Background

India accounts for approximately 10 million orthopaedically handicapped children and adults with limb deformity. Ilizarov ring fixator could treat most of these deformities.

Methods

Twenty cases of deformities of lower limb managed with Ilizarov technique during period between March 2001 and February 2003 were studied.

Results

55% were in the age group of 11-30 years. Out of the 20 cases studied, 6 were congenital talipes equino varus, 8 were fixed flexion deformity of knee, 4 were equines deformity of the ankle and 2 were malunited fracture shaft of tibia.4 patients who had recurrence were operated for fixed flexion deformity of the knee. The main complication encountered was pin tract infection, which was seen in 15(75%) cases. In 16(80%) cases, the results were excellent with no recurrence of deformity and patients were able to walk independently. In 4 (20%) cases, recurrence was mild to moderate (10 to 20) but all of them were able to ambulate idependently and carry out their routine activities.

Conclusion

Ilizarov ring fixator is a superior compared to conventional methods for correction of deformities of lower limb.Key Words: Ilizarov method, Ligamentotaxis, Distraction  相似文献

Medical specialists have--notwithstanding governmental policy--acquired a firmly established position in policy-making in Dutch hospitals. In all general hospitals the Executive Board and the Staff Executive frequently and systematically negotiate their interests. Three conclusions may be drawn on the basis of the research reported on in this article. In the first place, it is the primary objective of the medical staff executive to protect the interests of the medical domain against managerial dominance. In the second place, developments suggest that the parties are slowly but surely switching to the joint regulation of overlapping interests. Finally, the trend towards joint regulation is an enormous challenge for medical staff governance.  相似文献

AIM: Conflicting evidence exists on whether in vivo morphological characteristics can distinguish Ovarian Surface Epithelium (OSE) of ovaries obtained from women with and without a predisposition to develop female adnexal (ovarian and fallopian tube) carcinoma. This study aims to detect differences in growth potential and morphology that are maintained or specifically expressed in vitro. STUDY DESIGN: Ovarian surfaces were scraped to retrieve OSE cells from 56 women at hereditary high risk for female adnexal carcinoma, of whom 33 are BRCA1 and four are BRCA2 mutation carriers (Predisposed OSE, POSE) and from 26 women without such risk (Non Predisposed OSE, NPOSE). Number of passages and total cell yield until last passage, as well as morphology was compared between both groups. To confirm morphology, the expression of epithelial, mesothelial, and fibroblast markers was assessed. RESULTS: Both POSE and NPOSE cultures displayed similar growth potential and morphology. The expression of epithelial markers cyto-keratins 7 and 8 was similar between both groups. Only in cultures in which cells did not uniformly exhibit these markers, the percentage of cells expressing these markers was significantly lower at last passage when compared to the initial culture. In these latter cultures, cells that were morphologically indistinguishable from fibroblasts were observed. Mesothelial marker calretinin was expressed in 75% of cells of both POSE and NPOSE cultures and correlates with cyto-keratins 7 and 8 expression. CA 125 expression was equally low in POSE and NPOSE cultures (4.3%). Fibroblast markers FSM and vimentin were expressed in 100% and collagen IV was expressed in 16% of cells in all cultures. CONCLUSION: OSE cells derived from women with a hereditary predisposition to develop female adnexal cancer possess similar in vitro characteristics as OSE from women without this predisposition. On basis of our results, it seems advisable to study only 100% cyto-keratins 7 and 8 positive OSE cultures, since contamination of fibroblasts in some primary OSE cultures cannot be ruled out.  相似文献

