The underlying mechanism of the water-immersion skin wrinkling test, which is used as a test of sympathetic nerve function, remains elusive. We investigated changes of blood circulation in the hand occurring with water-immersion wrinkling by measuring the velocity of ulnar and digital artery blood flow, and of digit skin blood flow, in healthy subjects before and during wrinkling. Wrinkling was accompanied by significant reduction in blood flow velocity in all vessels, with a maximum in digital vessels. Our data show that water-immersion wrinkling is a function of digit pulp vasoconstriction. This test of sympathetic function can now be quantified using parameters of blood flow velocity, enabling its more widespread and accurate use.  相似文献

Little attention has been paid to small-fiber dysfunction in carpal tunnel syndrome (CTS) although its symptoms are common. This study investigates vasomotor dysfunction, which is controlled by small nerve fibers, in patients with CTS compared with control subjects. Vasomotor function was quantified by measuring, with laser Doppler velocimetry, skin vasoconstriction induced by a eutectic mixture of local anesthetic (EMLA) cream over digit tips 3, 4, and 5. Hands with CTS (n = 32) compared with controls (n = 19) demonstrated significantly reduced vasoconstriction in digits 3 and 4, but not digit 5. A blood flow ratio (digit 3/5) of less than 0.73 identified CTS in 69% with 68% specificity. Testing for vasomotor dysfunction in CTS allows for more comprehensive neurophysiological testing, which is heavily biased towards large nerve fibers.  相似文献

We have recently shown that syncoilin interacts with desmin in skeletal muscle and has a role in attaching and organising desmin filaments to the Z-lines. We have analysed patients with desmin accumulation and have found that syncoilin is both upregulated at the sarcolemma and aggregates with desmin indicating the presence of two distinct protein populations. Additional dystrophin-associated protein complex components also accumulate. The striking finding was that alpha-dystrobrevin-1 and neuronal nitric oxide synthase (nNOS) are almost completely lost from the membrane of these patients indicating that the myopathy may result from both the abnormal accumulation of proteins and an increase in ischaemic injury due to the loss of nNOS. We speculate that the loss of alpha-dystrobrevin from the membrane, and subsequent loss of nNOS, is due to the alpha-dystrobrevin-syncoilin-desmin interaction.  相似文献

A study conducted at Vinfen, a large Massachusetts-based provider of psychosocial rehabilitation services, sheds new light on staff tenure and its relationship to participant satisfaction. The researchers surveyed participant satisfaction in forty-six residential sites serving people with psychiatric disabilities. The study included client and staff demographics. The researchers correlated average participant satisfaction score and average staff tenure by program. The results of the study did not support the widely held belief that there is a relationship between staff tenure and participant satisfaction in residential programs.  相似文献

Schizophrenic patients present a higher risk for the development of hyperglycemic disorder and the use of antipsychotic drugs seems to increase the risk of diabetes mellitus. The present review concerns the relation between atypical antipsychotic drugs and the risk of developing diabetes mellitus. A Medline and Webofscience search was performed by using the terms "Hyperglycemia", "Diabetes Mellitus" and "Antipsychotic Agents", to identify original papers and reviews published between 1997 and september 2002. It is concluded that there is a higher risk of glycemic disorders in the population of patients treated by antipsychotic drugs. Dietetic measures and attention to risk factors should be taken into account during the treatment of psychotic patients.  相似文献

Mibefradil, a calcium channel blocker, was removed from the market because of adverse drug interactions with coadministered CYP3A4 substrates. This study examined the effect of mibefradil on the activity of hepatic and intestinal CYP3A4 in vivo, employing the erythromycin breath test (EBT) and oral midazolam pharmacokinetics. This was a two-period, single-blind, placebo-controlled crossover study in which 8 male volunteers were randomized to the order of receiving placebo and a single 100-mg oral dose of mibefradil. Oral midazolam was coadministered with intravenous [14C N-methyl] erythromycin 1 hour after mibefradil/placebo administration. The EBT was performed 20 minutes following erythromycin administration. Blood and urine were collected during the 36 hours following probe drug administration for analysis of midazolam pharmacokinetics. Coadministration of mibefradil increased the Cmax of midazolam 3-fold, the AUC 8- to 9-fold, and the t1/2 4-fold. Mibefradil coadministration decreased the amount of exhaled 14CO2 in 6 of 8 subjects, with a mean decrease of 25%. It was concluded that a single oral dose of mibefradil significantly inhibits CYP3A4 in intestine and liver. These data support that adverse drug interactions involving mibefradil reflect inhibition of CYP3A4 in intestine and liver. Also, they suggest that the EBT, while a valid probe of in vivo hepatic CYP3A4 activity, is a single time point measurement and may be less sensitive than oral midazolam pharmacokinetics in detecting CYP3A4 inhibition.  相似文献
