The World Health Organization considers adolescence to be the period between 10 and 20 years during which individuals undergo significant psychosexual and psychosocial development. The needs an well-being of people, especially girls, in this age group in India, however, are neglected. Almost 25% of India's population is comprised of girls below 20 years of age. Adolescent pregnancies make up 10-15% of the total and may be largely attributed to early marriage, but these girls are prepared neither physically nor emotionally for pregnancy and motherhood. Adolescents who marry between aged 15-19 will nonetheless bear on average 6-7 children over the course of their lives. In fact, 80% of women's time between the ages 15 and 35 is spent either being pregnant or nursing offspring. Sections discuss nutritional deprivation and poor fetal growth, obstetric risks, and maternal and infant mortality, literacy and school dropouts, and psychosocial aspects of adolescent pregnancy and motherhood. Intervention programs could aim to increase total food intake, prevent against iron deficiency anemia, provide education and health care for girls aged 10-15, promote the use of contraception, register marriages and provide family life education, and involve boys in the education process. While the marriage age needs to be increased, this change should not be legislated. Change must instead result from education syllabi at the school level.  相似文献

  1. Inhibitory modulation of sympathetic nerve function may have a favourable impact on the progression of congestive heart failure. Nepicastat is a novel inhibitor of dopamine-β-hydroxylase, the enzyme which catalyses the conversion of dopamine to noradrenaline in sympathetic nerves. The in vitro pharmacology and in vivo catecholamine modulatory effects of nepicastat were investigated in the present study.
  2. Nepicastat produced concentration-dependent inhibition of bovine (IC50=8.5±0.8 nM) and human (IC50=9.0±0.8  nM)dopamine-β-hydroxylase. The corresponding R-enantiomer (RS-25560-198) was approximately 2–3 fold less potent than nepicastat. Nepicastat had negligible affinity (>10 μM) for twelve other enzymes and thirteen neurotransmitter receptors.
  3. Administration of nepicastat to spontaneously hypertensive rats (SHRs) (three consecutive doses of either 3, 10, 30 or 100 mg kg−1, p.o.; 12 h apart) or beagle dogs (0.05, 0.5, 1.5 or 5 mg kg−1, p.o.; b.i.d., for 5 days) produced dose-dependent decreases in noradrenaline content, increases in dopamine content and increases in dopamine/noradrenaline ratio in the artery (mesenteric or renal), left ventricle and cerebral cortex. At the highest dose studied, the decreases in tissue noradrenaline were 47%, 35% and 42% (in SHRs) and 88%, 91% and 96% (in dogs) in the artery, left ventricle and cerebral cortex, respectively. When tested at 30 mg kg−1, p.o., in SHRs, nepicastat produced significantly greater changes in noradrenaline and dopamine content, as compared to the R-enantiomer (RS-25560-198), in the mesenteric artery and left ventricle.

Administration of nepicastat (2 mg kg−1, b.i.d, p.o.) to beagle dogs for 15 days produced significant decreases in plasma concentrations of noradrenaline and increases in plasma concentrations of dopamine and dopamine/noradrenaline ratio. The peak reduction (52%) in plasma concentration of noradrenaline and the peak increase (646%) in plasma concentration of dopamine were observed on day-6 and day-7 of dosing, respectively.
  5. The findings of this study suggest that nepicastat is a potent, selective and orally active inhibitor of dopamine-β-hydroxylase which produces gradual modulation of the sympathetic nervous system by inhibiting the biosynthesis of noradrenaline. This drug may, therefore, be of value in the treatment of cardiovascular disorders associated with over-activation of the sympathetic nervous system, such as congestive heart failure.

Background

99mTc-labeled teboroxime shows high myocardial extraction in both in vivo animal and in vitro cell culture and isolated heart studies. Whereas in vivo studies show rapid myocardial clearance of teboroxime, in vitro cell culture and isolated heart studies show slower washout comparable to that of201Tl. Binding of teboroxime to blood components may contribute to these conflicting results.

Methods and Results

We measured teboroxime extraction in the isolated blood-perfused rabbit heart after injection in saline solution, brief incubation in red blood cell perfusate, or 4-hour incubation with human red blood cells. Teboroxime in saline solution showed high extraction (Emax=0.89±0.02; Enet=0.69±0.02), whereas brief incubation in perfusate (Emax=0.60±0.06; Enet=0.48±0.05) or prolonged incubation with human red blood cells (Emax=0.43±0.09; Ene=0.38±0.07) resulted in reduced extraction. Teboroxime clearance was similar for all groups and was slower than201Tl clearance. Analysis of total residual cardiac teboroxime (comparable to external imaging) showed that teboroxime clearance was biexponential. Reduced extraction of teboroxime in red blood cells resulted in an increased size of the rapidly clearing (unextracted) fraction, giving the appearance of rapid myocardial washout.

