The underlying purpose of this commentary and position paper is to achieve evidence-based recommendations on prevention of work-related musculoskeletal disorders (MSDs). Such prevention can take different forms (primary, secondary and tertiary), occur at different levels (i.e. in a clinical setting, at the workplace, at national level) and involve several types of activities. Members of the Scientific Committee (SC) on MSDs of the International Commission on Occupational Health (ICOH) and other interested scientists and members of the public recently discussed the scientific and clinical future of prevention of (work-related) MSDs during five round-table sessions at two ICOH conferences (in Cape Town, South Africa, in 2009, and in Angers, France, in 2010). Approximately 50 researchers participated in each of the sessions. More specifically, the sessions aimed to discuss new developments since 1996 in measures and classification systems used both in research and in practice, and agree on future needs in the field. The discussion focused on three questions: At what degree of severity does musculoskeletal ill health, and do health problems related to MSDs, in an individual worker or in a group of workers justify preventive action in occupational health? What reliable and valid instruments do we have in research to distinguish ??normal musculoskeletal symptoms?? from ??serious musculoskeletal symptoms?? in workers? What measures or classification system of musculoskeletal health will we need in the near future to address musculoskeletal health and related work ability? Four new, agreed-upon statements were extrapolated from the discussions: 1. Musculoskeletal discomfort that is at risk of worsening with work activities, and that affects work ability or quality of life, needs to be identified. 2. We need to know our options of actions before identifying workers at risk (providing evidence-based medicine and applying the principle of best practice). 3. Classification systems and measures must include aspects such as the severity, frequency, and intensity of pain, as well as measures of impairment of functioning, which can help in prevention, treatment and prognosis. 4. We need to be aware of economic and/or socio-cultural consequences of classification systems and measures. 相似文献

Background: T1-shortening contrast media are routinely used in magnetic resonance (MR) examinations for the diagnosis of brain tumors. Although some studies show a benefit of 3 Tesla (T) compared to 1.5T in delineation of brain tumors using contrast media, it is unclear which pulse sequences are optimal.
Purpose: To compare gadopentetate dimeglumine (Gd-DTPA)-induced signal enhancements in rat brain C6 glioma in the thalamus region among different pulse sequences in 3T MR imaging.
Material and Methods: Five rats with a surgically implanted C6 glioma in their thalamus were examined. T1-weighted brain images of the five rats were acquired before and after Gd-DTPA administration (0.1 mmol/kg) using three clinically available pulse sequences (spin echo [SE], fast SE [FSE], fast spoiled gradient echo [FSPGR]) at 3T. Signal enhancement in the glioma (ET) was calculated as the signal intensity after Gd-DTPA administration scaled by that before administration. Pulse sequences were compared using the Tukey-Kramer test.
Results: ET was 1.12±0.05 for FSE, 1.26±0.11 for FSPGR, and 1.20±0.11 for SE. FSPGR showed significantly higher signal enhancement than FSE and comparable enhancement to SE.
Conclusion: FSPGR is superior to FSE and comparable to SE in its ability to delineate rat brain C6 glioma in the thalamus region. 相似文献

Low back pain is an enormous clinical and public health problem. When we use spinal instrumentation, measurements of spinal
and leg alignment in the standing position are thought to be important. Knee joint pain has also become an enormous clinical
problem in the increasing elderly population. However, the correlation between spinal alignment, particularly sacral inclination,
and knee joint pain is not clear. We examined the correlation between lumbar lordosis, sacral inclination, and patellofemoral
joint pain in elderly subjects. Three hundred and ninety-nine people aged 50–85 years were examined. Clinical findings and
physical status were determined. Measurements and determination of total lordosis from L1 to S1 and sacral inclination were
made from standing radiographs. The knee joints were evaluated by using the standing knee flexion angle, radiographs, and
the patellofemoral (PF) joint grinding test. Thirty-nine percent of subjects were excluded because they had definite osteo-arthritis
at the femorotibial joints. Fifty-eight percent of the subjects had felt low back pain within the previous 3 months, and 16%
of the subjects complained of PF joint grinding pain. Sixteen percent of the subjects showed knee flexion when standing. There
was a significant difference in sacral inclination between the groups with and without PF joint grinding pain (P < 0.01). Sacral inclination was approximately 5° less in the knee flexion group. A correlation between sacral inclination
and PF joint pain is defined, and its prevalence in the elderly is reported. We speculate that this phenomenon is caused by
changing of lumbar alignment. In addition, we think this is a new pathological concept that we call the knee – spine syndrome.

