Objective Heart rate has been shown to predict cardiovascular morbidity and mortality in a very reliable and easily accessible manner. The exact definition of the conditions under which heart rate is measured appears to be as crucial as the determinations of blood pressure or plasma catecholamines. It was investigated how accurately heart rate measurements were performed, and conditions were described in 56 studies identified from prominent journals within the Medline database: (a) search phrases were heart rate and rest or resting; (b) publication date was from 1996 to 2001; and (c) publication type was clinical trial.Methods Five conditions were considered as most influential: (a) resting period before measurement; (b) posture of the patient; (c) environmental conditions such as temperature or visual and acoustic stimuli; (d) method used to record heart rate; and (e) data analysis, i.e., derivation from raw data. An average of only 1.7 of those 5 criteria for the determination of heart rate were met in the studies included. Information on conditions of, for example, resting period, or environmental conditions is almost completely lacking.Results and conclusion The data show that a very important risk predictor and treatment target—heart rate—is not reported in a scientifically sufficient manner, even in large trials. Valuable information is lost despite the fact that the investment of adequately defining, controlling, and performing this determination is modest in comparison to the potential gain. It is recommended to standardize heart rate measurements in analogy to that of blood pressure determinations.The experiments comply with the current laws of the country in which they were performed, inclusive of ethics approval.  相似文献

Objective The MDR1-encoded P-glycoprotein (Pgp) represents the best-studied membraneous efflux pump defending the body against xenobiotics. Various polymorphisms (single nucleotide polymorphisms; SNPs) in the MDR1 gene have been identified, and a silent mutation in exon 26 (C3435T) has been correlated with duodenal expression of Pgp, which might affect the disposition of certain drugs. The C3435T SNP has been shown to be linked to another SNP (G2677T/A) in exon 21 which leads to an amino acid exchange. So far, the influence of age on Pgp function has been neglected. As the function of Pgp might be altered in advanced age, we investigated in groups of fit and frail elderly subjects whether the efflux of the Pgp-probe rhodamine 123 from CD56+ natural killer cells was age dependent and whether it was affected by the two SNPs.Methods Leukocytes were isolated from blood of 18 healthy elderly subjects (mean age 69 years) and 20 geriatric frail patients (mean age 78 years) and data compared with 21 previously studied young healthy Caucasian individuals (mean age 33 years). Subjects were homozygous for either CC- or TT-genotype (SNP C3435T) and additionally differentiated according to genotype GG or TT of the SNP G2677T. Using flow cytometry, rhodamine fluorescence was monitored in CD56+ cells.Results In contrast to the young controls, in both elderly populations no significant difference between the CC and TT genotypes (exon 26) could be observed in rhodamine fluorescence. Furthermore, only for the TT genotype (exon 26) did frail elderly demonstrate some reduced Pgp function (P=0.03) if compared with the young healthy subjects. If the three groups were compared independent of the genotype, no age effects were observed. For all assessed genotypes, there was no significant difference between fit and frail elderly subjects.Conclusion Aging and frailty have apparently only a minor impact on this validated cellular Pgp model and it could be assumed that function of Pgp is quite well preserved in patients of advanced age.  相似文献

Rationale Modafinil is a well-tolerated psychostimulant drug with low addictive potential that is used to treat patients with narcolepsy or attention deficit disorders and to enhance vigilance in sleep-deprived military personal. So far, understanding of the cognitive enhancing effects of modafinil and the relevant neurobiological mechanisms are incomplete.Objectives The aim of this study was to investigate the effects of modafinil on working memory processes in humans and how they are related to noradrenergic stimulation of the prefrontal cortex.Methods Sixteen healthy volunteers (aged 20–29 years) received either modafinil 200 mg or placebo using a double blind crossover design. Two computerized working memory tasks were administered, a numeric manipulation task that requires short-term maintenance of digit-sequences and different degrees of manipulation as well as delayed matching task that assesses maintenance of visuo-spatial information over varying delay lengths. The battery was supplemented by standardized paper pencil tasks of attentional functions.Results Modafinil significantly reduced error rates in the long delay condition of the visuo-spatial task and in the manipulation conditions, but not in the maintenance condition of the numeric task. Analyses of reaction times showed no speed-accuracy trade-off. Attentional control tasks (letter cancellation, trail-making, catch trials) were not affected by modafinil.Conclusions In healthy volunteers without sleep deprivation modafinil has subtle stimulating effects on maintenance and manipulation processes in relatively difficult and monotonous working memory tasks, especially in lower performing subjects. Overlapping attentional and working memory processes have to be considered when studying the noradrenergic modulation of the prefrontal cortex.  相似文献

