Dengue virus (DENV) was reintroduced to Peru in the 1990s and has been reported in Puerto Maldonado (population ~65,000) in the Peruvian southern Amazon basin since 2000. This region also has the highest human migration rate in the country, mainly from areas not endemic for DENV. The objective of this study was to assess the proportion of household income that is diverted to costs incurred because of dengue illness and to compare these expenses between recent migrants (RMs) and long-term residents (LTRs). We administered a standardized questionnaire to persons diagnosed with dengue illness at Hospital Santa Rosa in Puerto Maldonado from December 2012 to March 2013. We compared direct and indirect medical costs between RMs and LTRs. A total of 80 participants completed the survey, of whom 28 (35%) were RMs and 52 (65%) were LTRs. Each dengue illness episode cost the household an average of US$105 (standard deviation [SD] = 107), representing 24% of their monthly income. Indirect costs were the greatest expense (US$56, SD = 87), especially lost wages. The proportion of household income diverted to dengue illness did not differ significantly between RM and LTR households. The study highlights the significant financial burden incurred by households when a family member suffers dengue illness. 相似文献

The urinary metabolic pattern after administration of the radiolabeled non-provitamin A carotenoid canthaxanthin was investigated in rats. In the rather complex HPLC urinary metabolic pattern a fraction was found which was conjugated. Deconjugation of the polar conjugates with glusulase, purification of the metabolite with HPLC and identification with GC-MS and NMR revealed that it was 3-hydroxy-4-oxo-7,8-dihydro-beta-ionone. This structure was confirmed by comparisons with HPLC retention times, UV/VIS- and NMR-spectroscopy and GC-MS of the synthesized compound. 相似文献

Unlike many viral hemorrhagic fevers (VHFs), Lassa fever (LF) is not a rare disease that emerges only as sporadic cases or in outbreak form. Although surveillance is inadequate to determine the true incidence, up to 300,000 infections and 5000 deaths from LF are estimated to occur yearly. The highest incidence is in the "Mano River Union (MRU) countries" of Sierra Leone, Liberia, and Guinea. Although civil unrest in this region over the past two decades has impeded capacity building and research, new-found peace in recent years presents new opportunities. In 2004, the Mano River Union Lassa Fever Network (MRU LFN) was established to assist MRU countries in the development of national and regional surveillance, diagnosis, treatment, control, and prevention of LF. Here, we review the present literature on treatment and pathogenesis of LF and outline priorities for future research in the field made possible by the improved research capacity of the MRU LFN. 相似文献

