BACKGROUND AND PURPOSE: Modern conformal radiotherapy treatments require accurate dose calculation in any relevant clinical situation. One of these situations is the treatment of lung tumors, where irradiation has to be planned under challenging conditions for dose calculation. In this study we assess the errors in dose values predicted by fast Fourier transform convolution (FFTC) and multigrid superposition (MGS) algorithms implemented in a commercial treatment planning system (TPS). MATERIALS AND METHODS: FFTC and MGS algorithms were used in a FOCUS 3.0.0 (Computerized Medical Systems, USA) to calculate doses in treatment plans using photon beams of 6 and 25 MV nominal energy from a Saturne 43 linac (GE Medical Systems, USA). A 10x10-cm beam irradiating a mediastinum-lung and a thoracic wall-lung-thoracic wall modeled geometry was assessed. The calculated data were compared with measurements performed with radiographic films and ionization chamber. RESULTS: FFTC algorithm leads to an average deviation from ionometric dose measurements of over 10%. Discrepancies between measured and calculated beam fringe values (distance between 50 and 90% isodose lines) of up to 8 mm were observed. For MGS algorithm, all the points assessed in both geometries fulfilled the 3%-3 mm accuracy criteria and the average deviation of absolute dose was about 1%. A maximum of 3 mm deviation in the beam fringe for any depth was found and was within 2 mm beyond the buildup region. Deviations between ionometric and film measurements were within 3%. CONCLUSIONS: MGS algorithm assesses with reasonable accuracy dose distributions and absolute dose in inhomogeneous regions like the lung region. Therefore, and respecting the inhomogeneity dose calculation, the system could be used in routine clinical practice and in dose-escalation programs. This is not true in the case of FFTC algorithm which leads to errors greater than 10% in the absolute dose calculation and underestimates the beam fringe by up to 8 mm. 相似文献
Three cases of intravenous leiomyomatosis (IVL) of the uterus, a rare benign smooth-muscle tumor, are described. A preoperative
diagnosis of IVL was not made in any of the patients, all of which presented with a pelvic mass with the presumptive diagnosis
of leiomyoma. Surgical exploration confirmed the presence of uterine mass and two of the three cases showed extra-uterine
extension into the ovarian or uterine veins. Histological examination demonstrated a fascicular pattern of bland spindle-shaped
smooth-muscle cells, which extended to veins inside the myometrium or to extrauterine veins. This was confirmed by immunohistochemical
stain for desmin and factor VIII. Despite their histological benignity, these lesions have a tendency to metastasize and are
closely related to the conditions called “benign metastasizing leiomyoma” and “intracaval mass and cardiac extension”. The
primary treatment of IVL is hysterectomy and excision of any extrauterine tumor, when technically feasible. Anti-estrogenic
therapy has been suggested as potentially useful in controlling of unresectable tumor. According to the literature, the follow-up
must be long and periodic postoperative ultrasonic or magnetic nuclear resonance imaging studies may be useful in detecting

Across populations of children, Helicobacter pylori prevalence ranges from under 10% to over 80%. Low prevalence occurs in the U.S., Canada, and northern and western Europe; high prevalence occurs in India, Africa, Latin America, and eastern Europe. Risk factors include socioeconomic status, household crowding, ethnicity, migration from high prevalence regions, and infection status of family members. H. pylori infection is not associated with specific symptoms in children; however, it is consistently associated with antral gastritis, although its clinical significance is unclear. Duodenal ulcers associated with H. pylori are seldom seen in children under 10 years of age. H. pylori-infected children demonstrate a chronic, macrophagic, and monocytic inflammatory cell infiltrate and a lack of neutrophils, as compared with the response observed in adults. The effect of H. pylori infection on acid secretion in children remains poorly defined. The events that occur during H. pylori colonization in children should be studied more thoroughly and should include urease activity, motility, chemotaxis, adherence, and downregulation of the host response. The importance of virulence determinants described as relevant for disease during H. pylori infection has not been extensively studied in children. Highly sensitive and specific methods for the detection of H. pylori in children are needed, especially in younger pediatric populations in which colonization is in its early phases. Criteria for the use of eradication treatment in H. pylori-infected children need to be established. Multicenter pediatric studies should focus on the identification of risk factors, which can be used as prognostic indicators for the development of gastroduodenal disease later in life. 相似文献
Objective: To evaluate the effect of prednisolone plus low-dose aspirin (PSL/LDA) in women with autoimmune conditions who were enrolled in an IVF-ET program.