Adolescent idiopathic scoliosis (AIS) is a spinal deformity most commonly arising in apparently healthy girls around puberty. AIS has a strong genetic predisposition. Several genetic associations between AIS and single nucleotide polymorphisms (SNPs) have been reported; common SNPs in the genes for matrilin 1 (MATN1), melatonin receptor 1B (MTNR1B), tryptophan hydroxylase 1 (TPH1), and insulin‐like growth factor 1 (IGF1) are reported to be associated with AIS in Chinese. However, these associations have not been replicated so far. To confirm the associations, we compared these SNPs with AIS predisposition and curve severity in a population of Japanese females consisting of 798 AIS patients and 1,239 controls. All the subjects were genotyped using the PCR‐based Invader assay. We found no association of any of the SNPs with AIS predisposition or curve severity. Considering the statistical power and sample size of the present study, we concluded that these SNPs are not associated with either AIS predisposition or curve severity in Japanese. © 2011 Orthopaedic Research Society Published by Wiley Periodicals, Inc. We have detected an unusual allele at the vWF-Kimpton (vWF-K) loci in the DNA of a child (genotype: 1415) in a paternity trio case. One allele of the child's DNA was found to derive neither from the mother (1214) nor from the putative father (1314), whose paternity was established not only by conventional polymorphic markers (probability 0.99999) but also by the other 10 STRs and the D1S80 and HLA DQ alpha loci. Two STRs flanking vWF-K comprise vWF haplotypes, which allow the parental origin of the unusual allele to be determined. Sequencing of clones encompassing the three STRs showed that the unusual allele segregated with the paternal haplotype. The de novo allele of the child thus seemed to be generated from the longer allele (14) by gaining a single unit (TCTA) through slippage replication. 相似文献

18-year-old male was referred to our hospital due to persistent cough. The patient was admitted for the investigation of the abnormal shadow on a chest X-ray and chest computed tomography (CT). Chest CT showed a 2.5 cm nodular shadow in the right lower lobe. Bronchofiberscopy revealed the polypoid lesion at the right lower lobe bronchus obstructing the entire lumen of B8-10. The tumor surface was smooth and rich in small vessels. Right lower lobectomy was peformed. The diagnosis of schwannoma was confirmd with the S-100 positive immunohistochemical stain. Bronchial schwannoma is relatively rare disease; less than 90 cases have been reported with respect to schwannoma of case report in Japan. 相似文献

STUDY DESIGN: Retrospective case series. OBJECTIVE: To evaluate our recent treatment strategy for intramedullary spinal cord tumors. SETTING: Department of Orthopaedic Surgery, Keio University, Japan. METHODS: We reviewed 68 cases of intramedullary tumors (ependymoma, 33; astrocytoma, 23; hemangioblastoma, 12), treated surgically between 1994 and 2003. There were 42 males and 26 females whose mean age at the time of surgery was 43 years. The mean follow-up period was 6.2 years. The tumor malignancy grade according to the WHO classification was astrocytoma grade I, 3; grade II, 8 (low-grade: 11 cases); grade III, 10; grade IV, 2 (high-grade: 12 cases). All ependymomas were grade II. Three of the 12 hemangioblastomas were associated with von Hippel-Lindau disease. RESULTS: Total excision was achieved in 90% of the ependymomas and functional improvement was obtained when the preoperative neurological deficit was mild. Approximately 50% of low-grade astrocytomas could be totally excised with favorable survival outcomes, suggesting that total excision should be attempted for low-grade astrocytomas. However, total excision of high-grade tumors was difficult and the functional outcomes were poor. Cordotomy should be considered in patients with a thoracic high-grade astrocytoma. Total resection was possible in 92% of hemangioblastoma, and the functional outcomes were good, however, more attention should be paid for tumors with feeding arteries on the ventral side and for those associated with von Hippel-Lindau disease. CONCLUSIONS: Predictors of good surgical outcome for intramedullary spinal cord tumors were histological grades of the tumors, surgical margins, and neurological status of the patient before surgery. 相似文献

OBJECTIVE: Reoperative coronary artery bypass grafting with cardiopulmonary bypass tends to cause a higher mortality and morbidity than the primary operation. The purpose of this study was to discuss the effectiveness and safety of a minimally invasive coronary artery bypass procedure for patients who had previously undergone coronary artery bypass surgery. METHODS: We performed redo single coronary artery bypass grafting to the left anterior descending coronary artery in 9 patients and to the right coronary artery in 3 patients using minimally invasive cardiac surgery. The graft to the left anterior descending coronary artery was taken from the left internal thoracic artery in 5 patients, the right gastroepiploic artery in 3 patients, and from the saphenous vein in the other 1 patient. The graft to the right coronary artery was from the right gastroepiploic artery in all 3 patients. RESULTS: All grafts were patent. There was no major postoperative complication and no surgical or hospital death except one late death. CONCLUSIONS: In selected patients, we could safely and completely perform coronary artery bypass re-grafting to the left descending coronary artery or right coronary artery using a minimally invasive operation. 相似文献

Extended resection, comprising extended right hemicolectomy, splenectomy, and distal pancreatectomy, has been advocated for
carcinoma of the splenic flexure because the lymphatic drainage at this site is variable. The present study addresses the
problems associated with selecting the most appropriate operative procedure to achieve cure of splenic flexure cancers. We
conducted a retrospective review of 27 patients with splenic flexure cancer who under-went curative resection. Left partial
colectomy was performed in 20 patients and partial resection of the transverse/descending colon was performed in 7 patients.
The combined resection of adjacent organs due to tumor adherence was performed in three patients. The spleen and distal pancreas
were the organs most frequently resected among a collective total of six adjacent organs. The median duration of follow-up
was 60.9 months after resection for splenic flexure cancer. No patient developed local recurrence. There was no significant
difference in 5-year survival between patients with splenic flexure cancers and those with colon cancers at other sites. In
conclusion, splenic flexure cancer resected by left partial colectomy or partial resection of the transverse/descending colon
without routine extended resection was not associated with a worse prognosis than colon cancers at other sites.
