific radioimmunoassays; serum IGFBPs were also measured by Western ligand blotting. The size distribution of both IGF-I and IGF-II was measured in serum following size-exclusion fast-performance liquid chromatography. IGF-I treatment resulted in a normalization of serum IGF-I levels on days 1–7 of treatment and a decrease in serum IGF-II levels. The fall in IGF-II levels and the simultaneous rise in IGF-I levels, however, resulted in an unchanged total serum IGF level. The low IGFBP-3 values did not significantly change during treatment, whereas there was a slight increase in IGFBP-2 levels. Preliminary analysis of size-fractionated sera suggested an increase in IGF-I levels in the 40 and 150 kDa regions at the expense of IGF-II levels. The results suggest that despite the failure of IGF-I treatment to increase IGFBPs significantly, serum IGFBP concentrations were sufficient to maintain normal levels of IGF-I. 0 Laron syndrome, growth hormone receptor deficiency, insulin-like growth factors, insulin-like growth factor binding protein  相似文献