Meniscus replacement by a polymer meniscus prosthesis in dogs resulted in generation of new meniscal tissue. HYPOTHESIS: Optimal functioning of the prosthesis would involve realistic deformation and motion patterns of the prosthesis during knee joint motion. STUDY DESIGN: Controlled laboratory study. METHODS: The movements of the meniscus were determined during knee joint flexion and extension with and without internal and external tibial torque by means of roentgen stereophotogrammetric analysis. Subsequently, the meniscus in 6 human cadaveric knee joints was replaced by a meniscus prosthesis. RESULTS: All different parts of the meniscus showed a posterior displacement during knee joint flexion. The anterior horn was more mobile than the posterior horn. The prosthesis mimicked the movements of the meniscus. However, the excursions of the prosthesis on the tibial plateau were less. The knee joint laxity was not significantly higher after replacement with the meniscus prosthesis. CONCLUSIONS: The prosthesis approximated the behavior of the native meniscus. Improvement in both the gliding characteristics of the prosthetic material and the fixation of the prosthesis may improve the function. CLINICAL RELEVANCE: The meniscus prosthesis needs to be optimized to achieve a better initial function in the knee joint.  相似文献

Reproducible measurements of the range of motion are an important prerequisite for the interpretation of study results. The digital inclinometer is considered to be a useful instrument because it is inexpensive and easy to use. No previous study assessed inter-observer reproducibility of range of motion measurements with a digital inclinometer by physical therapists in a large sample of patients.  相似文献

The decision to treat a patient is often based on the results of randomised controlled trials (RCTs). These important investigations inform physicians and patients with regard to the chance of favourable results of treatment and the risks of adverse reactions. However, research has shown that especially the smaller RCTs in which no or even an adverse effect of new interventions is found run a relatively high risk of remaining unpublished, which leads to publication bias and an overestimate of the efficacy of an intervention. This is undesirable. The best way to identify possible publication bias and reduce its negative effects is to register RCTs from their starting date onwards in a prospective and publicly accessible register. Other motives for such a register are the prevention of duplication of research work and its financial support, and informing patients. Currently, a prospective Dutch national trial register is being developed by the Dutch Cochrane Centre. This is not only of importance to researchers and patients, but also to grant-providing bodies and the editors of medical scientific journals.  相似文献

BACKGROUND: The most widely used histologic grading system for endometrial carcinoma is the three-tiered International Federation of Gynecology and Obstetrics (FIGO) system. Although FIGO grading has significant predictive value, the reproducibility of Grade 2 is limited. Recently, a binary grading system was proposed based on the amount of solid growth, the pattern of myometrial invasion, and the presence of tumor cell necrosis. The authors analyzed and compared the prognostic significance and the interobserver variability of both grading systems and of the three criteria for the binary grading system. METHODS: Eight hundred patients with Stage I-III endometrioid endometrial carcinoma were reviewed and graded independently by two pathologists according to the three-tiered FIGO grading system and the novel binary grading system. RESULTS: The interobserver agreement for both systems was moderate, with 70% and 73% agreement rates for the FIGO (kappa = 0.41) and binary (kappa = 0.39) grading systems, respectively. When converting the FIGO grading system into an artificial, 2-tiered grading system (Grade 3 vs. Grades 1-2), the agreement was much better (agreement rate, 85%; kappa = 0.58). Of the 3 criteria for the binary grading system, amount of solid growth (< or = 50% vs. > 50%) had the greatest reproducibility (agreement rate, 80%; kappa = 0.50). Both the 2-tiered FIGO grading system and the binary grading system were significant predictors of local recurrence, distant recurrence, and disease-specific survival (hazard ratios [HRs]: 1.7, 2.5, and 2.6, respectively, for FIGO and 2.1, 4.1, and 3.8, respectively, for the binary grading system). The amount of solid growth also was a strong prognostic factor for these three endpoints (HRs: 2.4, 3.9, and 3.8, respectively). CONCLUSIONS: Both the binary grading system and the FIGO grading system had strong prognostic significance. Their reproducibility, however, was limited. A simple architectural binary grading system that divided tumors into low-grade lesions and high-grade lesions based on the proportion of solid growth (< or = 50% or > 50%) had superior prognostic power and greater reproducibility.  相似文献
