Current role of minimally invasive approaches in the treatment of early gastric cancer

Despite declining incidence,gastric cancer remains one of the most common cancers worldwide.Early detection in population-based screening programs has increased the number of cases of early gastric cancer,representing approximately 50%of newly detected gastric cancer cases in Asian countries.Endoscopic mucosal resection and endoscopic submucosal dissection have become the preferred therapeutic techniques in Japan and Korea for the treatment of early gastric cancer patients with a very low risk of lymph node metastasis.Laparoscopic and robotic resections for early gastric cancer,including function-preserving resections,have propagated through advances in technology and surgeon experience.The aim of this paper is to discuss the recent advances in minimally invasive approaches in the treatment of early gastric cancer.     

Square-edge polymethylmethacrylate intraocular lens design for reducing posterior capsule opacification following paediatric cataract surgery: initial experience

Purpose: To compare the incidence and severity of development of posterior capsule opacification (PCO) following implantation of square‐edged polymethylmethacrylate (PMMA) or hydrophobic acrylic intraocular lenses (IOLs) following paediatric cataract surgery. Design: Prospective, consecutive, interventional, comparative, randomized and cross‐sectional study of 40 eyes of 32 children aged between 4 and 12 years who underwent phacoemulsification and posterior chamber IOL implantation. Methods: The patients were randomized into two groups of 20 eyes each. Group 1 eyes received a square‐edge hydrophobic acrylic IOL (Acrysof SA 60 AT, Alcon Surgical, Fort Worth, Texas), and Group 2 eyes received a square‐edge single‐piece PMMA lens (Aurolab SQ 3600 Aurolab IOL Division, Madurai, India) in the capsular bag. No eye underwent a primary posterior capsulotomy. The PCO density was evaluated on slitlamp retroillumination photographs by using POCOman software at 3, 6, 9 and 12 months post surgery. Results: The average percentage PCO on POCOman analysis was 51.23 for Group 1 and 49.49 for Group 2 (P = 0.09), and the average PCO severity grade was 1.34 in Group 1 and 1.12 in Group 2 (P = 0.08). Visual axis remained clear in 14 of 20 eyes with the acrylic lens as compared with 13 of 20 eyes with the PMMA lens. (P = 0.32). Conclusions: Square‐edge PMMA IOLs offer a significant cost advantage over acrylic lenses at similar rates of PCO formation following paediatric cataract surgery, which is of significant value in developing countries.     

Preferences for active and aggressive intervention among patients with advanced cancer

Background  

Intrinsic to "Patient-Centered Care" is being respectful and responsive to individual patient preferences, expressed needs, and personal values. Establishing a patient's preferences for active and aggressive intervention is imperative and foundational to the development of advance care planning. With the increasing awareness and acceptance of palliative philosophies of care, patients with advanced cancer are increasingly transitioning from active and aggressive medical management (AAMM) to conservative palliative management (CPM).     

Uptake of critical knowledge in nursing practice: lessons learned from a knowledge translation study

This article is based on a knowledge translation (KT) study of the transition of patients from hospital to home. It focuses on the lessons learned about the challenges of translating research-derived critical knowledge in practice settings. The authors situate the article in current discourses about KT; discuss their understanding of the nature of critical knowledge; and present themes from their body of research, which comprises the knowledge that was translated. The findings have the potential to guide future KT research that focuses on the uptake of critical knowledge in nursing practice.     

Primary intraocular lens implantation in the first two years of life: safety profile and visual results

PURPOSE: To study the safety profile of primary intraocular lens (IOL) implantation in the first two years of life. MATERIALS AND METHODS: A prospective nonrandomized, interventional study was done at the tertiary care center. Forty-five eyes of 27 children (aged three weeks to two years) with congenital cataract who underwent phacoaspiration with continuous curvilinear capsulorrhexis combined with primary posterior capsulotomy and anterior vitrectomy with in the bag placement of IOL at primary surgery were included. Outcome measures were clarity of visual axis, postoperative inflammation, fixation pattern and retinoscopy. RESULTS: The mean IOL power was 23.95 +/- 0.87 diopter (D) (range 22D to 27D). Follow-up ranged from 12 months to 48 months (mean 18 +/- 9.13 months). In infants, hypermetropia decreased from 6.60D +/- 2.64D at one week postoperatively to 3.03D +/- 2.53D at one year following surgery and 4.78D +/- 1.93D to 2.56D +/- 1.5D in children older than one year at the time of surgery. Re-opacification of the central visual axis was observed in six eyes (13.3%). Significant posterior synechiae due to irido-capsular adhesion was seen in seven eyes (15.6%). Five of these seven eyes also had significant posterior capsular opacification. CONCLUSIONS: Meticulously performed primary IOL implantation and primary posterior capsulorrhexis with anterior vitrectomy in the first two years of life is a safe and effective method of aphakic correction.
