In the treatment of gastric cancer R0 surgical resection is the only hope for cure. Unfortunately most patients are first seen when they are in an advanced stage, when the possibility of R0 resection is very poor. In these cases administration of other therapies is justified. In the last decade neoadjuvant combination chemotherapy had been introduced with promising results. ECF neoadjuvant chemotherapy has 60% response rate in irresectable cases. We report about a patient with locally advanced gastric cancer treated with neoadjuvant ECF chemotherapy. The patient reacted with complete response, so following chemotherapy R0 resection could be carried out. We describe different therapies used for patients with locally advanced gastric cancer. Neoadjuvant chemotherapy is a recommended therapeutic modality in locally advanced gastric cancer, because it may have the effect of irresectable disease becoming resectable.  相似文献

Damage to interstitial cells of Cajal (ICC), pacemakers, and mediators of neuromuscular neurotransmission in the gastrointestinal tract contributes to the pathogenesis of diabetic gastroenteropathy in both patients and animal models. ICC depletion in diabetes may result from chronic hyperglycemia or lost/ineffective insulin signaling. Because independent control of insulin and glucose concentrations is difficult in chronic in vivo studies, we used long-term organotypic cultures to address this problem. Murine gastric muscles were cultured in normoglycemic or hyperglycemic basal media with or without insulin or IGF-I for 1-3 months, the time required for gastroparesis and ICC damage to develop in diabetic mice. ICC were assessed by c-Kit immunohistochemistry and quantitative analysis of c-kit expression. Electrical pacemaking was studied by intracellular recording of slow waves. ICC survived for at least 34 days in unsupplemented normoglycemic media, but their networks, c-kit expression, and slow waves were profoundly reduced after 68 days. These changes could be entirely prevented by insulin or IGF-I supplementation. ICC networks were completely resistant to hyperglycemia for at least 72 days. Thus, hyperglycemia is unlikely to be responsible for the diabetes-associated depletion of ICC. In contrast, maintenance of ICC requires insulin or IGF-I, which are reduced or ineffective in diabetes.  相似文献

We studied the fabrication of functionally graded Al2O3–CeO2-stabilized-ZrO2 (CTZ) ceramics by spark plasma sintering. The ceramic composite exhibits a gradual change in terms of composition and porosity in the axial direction. The composition gradient was created by layering starting powders with different Al2O3 to CTZ ratios, whereas the porosity gradient was established with a large temperature difference, which was induced by an asymmetric graphite tool configuration during sintering. SEM investigations confirmed the development of a porosity gradient from the top toward the bottom side of the Al2O3–CTZ ceramic and the relative pore volume distributed in a wide range from 0.02 to 100 µm for the samples sintered in asymmetric configuration (ASY), while for the reference samples (STD), the size of pores was limited in the nanometer scale. The microhardness test exhibited a gradual change along the axis of the ASY samples, reaching 10 GPa difference between the two opposite sides of the Al2O3–CTZ ceramics without any sign of delamination or cracks between the layers. The flexural strength of the samples for both series showed an increasing tendency with higher sintering temperatures. However, the ASY samples achieved higher strength due to their lower total porosity and the newly formed elongated CeAl11O18 particles.  相似文献

Background: Patients with bullous pemphigoid (BP), mucous membrane pemphigoid (MMP) and pemphigoid gestationis (PG) have IgG antibodies against BP180 and BP230, components of the hemidesmosomes. Patients with linear IgA bullous dermatosis (LABD) have IgA autoantibodies against a 97/120‐kDa protein which is highly homologous to a shedded fragment of the BP180‐ectodomain. Objectives: The aim of our study was to determine the incidence of IgA autoantibodies directed against BP180/BP230 in the pemphigoids and LABD and to determine the antigenic regions that are targeted by IgA autoantibodies. Methods: Utilizing baculovirus‐expressed recombinant BP180 and BP230 proteins, we performed immunoblot analyses for IgA reactivity of sera from patients with BP (n = 30), MMP (n = 10), PG (n = 6), LABD (n = 6) and from control patients with non‐related pruritic dermatoses (n = 8). Results: IgA reactivity against BP180 and/or BP230 was detected in 19/30 of the BP, in 7/10 of the MMP, in 6/6 of the LABD and in 3/6 of the PG sera, respectively, but not in the control group. In all subgroups, the major antigenic site recognized by IgA antibodies was located within the NH2‐terminus of the BP180‐ectodomain, but only a minority of the sera showed also IgA reactivity against the BP180‐NC16a‐domain. IgA reactivity against the central domain of BP180 was more frequently seen than against its COOH‐terminus. IgA against the COOH‐ and NH2‐terminus of BP230, respectively, was detected in 6/30 of the BP, 1/10 of the MMP, 1/6 of the LABD and 0/8 control sera. Conclusion: IgA reactivity against BP180 and/or BP230 is a common finding in the pemphigoids.  相似文献

In this work we report on a unique and ancient type of eye, in which the lower surface of the upper calcite lens units possessed an enigmatic central bulge making the dioptric apparatus similar to a bifocal lens. This eye belonged to the trilobite Dalmanitina socialis, which became extinct several hundred million years ago. As far as we know, image formation by bifocal lenses of this kind did/does not occur in any other ancient or modern animal visual system. We suggest that the function of these bifocal lenses may be to enable the trilobite to see simultaneously both very near (e.g. floating food particles and tiny preys) and far (e.g. sea floor, conspecifics, or approaching enemies) in the optical environment through the central and peripheral lens region, respectively. This was the only reasonable function we could find to explain the puzzling lens shape. We admit that it is not clear whether bifocality was necessary for the animal studied. We show that the misleading and accidental resemblance of an erroneous correcting lens surface (designed by René DesCartes in 1637 [DesCartes, R. (1637). Oeuvres de DesCartes. La Géometrie. Livre 2. pp. 134. J. Maire, Leyden] to the correcting interface in the compound Dalmanitina lens may be the reason why the earlier students of the Dalmanitina lens did not recognize its possible bifocality.  相似文献

Jejunal biopsy and hydrogen breath test were performed in 57 children, 34 having coeliac disease and 23 with other forms of malabsorption. In children affected by coeliac disease there was a gradually increasing incidence of positive findings with the H2 breath test as villous damage progressed. In the group of subtotal villous atrophy age dependence was also observed, the majority of positive results occurring below three years of age. In the non-coeliac group the most frequent cause of the positive finding was Giardia lamblia infestation. Among 27 cases with lactose malabsorption confirmed by a positive hydrogen breath test only 11 had diarrhoea. The test proved to be useful in differentiating between the contaminated intestine syndrome and malabsorption due to reduced absorptive surface.  相似文献

Colorectal manifestation of von Recklinghausen's disease is a rare entity. The typical dumbbell shape of the lesion has been described previously, but only from a resected cecal specimen.
