静脉药物配置中心的建立在临床中的应用

静脉输液药物集中配置、管理是我国医院管理的一项新举措.长期以来,临床静脉输液中的加药工作一直是由护士在治疗室独立完成,这种方式存在着很多不足之处.在这种环境污染下,职业健康安全管理越来越受到各国政府的重视.为了提高服务质量,预防和控制可能存在的环境污染及职业健康安全风险,国家颁布 GB/T24001～ 1996<环境管理体系规范和使用指南>, GB/T28001-2001<职业健康安全管理体系规范>标准,引进国外的先进管理模式,此项工作由药师和护士共同完成,以期达到合理用药减少药物的流失和浪费,科学配置、降低输液反应.静脉药物配置中心把静脉药物配置从普通环境的治疗室转为在具有洁净条件的配置中心进行集中管理配置,保证配置出来的药品安全无菌、有效.需要输液的患者在舒适、整洁和安静的环境下进行输液,减少临床护理工作量的目的,把更多的时间还给病人.这为顺利实施静脉配置中心的项目提供了保障,确保了输液中心正常运行.     

Training of interviewers in the utilization of standardized questionnaires in psychiatry: studies realized with the Present State Examination (PSE)]

Interview questionnaires offer more validity than self-administered format in exploring psychopathological or psychosocial phenomena of interest in psychiatric research. If used, special care needs to be paid to interviewers' training and ensuring that they maintain their reliability. No widespread training standards exist and each schedule may carry its own procedure. Our aims are to indicate how we trained interviewers with the French version of the Present State Examination (Wing, Cooper and Sartorius, 1974) and how we checked and kept acceptable interraters reliability during one study. We will provide data on the interraters reliability during the training and the study, as well as the test-retest reliability. These results will be used to support some guidelines when using this sort of psychiatric research questionnaires in order to ensure comparability both within the study and between studies.

饲料中添加钴、维生素B12对羔羊生长的影响

选用产期接近的3月龄小尾寒羊30只,随机分为二组(试验Ⅰ、试验Ⅱ),每组15只。试验Ⅰ饲喂本场加工的羔羊精饲料,试验Ⅱ饲喂本场加工的羔羊精饲料基础上,每公斤饲料中分别添加钴0.4mg和维生素B120.30mg,试验结果表明:试验Ⅰ与试验Ⅱ日增重相比差异极显著(P<0.01)。     

Predictors of dietary intake in a functionally dependent elderly population in the community.

OBJECTIVES. The aim of this study was to describe dietary intake and identify predictors of energy and protein intake in a group of high-risk elderly people. METHODS. All elderly persons receiving publicly financed home care services in the area of Sherbrooke, Quebec, Canada, were eligible. Subjects (n = 145) 60 to 94 years of age from three home care programs were interviewed to measure sociodemographic, health, and food-related behavior variables. Three nonconsecutive 24-hour recalls were used to describe usual dietary intake. Independent predictors of energy and protein intake were derived from multiple regression analyses. RESULTS. Very low mean energy intakes were observed in this functionally dependent population. More than 50% of the study subjects did not meet the recommended levels of daily protein intake (0.8 g/kg body weight). Significant independent determinants of intake were burden of disease, stress, poor appetite, and vision. CONCLUSIONS. Results suggest that community-living elderly people with loss of autonomy may have more nutritional problems than healthy elderly individuals. Surveillance of predictors of dietary intake may enable early detection and prevention of nutritional deficits.     

发展友情，暂缓爱情——从性心理角度看青少年的异性交往

处于青春期的青少年男女普遍进入一种试图发现、选择和试探自己的性心理需求对象的“寻梦”状态。在很长的一段时间里,“寻梦”双方的接触和亲近,具有未专一、不稳定、易更换的特点。这说明,交异性朋友不一定就产生爱情,而是泛指和不同性别的朋友来往,发展普通的友谊关系。从发展心理学角度看,交异性朋友是必要的和有益的,但应该受到理性约束。在校学生应该避免过早恋爱。     

汉语失语症计算机辅助流利性检测的研究

目的研究基于计算机模糊智能识别运算的语言障碍诊治仪ZM2．1（简称：语言障碍ZM2．1）对失语流利性检测的效度与信度。方法采用ABC法和语言障碍ZM2．1两种方法分别对58例失语症患者进行流利性检测，两种方法检测间隔时间在3d之内。结果ABC法流利性内部一致性检验，整体克朗巴赫α系数（Cronbach α）=0．8732；语言障碍ZM2．1平均语速和ABC法流利性总分有正相关性趋势，Spearman系数为0．721（P〈0．01）；语言障碍ZM2．1检测流利性的准确度为81％（以XABC法为标准）；测得两者流利性结果一致性良好（P〈0．01）。结论语言障碍ZM2．1在评价失语症流利性方面具有良好的信度和效度，可用于临床进行失语症的流利性检测。     

原发性小肠肿瘤诊断治疗的临床分析(附58例报告)

目的探讨原发性小肠肿瘤临床特型、病理分型及诊治经验。方法回顾性分析1993年10月。2003年10月收治的有完整资料的58例原发性小肠肿瘤病例。结果本组包括13例(22．4％)良性肿瘤，45例(77．6％)。小肠良性肿瘤均行局部肠段切除，除2例死于其它疾患外，余11例健在；小肠恶性肿瘤患者中，院内死亡6例，仅11例能行根治性肠切除，37例行化疗(总有效率83．4％)，随访33例(随访率89．5％)，存活5年以上者14例(5年生存率42．4％)，多为T细胞性非何杰金淋巴瘤。结论原发性小肠肿瘤恶性所占比例较高，缺乏特征性的临床表现及有效的诊断手段，易致长期延误诊治，预后甚差；对原因不明的腹痛、消化道出血及定位不明的腹部包块患者及早行剖腹探查是避免小肠肿瘤长期误诊、改善患者预后的可靠手段。     

An open-label efficacy pilot study with pimecrolimus cream 1% in adults with facial seborrhoeic dermatitis infected with HIV

BACKGROUND: Seborrhoeic dermatitis (SD) is a common dermatosis in human immunodeficiency virus (HIV)-positive patients, many of whom do not respond satisfactorily to conventional topical treatments such as corticosteroids and antifungals. OBJECTIVE: A pilot study to investigate the efficacy and tolerability of pimecrolimus cream 1% in HIV-positive patients with facial SD. METHODS: In a single-centre study, 21 HIV-infected patients with mild to severe SD were treated twice daily with pimecrolimus cream 1% for 14 days. Thereafter, treatment was discontinued and patients followed up for 5 weeks. Skin involvement at baseline and on days 7, 14, 21, 35 and 49 was assessed using a four-point clinical score and digital photography. MAIN OUTCOME MEASURES: Efficacy and safety of pimecrolimus cream 1% treatment and incidence of relapse in the follow-up phase. Results Marked improvement was seen in clinical parameters at day 7, with >or= 90% patients clear of symptoms at day 14. Relapse was observed at day 35 but signs were milder than at baseline. All patients responded to therapy, despite their immunological status. Pimecrolimus did not alter CD4(+) and CD8(+) T-cell counts or viral load during the treatment period. CONCLUSION: Pimecrolimus cream represents a new, effective therapeutic option for facial SD in HIV patients.
