目的探讨慢性多发性肌炎的发病机制、临床和病理特征。方法回顾性分析95例慢性多发性肌炎患者临床表现、肌酶学和肌电图检查结果,总结肌肉病理学特征。结果慢性多发性肌炎以四肢近端肌无力、肌萎缩为主要表现,血清酶谱轻-中度增高,肌电图以肌源性损害为主,病理改变为灶性坏死、炎性细胞浸润与再生肌纤维共存。结论临床特点结合病理学检查有助于慢性多发性肌炎的诊断,多数患者激素治疗有效。 相似文献

基于现代管理理论和医院的实际情况,设计了一种以岗位和业绩为导向的医院薪酬制度。该设计的主要特点是:①薪酬结构以岗位工资和绩效工资为主体,同时兼顾稳定和逐步过渡原则,原档案工资的固定部分(约占档案工资的60%)保留。②岗位工资采用全院统一的等级薪酬结构,等级的评定基于全院统一的指标体系和权重,岗位工资每年随绩效考核结果在工资带宽中升降;③绩效工资与岗位等级和绩效考核紧密挂钩,考核的依据是全院统一模式的岗位绩效合同;④整个薪酬设计建立在岗位调查、岗位评价和岗位说明书等的科学规范的工作基础之上。 相似文献

目的:探讨岛状皮瓣静脉淤血再通后对全身多脏器的影响。方法:按静脉淤血时间的不同将大鼠分为4组。观察耳部微循环的改变,测量术后肿瘤坏死因子α(TNFα),白细胞介素10(IL-10)的动态变化,观察心,肺,肝,肾,小肠及耳部血管等组织结构及中性粒细胞浸润数目。结果:皮瓣原位缝合组及静脉淤血2h组,耳部微循环、TNFα、IL-10浓度基本保持不变,各脏器结构改变较轻,中性粒细胞浸润数目少。静脉淤血6、10h组,微循环,肺,小肠,血管则有明显组织学改变,大量中性粒细胞浸润其中,但心,肝,肾组织学改变较轻。TNFα浓度再灌注1h达到高峰,其后逐渐下降,IL-10浓度3h达到最低,然后逐渐上升。结论:皮瓣静脉淤血再通后可造成肺、小肠及血管器官损伤,静脉淤血时间越长,再通后则损伤程度越重。全身微循环的改变,中性粒细胞在肺、小肠中的浸润,与血管内皮细胞的粘附及细胞因子TNFα与IL-10的浓度失衡是重要的操作原因。 相似文献

Semantic verbal fluency tasks are commonly used in neuropsychological assessment. Investigations of the influence of level of literacy have not yielded consistent results in the literature. This prompted us to investigate the ecological relevance of task specifics, in particular, the choice of semantic criteria used. Two groups of literate and illiterate subjects were compared on two verbal fluency tasks using different semantic criteria. The performance on a food criterion (supermarket fluency task), considered more ecologically relevant for the two literacy groups, and an animal criterion (animal fluency task) were compared. The data were analysed using both quantitative and qualitative measures. The quantitative analysis indicated that the two literacy groups performed equally well on the supermarket fluency task. In contrast, results differed significantly during the animal fluency task. The qualitative analyses indicated differences between groups related to the strategies used, especially with respect to the animal fluency task. The overall results suggest that there is not a substantial difference between literate and illiterate subjects related to the fundamental workings of semantic memory. However, there is indication that the content of semantic memory reflects differences in shared cultural background--in other words, formal education--, as indicated by the significant interaction between level of literacy and semantic criterion. 相似文献

The authors undertook comparative study of the Na, K-ATPase activity in the red cells, ghost corpuscles, and in the cortex and medulla of kidneys of rats with experimental glomerulonephritis (GN) induced by injection of nephrotoxic serum (NTS), during an acute course of the process (1 and 2 weeks after NTS injection) and in chronic affection of the kidneys (10 weeks after NTS injection). The activity of the enzyme both in the red cells and in the ghost corpuscles was reduced significantly, by 28 and 22%, respectively, in the acute period of the disease (one week after NTS injection) but was restored to normal values in the period of chronic affection of the kidneys. The activity of the enzyme did not change in the cortex during the whole period of the study, but in the medulla it diminished in the acute period of the disease and remained reduced (by 27% on the average) during the chronic phase. It is concluded that reduction of the enzyme activity in the studied objects was not caused by the presence of endogenous inhibitors of the enzyme. It is suggested that reduction of Na, K-ATPase activity in the renal medulla may be one of the factors of increase of Na and water excretion in the urine in the chronic phase of GN. 相似文献

