鼻咽癌组织中微血管的分布

目的：观察鼻咽癌组织中微血管的分布及存在状况,探讨其与鼻咽癌的临床病理联系。方法：采用免疫组织化学Ｓ－Ｐ法,检测血栓调节蛋白在鼻咽癌血管内皮细胞的表达,进行微血管的定性半定量研究。结果：鼻咽癌组织中微血管的分布不均匀。极性消失。血管呈分支芽状弥漫分布于癌组织中,或呈条索状围绕癌细胞团。微血管的数目与癌细胞的增殖指数屯预后正相关。结论：鼻咽癌组织中微血管数与患者的生存期有关。     

腹膜透析治疗小儿先天性心脏病术后急性肾功能衰竭

目的：探讨腹膜透析（ＰＤ）对小儿先天性心脏病（先心病）术后急性肾功能衰竭（ＡＲＦ）的治疗效果。方法：对３６例先心病术后ＡＲＦ行ＰＤ治疗患儿的临床资料作回顾性调查。结果：全组经ＰＤ治疗后的病死率为３０．６％,其中单纯ＡＲＦ的病死率１２．５％,显著低于合并其他系统器官衰竭组的４５％（Ｐ〈０．０５）。ＰＤ３～３０ｄ内肾功能恢复,其中１～２ｄ内血Ｋ＾＋恢复正常,２～５ｄ内血碳酸氢根（ＨＣＯ３＾－）恢复正常,４～６ｄ内血尿素氮下降４９．２％,血肌酐下降４２．６％。结论：对小儿先心病术后ＡＲＦ及早进行ＰＤ具有较好的治疗效果。     

慢径路消融改良房室结术中出现的交界性心律之特征

目的：分析房室结慢径路消融术中出现的一过性交界性心律（ＪＲ）的特征。方法：２１例（男６例,女１５例）进行房室结改良术的房室结折返性心动过速患者,术中详细记录释放射频的次数、能量、持续时间、以及出现一过性交界性心律的时间、频率及数目,比较释放射频有效时／无效时出现的一过性交界性心律之特征有无显著性差异。结果：共进行１０６次射频电流释放,其中２１次有效（Ａ组）,８５次无效（Ｂ组）。Ａ组有１９次出现ＪＲ     

安胃降逆饮对大鼠胃功能的影响

目的 观察安胃降逆饮对胃功能的影响,分析其镇吐作用机制。方法 采用大鼠离体胃条法测定胃条张力,小鼠粉红排空试验法测定胃排空量和０．１ｍｏｌ／Ｌ ＮａＯＨ滴定法测定胃酸含量。结果安胃降逆饮对大鼠胃肌物基础张力和乙酰胆碱引起的痉挛性收缩有明显抑制作用（Ｐ〈０．０１）,对小鼠胃排空,大鼠胃液量,胃液酸度及总酸度无明显影响（Ｐ〉０．０５）。结论 安胃降逆饮的镇吐作用至少部分是通过抑制胃肠平滑肌蠕动而发挥作

四类新药盐酸格拉司琼注射液预防抗肿瘤药物所致呕吐的Ⅱ期临床观察

目的在肿瘤的治疗中化疗药物所引起的恶心、呕吐是极为常见的,通过对盐酸格拉司琼注射液预防肿瘤化疗所致呕吐的Ⅱ期临床研究,探讨该药的止吐疗效及毒性反应.方法对31例病人的观察,采用自身对照新药盐酸格拉司琼与恩丹西酮预防抗肿瘤药物(DDP及蒽环类药物)所致呕吐进行比较.结果格拉司琼对强致吐剂顺铂所引起的呕吐其总的缓解率＞90%,恩丹西酮对顺铂引起的呕吐其缓解率＞85%.两止吐药毒性反应主要为便秘,恩丹西酮致便秘发生率高达41%,而格拉司琼所致便秘发生率为16%.结论两药预防呕吐的疗效相当,但格拉司琼的副反应明显低于恩丹西酮     

THE OPERATIVE TREATMENT OF SPINAL FRACTURE-DISLOCATION WITHOUT NEUROLOGIC DEFICITS

Objective. To evaluate the results of operative treatment of spinal fracture-dislocation without neumlogic deficits. Metods. Eighteen patients with spinal fracture-dislocation were neurologically intact at the time of injury, and all were treated operatively. The fracture sites were:8 cases in cervical spine, 3 cases in thoracic spine, and 7 cases in lumbar spine. Eight patients with cervical injuries had variant degrees of forward slide and kyphotic deformity. Of the 10 thoracic and lumbar fractures, one had lateral dislocation, 4 cases with kyphotic deformities, 5 cases withspinal canal compromise averaged 50% (ranging from 40% to 70% ).Results. The aveiage period of follow-up was 4.4 years with a range of 11 months to 13 years. All the patients returned to full-time work. No patient developed neurologic deterioration. Kyphotic deformity was corrected in the 4 cases, and no progressive kyphosis was noted. There was no operation-related complication. The averaged post-opera-five hospitalization time was 13 days. Conclusions. Despite the rare incidence of spinal fracture-dislocation without neurologic deficits, we suggested that kind of fracture be considered unstable fracture because of its potential risk of delayed neurologic deterioration and kyphotic deformity, and be treated operatively to restore the sagittal alignment and the stability of the spine.

Reconstruction of the two-dimensional distribution of p53 positive staining patches in sun-exposed morphologically normal skin

1,320 serial sections were microscopically examined from 11 excisions of human skin with a variety of neoplastic lesions. In total about 1 cm(2) of morphologically normal epidermis was scrutinized. The emphasis was on the topography of epidermal 'p53 patches' of morphologically normal keratinocytes apparently driven into clonal expansion by a mutation of the p53 gene or an analogous mechanism causing constitutive nuclear accumulation of p53. The p53 patches were only rarely in contact with dysplasia or carcinoma in situ supporting previous somatic genetic evidence that patches are not common precursors of squamous cell neoplasia. The size of the p53 patches varied from 0.03 to at least 12 mm(2). About 34% of epidermis was covered by p53 patches deduced to be originally derived from an average of 40 cells per cm(2) by clonal expansion. This implies that a large number of skin cells are at risk of contracting further mutations in the p53 gene and other parts of the genome, if the 'guardian of the genome hypothesis' of the p53 gene is correct and exposure to sun-light continues. This finding together with calculations based on number of basal cells/mm(2) skin area suggest that prevalence of mutations which apparently drive stem cells into clonal growth is about 1:15,000 stem cells in 70-80 years old Caucasian sun-exposed skin at latitude 60 degrees North.