Design: A retrospective clinical study.
Setting: In vitro fertilization unit, Niigata University Hospital, Niigata, Japan.
Patient(s): Three hundred seven women who underwent IVF-ET between January 1996 and December 1997.
Intervention(s): Prednisolone (10 mg/d) and aspirin (81 mg/d) were administered to the women with autoantibodies who chose to participate.
Main Outcome Measure(s): Pregnancy and implantation rates with IVF-ET.
Result(s): Women undergoing IVF who had positive antinuclear antibodies, with or without antiphospholipid antibodies, had significantly lower pregnancy and implantation rates than did women without autoantibodies (14.8% versus 21.7% and 6.8% versus 10.4%, respectively). The administration of PSL/LDA to women with antinuclear antibodies significantly improved the outcome of IVF-ET (40.6% pregnancy rate and 20.3% implantation rate).
Conclusion(s): A high proportion of women who are undergoing IVF-ET have autoantibodies, which are associated with poor IVF outcomes. The administration of PSL/LDA to these women may improve their implantation rate. 相似文献
BACKGROUND: Corticosteroids have been recommended to facilitate rapid recovery after cardiac surgery. We previously reported that dexamethasone given after induction of anesthesia decreases the incidence of postoperative shivering. We performed a post hoc analysis of the data obtained during that study, focusing on secondary outcomes. METHODS: A total of 235 adult patients undergoing elective coronary or valvular heart surgery were randomized to receive dexamethasone 0.6 mg/kg or placebo after induction of anesthesia. Patients who had pharmacologically treated diabetes mellitus, had hypersensitivity to dexamethasone, or were receiving treatment with corticosteroids were excluded. RESULTS: We found that, compared with placebo, patients receiving dexamethasone were more likely to remain tracheally intubated for 6 hours or less (26.4% vs 10.0%, p = 0.020) and had a lower incidence of early postoperative fever (20.2% vs 36.8%, p = 0.009) and new-onset atrial fibrillation during the first 3 days postoperatively (18.9% vs 32.3%, p = 0.027). However, we could not demonstrate a statistical difference in the intensive care unit or hospital length of stay, or in overall morbidity and mortality. The dexamethasone-treated patients were also more likely to have a higher blood glucose on admission to the intensive care unit (186 mg/dL vs 143 mg/dL, p = 0.012). CONCLUSIONS: Dexamethasone facilitates early tracheal extubation and is associated with a lower incidence of early postoperative fever and new-onset atrial fibrillation. Apart from a treatable decreased glucose tolerance, dexamethasone treatment was not shown to affect morbidity or mortality significantly. 相似文献
A growing body of evidence illustrates that 5-HT3 receptor antagonist drugs may be of benefit in the treatment of negative symptoms in schizophrenia.
Objective
The objective of this study was to assess the efficacy and tolerability of tropisetron add-on to risperidone on negative symptoms in patients with chronic stable schizophrenia.
Methods
In a double-blind, placebo-controlled 8-week trial, 40 patients with chronic schizophrenia who were stabilized on risperidone were randomized into tropisetron or placebo add-on groups. Psychotic symptoms were measured using the Positive and Negative Syndrome Scale (PANSS) every 2 weeks. Furthermore, extrapyramidal and depressive symptoms as well as side effects were assessed. The primary outcome measure was the difference in change from baseline of negative subscale scores between the two groups at week 8.
Results
Tropisetron resulted in greater improvement of the total PANSS scores [F(1.860,70.699)?=?37.366, p?<?0.001] as well as negative scores [F(2.439,92.675)?=?16.623, p?<?0.001] and general psychopathology [F(1.767,67.158)?=?4.602, p?=?0.017], but not positive subscale scores [F(1.348, 51.218)?=?0.048, p?=?0.893] compared to placebo. In a multiple regression analysis controlling for positive, extrapyramidal, and depressive symptoms, treatment group (standardized β?=??0.640) significantly predicted changes in primary negative symptoms. The side effect profile did not differ significantly between the two groups.
