The prediction and subsequent management of aggression by psychiatric inpatients is a crucial role of the mental health professional. This retrospective cohort study examines the predictive validity of 10 static and dynamic risk-of-violence measures and subscales in 37 forensic and 37 civil inpatients residing in a medium- to-low security psychiatric facility for a period of up to 6 months. Retrospective file records were sourced to conduct an AUC analysis of the ROC curve for short- and medium-term follow-up periods. The hypothesis that dynamic measures would be better predictors than static measures over the short term was supported. Albeit to a lesser extent, dynamic measures were still better predictors than static measures over the medium term. This result was seen in both civil and forensic groups. Three previously untested measures were found to predict aggression within the sample. It is recommended that mental health services employ the use of dynamic measures when making short-term risk-of-violence predictions for civil and/or forensic inpatients. 相似文献

A web-based system for rapid multidirectional communication has been created in the Radiology department at San Francisco
General Hospital. The system allows messaging among radiology attendings, residents, and technologists, as well as other members
of the hospital community, such as Emergency Department physicians and nurses. Instead of being tied to a particular workflow,
this system provides a flexible communication infrastructure which can be easily adapted for different functions and user
roles. The system has so far been configured to successfully support the standard “wet reading” workflow, to support marking
and tracking of critical results, as well as multiple educational and quality improvement workflows. In the 19 months of operation,
the system has gained over 1,800 users (virtually all providers at our institution), it has been accessed by radiologists
over 39,000 times and by non-radiologists over 34,000 times. It has become an integral part of the radiology department operations
and non-radiology clinical workflows. Unlike most existing softwares, our system is not a task-specific application, but a
multipurpose communication system. It is able to effectively accommodate multiple workflows and user roles through configuration
(without additional programming). This flexibility has helped this system to be rapidly and widely adopted within our enterprise.
The extended reach of the system enables improved monitoring and documentation of workflows, helping with management decision
making, and quality assurance. We report a successful radiology communication system based on the principles of flexibility
and inclusiveness of users inside and outside the radiology department. 相似文献

Biocompatibility of materials strongly depends on their surface properties. Therefore, surface derivatization in a controllable
manner provides means for achieving interfaces essential for a broad range of chemical, biological, and medical applications.
Bioactive interfaces, while manifesting the activity for which they are designed, should suppress all nonspecific interaction
between the supporting substrates and the surrounding media. This article describes a procedure for chemical derivatization
of glass and silicon surfaces with polyethylene glycol (PEG) layers covalently functionalized with proteins. While the proteins
introduce the functionality to the surfaces, the PEGs provide resistance against nonspecific interactions. For formation of
aldehyde-functionalized surfaces, we coated the substrates with acetals (i.e., protected aldehydes). To avoid deterioration
of the surfaces, we did not use strong mineral acids for the deprotection of the aldehydes. Instead, we used a relatively
weak Lewis acid for conversion of the acetals into aldehydes. Introduction of α,ω-bifunctional polymers into the PEG layers, bound to the aldehydes, allowed us to covalently attach green fluorescent protein
and bovine carbonic anhydrase to the surfaces. Spectroscopic studies indicated that the surface-bound proteins preserve their
functionalities. The surface concentrations of the proteins, however, did not manifest linear proportionality to the molar
fractions of the bifunctional PEGs used for the coatings. This finding suggests that surface-loading ratios cannot be directly
predicted from the compositions of the solutions of competing reagents used for chemical derivatization. 相似文献

The innovative BoneWelding? technology, where ultrasound energy bonds bioresorbable implants to bone, was tested for its feasibility in spine surgery
and its local thermal effects. The three tested concepts consisted of implementation of a resorbable plating system, two converging
polymer pins and suture anchors to the cervical vertebral bodies. Bioresorbable polylactide implants (PLDLLA 70/30) were inserted
ventrally into the third and fourth vertebral body of seven sheep, of which six were sacrificed at 2 months and one sheep
immediately after temperature measurements during implant insertion. Polymer screws were used as controls. Qualitative, semi-quantitative
histological, and quantitative histomorphometrical evaluation showed excellent anchorage of the implants, new mineralized
bone at the implant-bone interface, no inflammatory cell reaction or thermal damage to the adjacent bone in response to the
novel insertion technology. The application of two converging pins, parallel inserted polymer pins, or fusion of the implant
to the polymer plates did not affect the overall excellent tissue tolerance of the technology. Temperature increase during
insertion was noticed but never exceeded 47°C for less than 1 s. The BoneWelding? technology was proven to be safe and easy to apply. 相似文献

A randomized, prospective study compared a long-acting, second-generation cephalosporin, cefonicid (Monocid), with a short-acting, second-generation cephalosporin, cefoxitin (Mefoxin), for cesarean section prophylaxis. One hundred thirty-nine patients were enrolled, with 81 receiving a 1-g intravenous dose of cefonicid after cord clamping and 58 receiving a 2-g dose of intravenous cefoxitin after cord clamping and at 6, 12 and 18 hours postpartum. In those patients receiving cefonicid prophylaxis, endometritis incidence was 17.3% (14 of 81). This finding was not statistically significant (P less than .397) when compared to the 12.1% incidence of endometritis (7 of 58) with cefoxitin. In addition, the febrile morbidity incidence for cefonicid prophylaxis was 23.5% (19 of 81) as compared to 15.5% (9 of 58) for cefoxitin (P less than .25). Because the two drugs appear to be equally efficacious, cefonicid may be the better choice because of its markedly lower cost. 相似文献
