The microorganisms associated with mandibular third molar pericoronitis were investigated using direct microscopy and anaerobic culture method. The pericoronal pouch was sampled with paper points in A) 8 patients without mandibular third molar pericoronitis and B) 6 patients with mandibular third molar pericoronilis. Under the microscope, the microflora was found to be a complex mixture comprising gram-positive and gram-negative cocci, rods and filaments (including fusiform and curved rods), motile rods and spirochetes. Significantly higher proportions of motile, gram-negative rods were found in group B than in group A. The predominant cultivable microflora of 9 samples: A (4) and B (5) comprised several species of facultative and obligate anaerobic bacteria, namely Peptostreptococcus. Streptococcus, Actinomyces, Eubacterium, Propionibacterium, Veillonella, Porphyromonas, Prevotella, Bacteroides, Fusobacterium, Campylobacter, Staphylococcus, Stomatococcus, Lactobacillus, Neisseria, Capnocytophaga, Haemophilia, Selenomonas and Centipeda species. The microflora in pericoronitis appeared similar to that of diseased periodontal pockets.  相似文献

PURPOSE: The purpose of this study was to compare the emotional effects of tooth loss in three edentulous populations. MATERIALS AND METHODS: A questionnaire study involved 142 edentulous subjects undergoing routine prosthodontic care at Guy's, King's and St Thomas's Dental Institute, London; the Dental School, Dundee, Scotland; and the Faculty of Dentistry, University of Hong Kong. Data were analyzed using the chi-squared test. RESULTS: Difficulty in accepting tooth loss was a relatively common experience (44%) in all groups, with almost half feeling that their confidence had been affected. The majority (66%) felt that their choice of food was restricted and that the overall eating experience was less enjoyable, particularly the Hong Kong group. A significant proportion of the participants were concerned about their appearance without dentures, although the trend was less marked in Hong Kong. Forty-three percent felt that they were not adequately prepared for tooth loss, although the Hong Kong group was least concerned. CONCLUSION: In general, the emotional effect of tooth loss was significant in all groups. The restrictions on daily activities were generally greater in the Hong Kong group. However, this group was much less inhibited by denture wearing. The differences observed in the Hong Kong Chinese are most likely due to different cultural values and expectations associated with these aspects of daily living.  相似文献

OBJECTIVES: To investigate the effectiveness of two restorative materials for reinforcing thin-walled roots and their microtensile bond strengths to root canal dentin. METHODS: Twenty-one decoronated maxillary central incisor roots were root filled and the canals enlarged to leave approximately 1.0 mm thick dentin walls. The roots were distributed randomly to three equal groups. Group 1 (control): a large tapered cast post-core was fabricated. Group 2 (BIS-CORE): a thick layer of dual-cured composite was placed in the post-hole before fabrication of a small-diameter tapered cast post-core. Group 3 (ChemFil Superior): a thick layer of glass-ionomer was placed, as before. Metal-ceramic crowns were fabricated for all teeth. Microtensile bond strength tests and optical and atomic force microscope observations were employed to examine adhesion between the two restorative materials and the root canal dentin. RESULTS: Mechanical loading (kN) demonstrated that composite 0.64 (S.D. 0.05), but not glass-ionomer 0.49 (0.05), significantly reinforced the fracture resistance of thin-walled roots, P<0.001. Microtensile bond strengths (MPa) were 21.7 (1.6) for composite and 12.9 (1.7) for glass-ionomer, P=002. SIGNIFICANCE: A thick intermediate layer of resin-bonded composite, sandwiched between the root dentin and a small-diameter cast Ni-Cr post or dowel, increased significantly the fracture resistance of the roots.  相似文献

BACKGROUND: Gingival overgrowth occurs in patients receiving nifedipine. Gingival inflammation may be an etiologic factor. METHODS: Gingival fibroblasts were either exposed to (i) 0-500 ng/ml TNF-alpha or 10(-7) M nifedipine or (ii) 0-500 ng/ml TNF-alpha + 10(-7) M nifedipine for 7 days. 3H-proline was used to quantify collagenous protein synthesis. RESULTS: Both TNF-alpha and 10(-7) M nifedipine significantly decreased cell proliferation, and 10(-7) M nifedipine + 500 ng/ml TNF-alpha reversed these effects. Collagenous protein synthesis was significantly reduced by TNF-alpha and was significantly enhanced by either 10(-7) M nifedipine or 5-500 ng/ml TNF-alpha + 10(-7) M nifedipine. CONCLUSIONS: Our data report that nifedipine reverses the primary effects of TNF-alpha on collagenous protein synthesis. Patients with gingivitis could be susceptible to gingival overgrowth during nifedipine therapy as a result of synergistic effects of these agents on fibroblast metabolism, which occurs irrespective of reduced cell numbers.  相似文献