Uveal melanoma is the most frequent primary malignant neoplasm of the eye and has a poor prognosis in metastatic stage. Fotemustine or a combination of gemcitabine and treosulfan has demonstrated some efficacy in metastatic disease. We conducted a phase II trial to assess the second-line activity and toxicity of bendamustine hydrochloride, a nucleoside analogue with alkylating activity. Inclusion criteria were a Karnofsky performance status of > or = 60% and progressive disease during or after first-line chemotherapy. Bendamustine was administered at a dose of 120 mg/m2 on days 1 and 2. Cycles were repeated on day 22. The primary endpoint of the study was the determination of the number of patients achieving an objective response or stable disease. The secondary endpoint was toxicity. Eleven patients were enrolled into the trial. Grade III and IV toxicity consisted of anaemia, thrombocytopenia and leucocytopenia in two, one and two patients, respectively. No grade III or IV non-haematological toxicity was observed. According to Response Evaluation Criteria in Solid Tumours (RECIST), all patients showed progressive disease. We conclude that bendamustine is ineffective as second-line chemotherapy for metastatic uveal melanoma.  相似文献

B-cell chronic lymphocytic leukemia (B-CLL) is phenotypically characterized by cell surface co-expression of CD19, CD20, CD5, and CD23. However, the concomitant presence of other antigens distinctive of a particular leukocyte subset, e.g. T cells, is an unusual finding in B-CLL. In the present report, a case of B-CLL with aberrant expression of the T-cell-associated antigen CD8 is described. Flow cytometric analysis of the patient's cells demonstrated lack of CD38 expression, and cytogenetic analysis by FISH did not show any abnormalities of chromosomes 17p, 11q, and 12. Furthermore, the B-CLL cells expressed only low levels of the tyrosine kinase ZAP-70, which was in agreement with the mutated status of the immunoglobulin heavy-chain variable-region gene (IgVH). In summary, the determined profile of prognostic markers together with the highly stable and indolent disease in the described patient suggest a good prognosis of B-CLL with aberrant CD8 expression.  相似文献

Differentiation of neurospheres from the enteric nervous system   总被引:8,自引:0,他引:8  
The enteric nervous system (ENS) derives from neural crest cells, which migrate from the neural tube into the developing gut. The neuronal and glial precursor cells migrate mainly from the oral towards the anal end of the gastrointestinal tract. So far, knowledge about the multipotent influences upon the ENS development, especially its neurotrophic support, derives mainly from knock-out models. The in vitro technique of isolating enteric neuronal precursor cells allows to study the effects of various factors upon their appropriate development in more detail. We therefore adapted the method of growing neurospheres, which are agglomerates of neuronal precursor cells and differentiated neurones and glial cells, from the central nervous system (CNS) for the ENS. The gut of NMRI mice at E12 were dissected, mildly dissociated and plated in 25-cm2 culture flasks. The cultures were maintained in N1 supplemented DMEM/F12 medium with the appropriate neurotrophin cocktails (bFGF, GDNF, Neurturin, CNTF). After several days in culture most of the cells die, while the surviving cells form clusters from which domes, and later spheres arise. The spheres could be harvested and processed for further experiments. First investigations revealed, that the amount of precursor cells was much less in enteric neurospheres as seen in corresponding cultures from the CNS. We found about 43% HNK-1-NCAM+ in enteric and approximately 90% Nestin-+ cells in midbrain neurospheres. Differentiation studies of the enteric neurospheres showed that especially ciliary neurotrophic factor (CNTF) increased the number of enteric neurones (PGP positive), while the amount of HNK-1 precursor cells decreased under the influence of all tested neurotrophins but GDNF. The culture of the freshly dissociated enteric neurospheres in a three-dimensional matrix yielded a secondary network which allows to investigate the pattern formation of the ENS. The generation of enteric neurospheres and the following differentiation and 3D culture in vitro can increase our knowledge of the amount and time point of neurotrophic as well as the ECM-protein influence upon the appropriate development of the ENS.  相似文献
