BACKGROUND: The joint gap is set rectangular at 90 degrees flexion during total knee arthroplasty (TKA). However, the condition of the joint gap in deep knee flexion is obscure. METHODS: The method for obtaining a posteroanterior view radiograph of the knee at 90 degrees flexion (the epicondylar view) was modified, and a method to obtain an anteroposterior view radiograph at 120 degrees flexion (deep flexion view) was established. With this method, subjects lie on the radiography table with their thighs placed on a device so their lower legs hang down in neutral rotation with a 1.5-kg weight attached to the ankle. The joint gap angle and medial and lateral joint space widths were measured on epicondylar view and deep flexion view radiographs in 20 normal male subjects, 20 normal female subjects, and 20 subjects after TKA. RESULTS: The joint gap was almost rectangular at two flexion angles in normal subjects. In the implanted knees, the gap angle was 1.4 degrees varus +/- 3.3 degrees (mean +/- standard deviation), and no significant difference was found between medial and lateral joint space widths at 90 degrees flexion. In contrast, the gap angle was 2.5 degrees varus +/- 2.5 degrees and the lateral joint space width was significantly wider than the medial joint space width at 120 degrees flexion (P < 0.001). The gap angle was more varus with a significant difference in the implanted knees than that in the normal subjects at 120 degrees flexion (P < 0.001). CONCLUSIONS: The joint gap was trapezoidal with a wider lateral side at 120 degrees flexion even though it was almost rectangular at 90 degrees flexion after TKA. 相似文献
We report three cases of small cell carcinoma of the urinary bladder. Case 1: A 69-year-old man showed microscopic hematuria during follow up of prostate cancer of stage D2. The patient was diagnosed with small cell carcinoma of the urinary bladder at the stage of pT2N0M0. Complete remission was achieved by three courses of chemotherapy consisting of irinotecan and carboplatin. The patient was died by prostate cancer 16 months after the chemotherapy. Case 2: An 83-year-old woman presented with macroscopic hematuria. The patient was diagnosed with small cell carcinoma of the urinary bladder at the stage of pT2N0M0 and partial cystectomy was performed. The patient has been alive without any evidence of tumor recurrence at 6 months after surgery. Case 3: An 84-year-old man presented with macroscopic hematuria. The patient was diagnosed with small cell carcinoma of the urinary bladder at the stage ofcT3bN0M1 with multiple liver metastases. A 22-year-old man presented with renovascular hypertension, based on a stenosis of the distal portion of the right renal artery with a "string of beads"-like appearance. An intravascular ultrasound image at the renal artery lesion revealed irregularity of the vascular wall. Directional atherectomy was performed and histopathology of atherectomised tissues showed medial fibroplasia, a common type of fibromuscular dysplasia. After atherectomy his hypertension was markedly improved. We report here a case of renovascular hypertension due to fibromuscular dysplasia, successfully diagnosed and treated with IVUS-guided renal atherectomy. 相似文献
BACKGROUND: Infection is a serious complication after liver transplantation. Immunization is one means of controlling infections. The objective of this study was to investigate the efficacy and safety of simultaneous administration of several vaccines before transplantation and the efficacy and safety of administration under immunosuppressive conditions after transplantation. METHODS: Fifty-eight patients who underwent living-related liver transplantation between April 1994 and March 2000 were included in this study. Simultaneous administration of a maximum of six vaccines was performed in a short period of time before transplantation. We also readministered vaccines to 15 patients with waning antibody titers after transplantation from June 1999. We investigated whether patients could seroconvert for measles, rubella, mumps, and varicella after immunization and how long antibody titers could be retained by measuring them several times throughout the period before and after transplantation. We also examined side effects caused by immunization. RESULTS: The rates of seroconversion against measles, rubella, mumps, and varicella after the pretransplantation vaccination were 82%, 100%, 90%, and 95%, respectively. The rates of reseroconversion against measles, rubella, mumps, and varicella after the posttransplantation revaccination were 85%, 100%, 100%, and 71%, respectively. Although antibody titers against these viruses generally waned with time, no patient exhibited any serious illness or side effects. CONCLUSION: Although 12 of 58 patients (21%) had an infection, pretransplantation immunization was effective to prevent serious illness, especially for the 6 months after transplantation. Posttransplantation live-vaccine administration under immunosuppressive conditions is effective and safe. 相似文献
Background: Ketamine potentiates intravenous or epidural morphine analgesia. The authors hypothesized that very-low-dose ketamine infusion reduces acute and long-term postthoracotomy pain.
Methods: Forty-nine patients scheduled to undergo open thoracotomy were randomly assigned to receive one of two anesthesia regimens: continuous epidural infusion of ropivacaine and morphine, along with intravenous infusion of ketamine (0.05 mg [middle dot] kg-1 [middle dot] h-1 [approximately 3 mg/h], ketamine group, n = 24) or placebo (saline, control group, n = 25). Epidural analgesia was continued for 2 days after surgery, and infusion of ketamine or placebo was continued for 3 days. Pain was assessed at 6, 12, 24, and 48 h after surgery. Patients were asked about their pain, abnormal sensation on the wound, and inconvenience in daily life at 7 days and 1, 3, and 6 months after surgery.