PURPOSE: To compare the dimensional measurements and surface topography of stereolithographic models generated from computerized tomography (CT), magnetic resonance imaging (MRI), and laser scanning (LS) data with the same subjects' natural ears and ear casts. MATERIALS AND METHODS: Stereolithographic models were manufactured from images of the subjects' ears and ear casts recorded by CT, MRI, and LS, and dimensional measurements were compared. In the second part of the study, all stereolithographic models were CT scanned and reconstructed in an STL file format. A comparison of the surface topography of the CT, MRI, and LS model ears was made. RESULTS: Intraclass correlation coefficients indicated that dimensions could be reliably measured on the CT, MRI, and LS stereolithographic models. A 2-way analysis of variance revealed no statistical differences between the various sources of data (P = .991). The smallest differences of surface topography were observed on the MRI/CT superimpositions. CONCLUSION: The dimensional measurements on the stereolithographic models were similar to those from the original source. Only small differences were apparent between the surface topography of the CT, MRI, and LS models. MRI may be particularly appropriate to fabricate a prosthesis because it involves no radiation for the patient and internal form can be reproduced. The use of this technique in clinical practice requires further study.  相似文献

OBJECTIVE: To explore the effects of high-concentration hydrogen peroxide bleaching agents on the microleakage of composite restorations. METHODS: In 60 extracted human molars, Class V restorations were prepared with Scotchbond 1/Filtek Z250 composite. Teeth were randomly divided into four groups: (1) no bleaching; (2) bleaching with 14% hydrogen peroxide gel from Crest Whitestrips; (3) bleaching with 20% carbamide peroxide gel from Opalescence PF 20; and (4) bleaching with 38% hydrogen peroxide gel Opalescence Xtra Boost. Bleaching procedures were carried out at 37 degrees C for 21 days/42 hours (2); seven days/42 hours (3); one day/45 minutes (4). Varnish was applied on the apical portion of the teeth only, excluding the restoration, prior to immersion in a 0.1% rhodamin-B-isothiocyanate solution for 24 hours at 37 degrees C. After rinsing, specimens were embedded in methacrylate blocks, and sectioned with a water-cooled microtome with three restoration cuts positioned centrally parallel to the long axis of the tooth. Microleakage was evaluated at the occlusal margins of the Class V restorations using a stereo microscope, separate for dentin and enamel margins. RESULTS: Over 90% of enamel margins exhibited no microleakage following cycling. Bleaching agents had almost no effect on numerical averages. Eighty-eight percent of the dentin margins were free of microleakage for the non-treated control group. Bleaching treatments collectively had slight numerical reductions to around 80%. The statistical evaluation (Kruskal-Wallis-test) showed no significant difference in microleakage between groups for enamel or dentin. CONCLUSION: Bleaching with the materials tested had no influence on microleakage of Filtek Z250 composite bonded with Scotchbond 1.  相似文献

Sharpey's fibers support teeth by attachment of periodontal ligament fibers to alveolar bone. The effects of diabetes mellitus on this support mechanism have not been described and were the subject of this study. Male Swiss mice were rendered diabetic by streptozotocin. Mandibles were removed 9 weeks after injections, the mineralizing front of the depository surface of the interdental septum was exposed by fracture through the periodontal ligament, rendered anorganic, and examined by scanning electron microscopy. No significant reduction in alveolar crest height was evident in diabetic as compared to control animals; however, significant changes in the a) mineralization patterns of bone depository surfaces and Sharpey's fibers, and b) number of Sharpey's fibers inserting into alveolar bone were evident in diabetics. Unmineralized fissures, characteristic of Sharpey's fibers of control, were nearly obliterated by mineralized tissue in diabetic animals. The mineralizing front of the middle and apical thirds of the diabetic alveolar wall was covered by numerous large calcified globules resembling enlarged calcospherites, which were not evident over the cervical third or control tissues. The mean Sharpey fiber density was greater in controls than in diabetics (p less than 0.001); however, there was no significant difference between their mean diameters. These observations suggest that, in early diabetes, Sharpey's fibers and depository surfaces of the middle and apical thirds of the interdental septum have morphologic evidence of aging, which precedes significant reduction in alveolar crest height. These changes may weaken the attachment of periodontal ligament fibers to bone and reduce resistance of the periodontium to intrusive forces.  相似文献