The possible excretion of biocytin or biotinyl-peptides was investigated in urines of children with congenital biotinidase deficiency. Treatment of urine samples of affected children with normal plasma (as source of biotinidase) liberated additional free biotin (L. plantarum assay), indicating the presence of biotinyl-compounds. Using thin-layer chromatography the occurrence of biocytin was demonstrated in all urine samples of the affected children. Von Hippel‐Lindau disease (VHL) is an autosomal dominant cancer syndrome. Affected individuals are predisposed to multiple tumors, primarily of the central nervous system (CNS), eyes, adrenals, and kidneys. The VHL tumor suppressor gene on chromosome 3p26–25 is partially or completely deleted in 20 to 30% of families with VHL. We identified deletions ranging from 0.5 kb to 250 kb affecting part of or the entire VHL and flanking genes in 54 families. In 33 of the index patients, the breakpoints were precisely characterized by DNA sequencing. Of the 66 breakpoints, 90% were located in Alu elements, revealing Alu‐mediated recombination as the major mechanism for large germline deletions of the VHL gene, which lies in a region of high Alu density. Interestingly, an AluYa5 element in VHL intron 2, the evolutionarily youngest Alu element and the only such element in the entire region, was found to be the most recombinogenic, involved in 7 out of the 33 deletions. In comparison to VHL patients in general, the 54 index cases and their affected relatives showed a higher occurrence of renal cell carcinomas (RCC) and of CNS hemangioblastomas. We not only noted the association of RCC with retention of the HSPC300 gene, but also observed a significant correlation between retention of HSPC300 and the development of retinal angiomas (AR). This study reveals that germline VHL deletions provide a particularly rich source for the study of Alu‐mediated unequal crossover events, and provides evidence for a protective role of the loss of the actin‐regulator gene HSPC300 for the development of both RCC and AR. Up to now lidocain was regarded as remedy of first choice in life-threatening ventricular arrhythmias. According to more recent investigations in the emergency situation ajmalin showed itself superior to lidocain in the treatment of ventricular tachycardias. For the prophylaxis of primary ventricular fibrillation lidocain is still recommended, though despite numerous studies an effectiveness is not verified. The treatment of the primary ventricular fibrillation should always be performed by defibrillation as far as possible. The earlier it is performed the more successful is the measure. Summary Stone analyses (kidney, upper urinary tract) of the department of Urology, University of Erlangen, from a four-year-period (1974–1977) have been recorded with emphasis to stone composition, sex and age of the pertinent stone forming patients.During this time period there were no substantial changes as regards the per cent frequency of the various stone types. The most frequent type was calcium oxalate (CaOx), followed by uric acid, calcium phosphate (CaP), struvite and cystine. Stone analyses were mostly requested for patients between 46 and 55 years of age. Stone incidence in our clinic is calculated to be 1.22 times higher in males than females, especially beyond 36 years of age. The frequency peaks are: pure (=100 per cent) CaOx 36–45 years; CaOx with additional mineral phases (mostly CaP) 46–55 years; uric acid 56–65 years; CaP 26–35 years.From those patients who underwent further investigations in searching for metabolic abnormalities serum concentrations, urine mineral clearances in fasting urine samples, and activity products of stone forming mineral phases in sequentially collected specimens from 24 h and 2 h fasting urine had been measured and compared with values from healthy control subjects. In urolithiasis (idiopathic) there is a normal parathyroid hormone blood level, a generally lower serum inorganic phosphate and magnesium concentration.In pure (=100 per cent) CaOx and uric acid lithiasis serum uric acid and creatinine are higher than in controls, urine pH and calcium clearance in some groups are different too. Clearances of magnesium, uric acid, phosphate, sodium are within normal limits in urolithiasis. When expressing the propensity to form stones in terms of activity products, then only uric acid lithiasis deviates substantially from normal. All other stone types differ only slightly or not at all from each other and controls respectively.It is concluded that 1) in our geographic region the various stone types prevail in different age periods; 2) there are distinct alterations of parameters of mineral metabolism in urolithiasis; 3) measuring urine clearances may lead to assume falsely normal mean urine excretion of stone forming constituents. 相似文献

Animal liver is a rich source of vitamin A. Due to retinoic acid (RA) metabolites, vitamin A has a teratogenic potential and women are generally advised to avoid or to limit the consumption of liver during pregnancy. In a recent study in non-pregnant female volunteers following single and repeated doses of up to 30,000 IU/day of vitamin A as a supplement, the plasma concentration time curve of all-trans RA acid showed a diurnal-like profile. But, the overall exposure (AUC24h) remained essentially unaltered whereas AUC24h increased linearly with dose for 13-cis and 13-cis-4-oxo RA. The current study in non-pregnant female volunteers showed that a single high vitamin A intake with a liver meal (up to 120,000 IU) exhibited a similar diurnal-like plasma concentration time curve for all-trans RA and its overall exposure remained also unaltered, despite a temporary two-fold increase in peak plasma concentration. Concentrations of 13-cis and 13-cis-4-oxo RA increased several-fold after a liver meal, and exposure (AUC24h) increased three- to five-fold. Pooling our results with data in the literature revealed a linear relation between the mean AUC24h of 13-cis and 13-cis-4-oxo RA and vitamin A intake with liver. Metabolism to all-trans RA of vitamin A with liver seems not to be of safety concern. However, the observed increase of plasma concentrations and the dose-dependent increase in exposure to 13-cis and 13-cis-4-oxo RA support the current safety recommendations on vitamin A intake and suggest that women should be cautious regarding their consumption of liver-containing meals during pregnancy. 相似文献