Conclusions

Teboroxime has a high myocardial extraction. Binding to blood components reduces teboroxime extraction and increases the rate of cardiac teboroxime clearance.  相似文献

The myocardial deposition of radiolabeled perfusion agents permits the noninvasive assessment of regional coronary blood flow. The design of imaging protocols and the optimal interpretation of clinical perfusion studies are based on an understanding of the kinetics of blood-tissue exchange for these compounds. Thallium 201 and the technetium 99m-labeled compounds sestamibi, teboroxime, and tetrofosmin show differing myocardial extraction and retention. This review focuses on studies that used cell culture, isolated heart, and intact animal models that form the basis of our current understanding of the myocardial kinetics of these imaging agents.  相似文献

Episodic Vertigo Comprises a large group of patients seen in any neurootology unit. A total of 118 subjects were reviewed who attended the neurootology unit. Vertigo of shorter duration was noted in 78 and of longer duration in 40 cases. 51 cases showed bilateral neurosensory deafness, 15 cases had bilateral and another one case had unilateral conductive loss. Spinovestibular tests were abnormal in all cases of vertigo of short duration but only 29 cases showed abnormal reflex in patients with a long duration of illness. Thermal vestibulometry revealed hyporeflexia in 33 cases, directional preponderance in 30 cases and 4 cases showed hyperreflexia 28 cases had peripheral lesions while 81 cases had central lesions. The acute cases showed significant improvement on follow up except cases with abnormality in thermal vestibulometry . The cases with positional nystagmus showed improvement with head and body balance exercises expect in those who received treatment with vestibulosedative and vestibular suppersive drugs. Hearing loss remained the same except in patients with middle ear disorders.  相似文献

Of 613 children evaluated in a village in Haryana 94 (15.3%) were observed to have chronic suppurative otitis media (CSOM). Fifty eight (61.7%) children had hearing impairment. CSOM contributed to 71.6% of the hearing impaired (58/81). On analysis of association of CSOM with literacy and socio-economic status of mothers, and age, sex, and upper respiratory tract infections (URI) in children positive correlation was observed only with URIs (P<0.001). Literacy and socio-economic status of the mothers did not correlate significantly with knowledge about treatment seeking, and ear cleaning practices, probably due to the narrow range of incomes and literacy levels. An intervention program consisting of play, demonstrations, health charts and slogans, and aural cleaning and antibiotic drops was introduced.  相似文献

Neurofibrillary tangles are described in Guamanian and post-encephalitic forms of motor neuron discase (MND) but not in sporadic MND. We report the neuropathological findings in a 79-year-old man who died after a 1-year history of MND without extrapyramidal features or dementia. There was no family history of neurological disease and he had not visited Guam. The spinal cord showed loss of anterior horn cells, and skeletal muscle typical changes of denervation. The brain appeared macroscopically normal but histology revealed many neurofibrillary tangles, particularly in medial temporal lobe structures, insula, nucleus basalis, hippocampus, oculomotor nucleus, raphe nuclei and locus ceruleus. Neurofibrillary tangles were not seen in the primary motor cortex, which appeared histologically unremarkable. Occasional tangles were present in the substantia nigra and pontine nuclei. None were seen in the cerebellum, medulla or spinal cord. The tangles were argyrophilic, and, in sections stained with thioflavin-S, both the intracellular and the extracellular tangles fluoresced strongly under ultraviolet light. The intracellular neurofibrillary tangles reacted strongly with an antibody to tau protein, and only occasional tangles showed weak ubiquitin immunoreactivity. Scattered neuropil threads were present in the cortex in the areas of neurofibrillary tangle formation. No plaques were present in any part of the brain and no A4/ protein immunoreactivity was detected. Ultrastructural examination revealed Alzheimer-type neurofibrillary tangles composed of paired helical filaments. The present findings further extend the spectrum of diverse neurological disorders associated with neurofibrillary tangles.  相似文献

Ten preserved biopsy suspensions from leprosy patients were inoculated in biphasic medium and incubated alternatively in refrigerator and at 37 °C. The cultures were observed every week for 3 months. The typical actinomycetoid growth appeared within 3-5 days. Microscopically, two types of cysts (unstained and dark) were seen along with oval cells. Dark cysts showed developement of irregular septae or cracks breaking the mass into irregular quadrangular pieces. Granular acid-fast material could be made out inside the cracks and around the cysts. Oval cells were seen either independently or organised around pink homogeneous material. The oval cells were stained blue or pink. Some of them showed both blue and pink shades.  相似文献