Conclusion
Tropisetron add-on to risperidone improves the primary negative symptoms of patients with chronic stable schizophrenia. 相似文献
OBJECTIVE: To assess the posterior corneal power and asphericity changes after myopic laser in situ keratomileusis (LASIK) and to correlate these changes with the amount of correction and the residual stromal bed thickness. DESIGN: Prospective nonrandomized (self-controlled) comparative study. PARTICIPANTS: Fifty-seven eyes of 14 women and 15 men, mean age at the time of surgery 33 +/- 9 (range, 19-53) years with a spherical equivalent (SEQ) of -1.00 to -15.50 (mean, -5.07 +/- 2.81) diopters (DI). INTERVENTION: All procedures were performed with the Keratom II Coherent-Schwind excimer laser and and the Moria Model One microkeratome (150-microm head). Subjective refractometry, Orbscan slit scanning corneal topography analysis and pachymetry were performed before and 3 months after LASIK for myopia (n=35, -1.00 to -15.50 D, mean -4.75 +/- -3.07 D) or myopic astigmatism (n=22, sphere 0.00 to -9.75 D, mean -4.75 +/- 2.36 D; cylinder -0.75 to -3.50 D, mean -1.68 +/- 0.86 D). Intended ablation depth ranged from 12 to 108 (mean, 48 +/- 22) microm. Topographic raw data were decomposed into a set of Zernike polynomials as published in detail previously, and parameters potentially indicative for detection of a "mild keratectasia" were derived. MAIN OUTCOME MEASURES: Posterior central corneal power and asphericity before and after LASIK were compared, and changes of these variables were correlated with the SEQ change (deltaSEQ)and the residual corneal bed thickness RBT). RESULTS: The mean RBT after LASIK was 280 +/- 42 microm. Overall, change of posterior power (-6.28 +/- 0.22 D/ -6.39 +/- 0.23 D, P=0.02) was statistically significant and change of asphericity (0.98 +/-0.07/1.14 +/- -.20, P<0.0001) was highly significant. In eyes with RBT < or =250 microm, the average change of posterior central power (-0.20 +/- 0.10 D vs. -0.08 +/- 0.18 D) was significantly greater than in eyes with RBT >250 microm (P=0.003). The change of posterior corneal power correlated significantly with deltaSEQ (P=0.004) and the RBT (P=0.002). CONCLUSIONS: Increased negative keratometric diopters and oblate asphericity of the posterior corneal curvature suggest that mild "keratectesia" of the cornea may be common early after LASIK. Further stuudies with longer follow-up are required to clarify whether this biomechanical deformation is progressive and whether a residual bed thickness of >250 microm can completely prevent it. 相似文献
The experimental finding of Asian traditional medicine revealed the pharmacological effect of the local application of ghee which was taken from cow butterfat and the rhizomes of Curcuma longa. These materials significantly improved the healing process of the wound. In addition, ancient physicians of Middle East discovered that the powdered rhizomes of Curcuma longa (common turmeric) also had impressive medicinal qualities. Over the centuries, this spice has been used as a pain relieving, anti-inflammatory agent to relieve pain and inflammation in the skin and muscles.
Aim of the study
We decided to mix ghee which was taken from sheep butterfat with the powdered rhizomes of Curcuma longa to formulate a novel cost-benefit material and then, evaluate its potential therapeutic effect on acceleration of surgical wound healing; moreover, this present study was performed to compare the effects of Curcuma longa–ghee formulation and hyaluronic acid on gingival wound healing following surgery.
Materials and methods
Five healthy 3-year-old male beagle dogs were used in this study. They had intact teeth and the clinical and radiographic examination revealed no periodontal disease. Ghee was obtained from the refined sheep butterfat heated to 70 °C mixed with the powdered rhizomes of Curcuma longa and was applied with two different ratios including materials A and B. Randomly, these three materials including hyaluronic acid, materials A and B were applied topically in test regions and then covered with periodontal pack. Histological changes were monitored in days 4 and 7 after operation to evaluate the inflammatory and repair stage of healing process.
Results
We observed significant difference in the inflammatory and repair parameters of the healing process between cases treated with this new formulation and cases of hyaluronic acid application.
Conclusion
The results suggested a positive potential therapeutic effect on surgical wound healing particularly improvement of periodontal treatment consequences after surgery. 相似文献
