温针灸为主治疗前列腺增生症疗效观察

笔者于1998年2月～1999年12月应用温针灸为主治疗前列腺增生症51例,效果理想.现报道如下. 1临床资料 1.1病例选择全部98例观察病例均来自男科门诊.按随机法分为治疗组和对照组.其中治疗组51例,年龄最小49岁,最大72岁,平均56.3岁;病程最短2年,最长5年,平均4.1年.对照组47例,年龄最小48岁,最大71岁,平均57.1岁;病程最短3年,最长7年,平均5.0年.两组具有可比性.

Ｇ１、Ｇ２引物是对问号钩体具有特异性引物。分别用Ｇ１或Ｇ２单引物对问号钩体中国参考株进行前４个低严格度循环的ＰＣＲ扩增，扩增带谱显示赖型、犬型、致热型、秋季型、澳洲型、临海型、乌尔夫型、溶血型为一类，而爪哇型、拜伦型、波摩那型、七日热型、巴叶赞型塔拉索夫型、曼耗Ⅱ型是不与以上赖型等血清型本为一类，双曲钩体ａｔｏｃ型及伊利尼细丝体伊利尼型的扩增带谱与问号钩体截然不同。应用苯酚法提取的高纯度钩体ＤＮＡ     

黄连细胞二步法悬浮培养生产黄连生物碱类成分的探索

为提高用植物细胞培养生产镒生代谢产物的单位产量，作者对黄连细胞采用二步法悬浮培养生产黄连生物碱进行了探索。结果显示：黄连细胞采用一步法悬浮培养６周，细胞干，鲜重产率分别为２０．９６ｇ／Ｌ和１７４．９２ｇ／Ｌ，增重约７００％，细胞中总生物碱含量为１４．７９ｍｇ／ｇ．ｃｅｌｌ ｄｗ，培养基中生物碱量为１２．９６ｍｇ／Ｌ，每升培养液共可收获生物碱３２３ｍｇ。采用先在生长培养基中培养３周，在合成碱量为１２     

Fictive oesophageal peristalsis evoked by activation of muscarinic acetylcholine receptors in rat nucleus tractus solitarii

The aim of the present study was to determine if muscarinic acetylcholine receptor-mediated peristaltic rhythmogenesis in the rat oesophagus is a central motor program that can be generated without peripheral sensory support. In anaesthetized male Sprague-Dawley rats, pressure-ejection of glutamate (10–20 pmol) and muscarine (5–10 pmol) in the subnucleus centralis of the nucleus tractus solitarii (NTS_C) evoked monophasic pressure waves and rhythmic oesophageal peristalsis, respectively, but did not change mean arterial blood pressure or respiration. Application of muscarine (50–100 pmol) to the NTS extraventricular surface evoked rhythmic multi-unit burst discharges in the compact formation of the nucleus ambiguus (AMB_C) that led to oesophageal peristalsis in a phase-locked manner. Evoked rhythmic AMB_C activity persisted during neuromuscular blockade with curare, although the peak frequency of individual bursts was decreased. In a brainstem slice preparation, intracellular and whole cell patch recordings from AMB_C neurones during focal stimulation of the NTS_C region with muscarine revealed rhythmic depolarizing waves that showed a pattern similar to that of rhythmic oesophageal peristalsis. The present findings support the concept that medullary circuits comprising premotor neurones of the NTS_C are intrinsically capable of generating rhythmic oesophagomotor output, but are subject to a powerful modulation by peripheral sensory feedback.     

大肠癌手术后肝转移的诊断与治疗

目的：探讨大肠癌手术后肝转移的诊断与治疗。方法：对35例大肠癌手术后肝转移的诊治效果进行回顾性分析。结果：所有的病例均得到CEA、B超和（或）CT检查证实，其1年和3年生存率在根治性手术切除组明显高于其他治疗组。结论：肝转移癌的早期诊断是提高生存率的关键，手术切除及选择性肝动脉栓塞化疗是改善其预后的选择。     

Clinical pathologic study on effect of Qianggan capsule in treating patients of chronic hepatitis B with liver cirrhosis

Objective  

To explore the clinical therapeutic effect of Qianggan Capsule (QGC) in treating chronic hepatitis B with liver fibrosis from the pathological aspect.     

