 The present study investigated the pathogenesis and the time course of kidney injury in experimental IgA nephropathy. In order to determine an appropriate period in the course of experimental IgA nephropathy to study renal injury and repair, we examined proteinuria and IgA deposition in the renal mesangium after 4, 8, and 16 weeks of mucosal challenge by bovine gamma globulins (BGG) provided in the drinking water. The hallmark of IgA deposition in the mesangium was present after 4 weeks and 8 weeks of BGG inoculation, but by 16 weeks, the mesangial IgA deposition had resolved. In addition, we confirmed our previous report on the beneficial effects of α-tocopherol in reducing proteinuria in IgA nephropathy at 8 weeks, and extended this observation to investigate the effects of dietary supplementation of α-tocopherol at both 4 weeks and 16 weeks. Proteinuria resolved spontaneously at 16 weeks. There is oxidative stress, as suggested by the elevation in plasma and renal malondialdehyde content, and increased fibrogenic cytokine message, as suggested by elevated transforming growth factor β1 mRNA. These increases were clearly blunted by α-tocopherol at both 4 weeks and 8 weeks. Treatment with α-tocopherol was associated with a significant reduction in the severity of proteinuria. Thus, our data suggest that the period between 4 and 8 weeks of BGG vaccination could be relevant in designing an appropriate model to study the molecular biology of the pathogenesis of renal injury and the effects of treatment. PURPOSE: To evaluate the role of factors that may affect the level of plasma ascorbic acid (AA), including age, body weight, physical activity, minor illness and the impact of prior depletion and repletion. METHODS: After one month of stabilization on 60 mg vitamin C/day, subjects underwent two complete depletion-repletion cycles (one cycle=one month of vitamin C depletion with nine mg/day, followed by one month of repletion with 117 mg per day). Subjects (68 men, ages 30 to 59 years) did not smoke or drink alcohol during the study. All food was provided by the study. RESULTS: There was extreme individual variability in the plasma AA level achieved on an identical repletion dose: after four weeks at 117 mg/day of vitamin C, AA ranged from 26.8 micromol/L to 85.8 micromol/L. Body weight was inversely associated with plasma AA attained (p<0.0001). Regression analysis indicated that, compared to a 130-lb man, a 200-lb man reached 10 micromol/L lower AA after the first repletion and 18 micromol/L lower AA after the second repletion. One-third of the subjects did not reach a plasma plateau after the first repletion. Prior depletion and apparent repletion also had a major impact, and only 10% of subjects reached the same plasma AA on the second repletion as on the first repletion. CONCLUSIONS: Plasma AA attained on a given dose depends on body weight (or dose per kg of body weight) and on whether or not any prior depletions had been repleted adequately. The possibility of using a new synthetic material mersilene for scleroplastic operations has been evaluated in experiments and clinical trials were carried out in patients with progressive myopia subjected to sclera fortification. Morphological studies showed the formation of a connective tissue capsule round mersilene; new collagen fibers grow through its cellular structures, thus fortifying the strength of the sclera-graft complex. Clinical results indicate that sclera fortification by a mersilene graft arrested the progress of myopia in almost all (98%) patients by the end of the first year postoperation; by the end of the second year, myopic refraction stabilized in 92% patients. Hence, mersilene transplants are recommended for scleroplasty in progressive myopia.  相似文献

PURPOSE: Some surgeons are wary of using alcohol-preserved sclera for allografts because they fear a toxic effect on surrounding tissue after placement. We set out to determine the amount of ethanol remaining in scleral allograft material after storage in 95% ethanol. METHODS: Sixty half scleras from 30 donors were preserved in 95% ethanol for an average of 31+/-14 days (range, 11 to 50 days). Rehydration was performed by soaking each half sclera in 4 ounces of balanced salt solution. Half scleras were randomly assigned to six groups of 10 each. Assays for ethanol were performed on the following groups: no balanced salt solution soak and balanced salt solution soak for 10 minutes, 20 minutes, 30 minutes, 40 minutes, and 50 minutes. Ethanol assay was performed by Headspace Gas Space Chromatography at ChemaTox Laboratory, Inc, Boulder, Colorado. RESULTS: The 10 half scleras without balanced salt solution soak had a mean ( SD) ethanol level of 175+/-14.1 mg per g of sclera. After 10 minutes of balanced salt solution s oak, the level decreased to 7.57+/-1.56 mg per g, then 3.77+/-3.02 mg per g at 20 minutes, 1.59+/-0.61 mg per g at 30 minutes, 1.07+/-0.30 mg per g at 40 minutes, and 0.96+/-0.26 mg per g at 50 minutes. Approximately 96% of the ethanol is leeched out of the half sclera by 10 minutes and 98% by 20 minutes. CONCLUSIONS: For sclera preserved in 95% ethanol, soaking in balanced salt solution for 20 minutes or longer leeches approximately 98% of the ethanol from the preserved donor sclera.  相似文献

The use of institutional placement in juvenile justice, mental health, and child welfare continues to be widespread. Yet there is little information about the decision-making processes connected with the choice of this alternative for adolescents. Focus groups were held with administrators, staff, and youth in a variety of institutional placements. The discussions of these groups were qualitatively analyzed for content regarding the factors that motivated reliance on placement in institutional care and the effects of policies to regulate this practice. Agreement about the diversity of adolescents in institutions, the difficulties of (but necessity for) preventive interventions with families, and possible causes for reliance on institutional care emerged. Differences about the risks and benefits of institutional placement were noted.  相似文献