Results: The visual analog scale scores for pain at rest and on coughing 24 and 48 h after thoracotomy were lower in the ketamine group than in the control group (pain at rest, 9 +/- 11 vs. 25 +/- 20 and 9 +/- 11 vs. 18 +/- 13; pain on coughing, 26 +/- 16 vs. 50 +/- 17 and 30 +/- 18 vs. 43 +/- 18, mean +/- SD; P = 0.002 and P = 0.01, P < 0.0001 and P = 0.02, respectively). The numerical rating scale scores for baseline pain 1 and 3 months after thoracotomy were significantly lower in the ketamine group (0.5 [0-4] vs. 2 [0-5] and 0 [0-5] vs. 1.5 [0-6], median [range], respectively; P = 0.02). Peritoneal relapse is the most common pattern of tumor progression in advanced gastric cancer. Clinicopathological findings are sometimes inadequate for predicting peritoneal relapse. The aim of this study was to identify patients at high risk of peritoneal relapse in a prospective study based on molecular prediction.
Methods
RNA samples from 141 primary gastric cancer tissues after curative surgery were profiled using oligonucleotide microarrays covering 30,000 human probes. Firstly, we constructed a molecular prediction system and validated its robustness and prognostic validity by 500 times multiple validation by repeated random sampling in a retrospective set of 56 (38 relapse-free and 18 peritoneal-relapse) patients. Secondly, we applied this prediction to 85 patients of the prospective set to assess predictive accuracy and prognostic validity.
Results
In the retrospective phase, repeated random validation yielded ~68% predictive accuracy and a 22-gene expression profile associated with peritoneal relapse was identified. The prediction system identified patients with poor prognosis. In the prospective phase, the molecular prediction yielded 76.9% overall accuracy. Kaplan–Meier analysis of peritoneal-relapse-free survival showed a significant difference between the “good signature group” and “poor signature group” (log-rank p = 0.0017). Multivariate analysis by Cox regression hazards model identified the molecular prediction as the only independent prognostic factor for peritoneal relapse.
Conclusions
Gene expression profile inherent to primary gastric cancer tissues can be useful in prospective prediction of peritoneal relapse after curative surgery, potentially allowing individualized postoperative management to improve the prognosis of patients with advanced gastric cancer. 相似文献
Intraoperative MRI (iMR) and neuronavigation have substantially changed the principles of surgery for brain tumors. iMR provides updated information on anatomical data and unanticipated brain events, thereby allowing safer and more accurate surgery. We herein report a case of unanticipated intracranial hemorrhage in an iMR imaging suite. The patient was a 53-year-old man with a chief complaint of generalized convulsion. MRI showed a lesion in the right temporal lobe about 6.0 cm in diameter. The tumor was resected using an iMR system and neuronavigation. The first iMR images showed a residual tumor in the medial temporal lobe. No brain events were detected at this time. We obtained updated navigation data and performed additional resection. After completion of the planned tumor resection, additional iMR images were taken. The second iMR images confirmed that the tumor had been completely removed, but they also revealed a contra-lateral subdural hemorrhage. After expedited closure of the original incision, left unilateral craniotomy was performed and the hematoma was evacuated. Fortunately, the patient had no new neurological deficits. The use of iMR imaging allowed the complete removal of the tumor and facilitated prompt and effective identification of an unanticipated life-threatening complication. 相似文献
To clarify the relationship between the vertebral level reached by the thumb and the internal rotation angle of the humerus, 7 shoulders in healthy volunteers were examined by use of an electromagnetic tracking device. Measurements were repeated in the hanging-arm position with the thumb pointing anteriorly and at the buttock, sacrum, and each vertebral level up to T6. From the hanging-arm position to the buttock, 54.3% of internal rotation occurred (mean, 39.8 degrees ), and from the buttock to the sacrum, 11.7% occurred (mean, 8.6 degrees ). In total, 66% of internal rotation occurred from the hanging-arm position to the sacrum. Above the sacrum, the contribution of elbow flexion to the level of the vertebral spine became much greater, and internal rotation of the shoulder did not change significantly above the T12 level. We recommend that the level of the thumb below the buttock be subdivided for more accurate assessment of internal rotation. 相似文献
Introduction The aging population has led to a significant rise in the number of patients undergoing operations such as cholecystectomy.
We have evaluated and compared the results of laparoscopic cholecystectomy (LC) in patients aged 80 years and over with those
of patients aged between 65 and 79 years.
Methods A total of 471 patients aged 65 to 79 years (group 1) and 45 patients aged ≥80 years (group 2) underwent LC. All patients
underwent preoperative spiral computed tomography after intravenous infusion cholangiography and intraoperative cholangiography.
Results There was a higher incidence of choledocholithiasis and gallbladder cancer in the patients ≥80 years of age. In addition,
group 2 patients had a higher incidence of cardiopulmonary disease and higher American Association of Anesthesiology scores
than did those in group 1. With respect to the conversion rate to open surgery, morbidity, mortality, and length of hospital
stay, there were no significant differences between the two groups. There was a significantly higher incidence of positive
bile cultures and gram-negative rods in group 2 patients than in those in group 1.
Conclusions Octogenarians tolerated LC well. Therefore, early elective LC should be encouraged to minimize morbidity and mortality in
these elderly patients who have symptomatic cholelithiasis. 相似文献