Whole CanA Gene Amplification of Helicobacter pylori and Its Fingerprinting by Restriction Fragment Length Polymorphism

To set up a method of amplification for the whole CagA gene of Helicobacter pylori and its fingerprinting by restriction fragment length polymorphism(RFLP),nested PCR was employed in combination with TD-PCR to amplify the gene and EcoRI and Hind Ⅲ were used to generate the RFLP fingerprinting.Target DNA fragments from 13 of 20 samples were successfully amplified and the relevant RFLP fingerprintings were obtained.It is concluded that the method can be used to amplify the whole CagA gene and CagA gene has apparent diversity of RFLP profile.     

碳酸利多卡因硬膜外麻醉用于乳腺癌根治术的评价

40例乳腺癌根治术患者，美国麻醉医师协会（ASA）分级Ⅰ-Ⅱ级，随机分为2且，第1组硬膜外阻滞＋臂丛神经阻滞，应用1．33％盐酸利多卡因＋0．15％盐酸地卡因＋1：20万肾上腺素，硬膜外阻滞首量12ml，臂丛神经阻滞20ml;第2组硬膜外阻滞，应用1．73％碳酸利多卡因＋1：20万肾上腺素，首量15ml。两组硬膜外阻滞选为T4－5间隙穿刺。首量给完后测定麻醉药效，并监测患者心率（HR）、平均动脉压（MAP）、血氧饱和度（SpO2)，在麻醉前、后30、60、120min抽取动脉血作血气分析。结果显示：第2组麻醉的起效时间、阻滞完善时间要比第1组短（P＜0．01）。感觉神经阻滞分段数要比第1组多（P＜0．01）；两组病人的HR和MAP，麻醉手术中改变不明显。麻醉后面罩给氧。两组病人的SpO2、PaO2增加，PaCO2有所下降，两组比较无统计学意义，但组内前后对照有显著差异（P＜0．05）。结果提示碳酸利多卡因硬膜外麻醉下施行乳腺癌根治术具有良好的麻醉效果和更高的安全性。     

Strength and fracture toughness of MgO-modified glass infiltrated alumina for CAD/CAM.

OBJECTIVE: This study was conducted to investigate the effect of MgO additive to Al2O3 on the flexural strength, fracture toughness of glass infiltrated alumina for CAD/CAM application. METHODS: Alumina blanks with additive of 0.5 wt% MgO were prepared via isostatic pressing and sintering at 1400 degrees C for 2h, and then alumina-glass composites were fabricated by infiltrating the molten glass into the partially sintered alumina compact. Flexural strength and fracture toughness were determined using three point bending methods and a single edge notched beam method. The mechanism of crack propagation was observed under a field emission scanning electron microscope. RESULTS: The three-point flexural strength and fracture toughness of partially sintered alumina and alumina-glass composite were 210 MPa, 1.86 MPam(1/2), and 432.2 MPa, 5.12 MPam(1/2), respectively, and they were free of shrinkage during the processing of glass infiltration. The field emission SEM micrograph indicated that indentation caused a non-planar crack propagation including crack deflection and crack bowing. SIGNIFICANCE: MgO was used as an additive to alumina to improve the strength and fracture toughness of partially sintered alumina and alumina-glass composite. The high strength and toughness are related to toughening by the distribution of alumina with uniform particle sizes, crack bowing, crack deflection and the beneficial wetting properties of the particle surface.     

病案质量整体管理的做法

病案质量是衡量临床工作质量的重要标志。目前，因重效益轻质量、重科研轻临床现象突出，致病案质量有所下降。因此，应加强病案质量整体管理，建立医院、科室、个人三级病案质量控制体系；以病案内涵质量为核心内容，制定完善科学的病案质量评价标准；加强岗前培训，抓人员素质的提高；强化病案过程质量管理和终末质量管理，重视病案质量信息反馈作用。
