类风湿关节炎患者社会支持状况调查分析

Objective To learn the status of social support for patients with rheumatoid arthritis and formulate the theoretical basis for providing physical and psychological nursing.Methods A total of 112 patients with rheumatoid arthritis were investigated with the social support.Results The total score of social support was (36.56±7.97) in the patients with rheumatoid arthritis,which showed significantly different compared with the norm(33.05±3.85 ).Subjective support was influenced by course and hospitalizations,while Objective support was influenced by medical treatment fee and joint function.Marriage,hospitalization and joint function had influenced the utilization degree of support.Conclusions the patients with comprehensive assessment should nurse social support for patients,provide proper social support and guide patients of social support network,develop a new social support network and social support network using active rheumatoid arthritis to strengthen the social support.

类风湿性关节炎患者不依从性用药原因分析及护理

目的:分析造成类风湿性关节炎患者不依从性用药的原因,采取行之有效的护理措施以提高患者的遵医行为。方法:对我科2005年10月～2006年10月门诊及入院186例复治的不依从性用药的类风湿性关节炎患者不依从性用药的原因进行调查和分析。结果:186例复治的类风湿性关节炎患者,不依从性用药的主要原因如下:对自身疾病的重视不足39.25%,担心药物不良反应21.51%,经济困难不能坚持用药15.59%,认为疾病已控制或治愈无需继续用药7.53%。结论:加强对类风湿关节炎相关知识的宣传。建立“指导-合作型“和“相互-参与型“的护患关系,提高类风湿关节炎病人的就医意识,建立社区和家庭的双重监督机制,从而提高类风湿性关节炎病人的遵医率,提高患者的生活质量。     

涉及髋臼顶的髋臼后壁骨折手术治疗效果

目的观察涉及髋臼顶的髋臼后壁骨折手术治疗效果。方法选择我院2011年~2017年髋臼后壁及髋臼顶骨折的36例患者,观察采用Kocher-Langenbeck入路切开复位重建钛板内固定治疗疗效。结果骨折复位按照Matta标准评价:解剖复位24例,复位满意10例,复位欠佳2例,随访时间6~20个月,平均12个月,所有骨折均固定愈合,内固定无松动,骨折移位2例,髋臼顶再骨折1例,评定关节功能参照Epstein标准,关节功能优22例,良8例,一般4例,差2例,结论涉及髋臼顶的髋臼后壁骨折不仅需要解剖复位,还需要对髋臼顶进行坚强的固定。     

布地奈德联合鼻内镜手术治疗慢性泪囊炎

目的 探讨布地奈德鼻喷雾剂联合鼻内镜手术治疗慢性泪囊炎的效果。方法 将慢性泪囊炎患者80例(80眼)纳入前瞻性单盲研究,随机分为布地奈德组(40例)和对照组(40例)。所有患者均接受鼻内镜下鼻腔泪囊吻合术。布地奈德组应用布地奈德鼻喷雾剂喷鼻3个月,对照组应用生理性海水喷鼻3个月。比较2组手术疗效、Lund-Kenndey评分和并发症发生情况。结果 随访6个月时,布地奈德组疗效优于对照组(P<0.05)。术后3个月时,2组Lund-Kenndey评分低于术前,布地奈德组Lund-Kenndey评分低于对照组(P<0.05)。布地奈德组并发症发生率低于对照组(5%vs. 20%,P<0.05)。结论 鼻内镜下鼻腔泪囊吻合术后应用布地奈德鼻喷剂治疗慢性泪囊炎安全有效,手术成功率更高、术后出现吻合口粘连、肉芽肿的概率更小,且不增加术后感染的风险。

承德市群众角膜捐献意愿调查与关联性分析

目的:探讨承德市群众角膜捐献意愿情况及关联性因素.方法:以调查问卷形式,对承德市2 822名普通群众调查.结果:发放问卷2 822份,有效答卷2 625份.大于50%的人赞同角膜捐献并且能够捐献角膜和支持亲友捐献角膜,对捐献并不了解成为影响承德市群众捐献的最主要因素.结论:承德市群众对角膜捐献持肯定、积极的态度,加强角膜捐献相关知识的社会宣传,建立通畅的捐献途径,完善角膜捐献相关的法律、法规,将会促进角膜捐献行为.
