The growth and aging of the population of Hawai‘i with a high incidence of diabetes mandates a need for more effective strategies to manage the healing of complicated wounds. Maggot debridement therapy (MDT) is one alternative utilized with successful results. Observations have indicated that maggots have the ability to debride wound beds, provide anti-microbial activity and also stimulate wound healing in diabetic patients. None of the patients refused MDT due to aversion of this treatment modality and the majority of patients had minimal discomfort. In 17 of 23 patients with multiple co-morbidities, the treatment of their complex diabetic wounds by MDT resulted in improvement or cure. Maggot debridement therapy is an effective treatment of diabetic wounds.  相似文献

Sensory impairments are common in older adult populations and have notable impacts on aging outcomes. Relationships between sensory and cognitive functions have been clearly established, though the mechanisms underlying those relationships are not fully understood. Given the growing burden of dementia, older adults with sensory deficits are an important and growing population to study in cognitive aging research. Yet, cognitive research sometimes excludes those with uncorrected significant/severe sensory deficits and often poorly or inconsistently assesses those deficits. Observational and interventional studies that exclude participants with sensory deficits will be limited in their generalizability to the narrower subset of the older adult population without vision or hearing impairment and may be missing an opportunity to study a growing population of older adults at higher risk of cognitive impairment. Strategies exist for adapting cognitive testing instruments, and inroads could be made into collecting normative data to inform ongoing research. Bringing together psychometricians with researchers who specialize in vision and hearing impairments could launch highly innovative research on both measurement methods and cognitive disease etiology, as sensory organs provide readily accessible neuronal and vascular beds that may show pathology earlier and elucidate innovative screening opportunities for early signs of cognitive disease.  相似文献

BackgroundPrior research has demonstrated that the needs of the disability community have not been met during public health emergencies. The COVID-19 pandemic has exacerbated existing inequities for many populations including people with disabilities, and data is needed to develop inclusive public health response policies.ObjectiveTo identify how COVID-19 has uniquely impacted the lives of adults with disabilities.Methods38 participants were recruited through disability advocacy groups and social media. Semi-structured virtual focus groups were conducted with adults (≥18 years) who self-identified as having a disability. Focus groups were conducted for each of six disability sub-groups: vision, hearing, mobility and physical, mental health, cognitive, intellectual, and developmental, and chronic illness. Using inductive coding, major themes were identified and compared across the disability sub-groups.ResultsThree major themes and thirteen sub-themes were identified from the focus groups. The three major themes comprised: new problems created by the pandemic, obstacles in daily life that were exacerbated by the pandemic, and broader changes to accessibility and disability identity. Sub-themes such as difficulty with COVID-19 testing and regular medical care were reported by participants of all disability sub-groups, while other sub-themes like direct care needs and medical rationing were reported by participants from a subset of the disability sub-groups.ConclusionsThese results indicate how the COVID-19 pandemic unequally impacts disabled people. The participants indicated that to fully address their needs, disability perspectives must be included in the public health pandemic response. As new research shows that COVID-19 can cause long-term disability, the urgency to ensure the disability community is part of public health policies will increase.  相似文献

Objectives: To assess the longitudinal relationship between visual acuity (VA) and depressive symptoms (DSs) among older adults.

Methods: A population-based sample of 2520 white and black individuals aged 65–84 years in 1993--1995 was assessed at baseline and at two, six, and eight years later. Presenting and best-corrected VA was assessed using early treatment diabetic retinopathy study chart. DSs were assessed using the severe depression subscale of General Health Questionnaire 28. Latent growth curve models estimated VA and DS trajectories and age-adjusted associations between trajectories.

Results: Best-corrected logMAR VA worsened over time (slope = 0.026, intercept = 0.013, both p < 0.001). No change in DS over time was observed (slope = ?0.001, p = 0.762; intercept = 1.180, p < 0.001). However, a small change in DS was observed in participants who completed all rounds (slope = 0.005, p = 0.015). Baseline VA levels correlated with baseline DS levels (r = 0.14, p < 0.001). Baseline DS was associated with best-corrected VA change (r = 0.17, p = 0.01). Baseline best-corrected VA was not associated with DS change (r = 0.017, p = 0.8). Best-corrected VA change was not significantly associated with DS change (r = ?0.03, p = 0.7).

Discussion: DSs are significantly associated with VA cross-sectionally, and persons with higher baseline DS scores were more likely to experience worsening VA over time. The complex relationship between visual impairment and DS suggests the need for a continued effort to detect and treat both visual decline and severe DSs in a growing elderly population.  相似文献