Plasma autoantibodies (aAbs) against the oxidized DNA base derivative 5-hydroxymethyl-2'-deoxyuridine (5-HMdU) are potential biomarkers of cancer risk and oxidative stress. We examined their association with a number of cancer risk factors: smoking, alcohol habits, body fatness, and absence of the glutathione S-transferases M1 and T1 (GSTM1 and GSTT1) in a sample from the population-based Malm? Diet and Cancer cohort (Sweden). This was a cross-sectional study of 264 men and 280 women, 46-67 years of age. Anti-5-HMdU aAb concentration was determined by an ELISA. Data on tobacco exposure were collected through a questionnaire. Alcohol consumption was estimated by a modified diet history method. Body fatness was assessed by a bioimpedance method. The absence or presence of genes coding for GSTM1 and GSTT1 was determined in granulocyte DNA by a multiplex PCR technique. aAb titers were significantly greater in those with high alcohol consumption. Current smokers lacking GSTM1, particularly men, had greater aAb titers compared with nonsmokers or persons expressing GSTM1. Body fatness was inversely associated with antibody titers in men. GSTT1 genotype was not associated with aAb titers. Overall, women had higher aAb titers than men. Adjustment for potential confounders (history of chronic diseases, anti-inflammatory medication, and season of blood sampling) did not change the results. Our study shows that a high alcohol consumption, smoking in combination with lack of GSTM1, and low body fatness (in men) is associated with high titers of anti-5-HMdU aAbs in this population. 相似文献

目的 研究糖尿病膀胱病变(Diabetic cystopathy,DCP)储尿期尿动力学特征及膀胱感觉功能障碍,为阐明DCP的发病机制提供实验依据。方法 分别以链脲佐菌素(STZ)、蔗糖诱导制作大鼠DCP模型及利尿模型,正常大鼠作对照。采用免疫组化结合图像分析技术观察膀胱感觉神经递质CGRP及其纤维改变;制备离体全膀胱,作膀胱灌注测压观察膀胱顺应性改变;放免法测定逼尿肌舒张信号转导分子cAMP。结果 DCP8周时,膀胱壁尤其是粘膜下层的CGRP及其神经纤维明显减少;离体膀胱测压显示糖尿病膀胱顺应性升高;膀胱舒张作用明显弱于对照组和利尿组,逼尿肌cAMP含量显著低于后者。结论 ①DCP对膀胱功能的损害是多方面的:CGRP为主要递质的粘膜下层膀胱容量感觉纤维明显减少,膀胱顺应性改变是DCP的致病机制之一。②DCP早期即已发生储尿期功能障碍,并且可能是膀胱收缩功能损伤的因素。③非糖尿病性利尿状态(多尿)对膀胱功能构成一定的损害作用,但不是DCP形成的主要原因。提示临床上对糖尿病患者作必要的尿动力学检查能较早发现DCP,有助于保护膀胱功能。 相似文献

OBJECTIVE: To conduct an in vitro study to assess the preliminary possibility of using formalin-fixed, instead of fresh, human bone tissues for allografting. METHODS: Fresh cadaveric bone tissues were fixed by formalin for more than 6 months and dissected into 5 mmx5 mmx5 mm pieces and 5 mmx5 mmx40 mm sticks, followed by chemical treatments to prepare the allograft bone materials. When alls treatments were completed, the bone grafts were centrifuged and their properties and cellular compatibility assessed in comparison with the currently used bone grafts clinically. RESULTS: The residual formaldehyde of the fixed allograft bone material was much below the controlled level and no significant differences were noted between the bone graft materials tested in regard to the chemical and mechanical properties and biocompatibility. CONCLUSION: This material we have prepared may meet the clinical demands for bone grafting, with good biocompatibility and less chance for infection by pathological agents. 相似文献
