The change process in alcoholics: client motivation and denial in the treatment of alcoholism within the context of contemporary nursing

Despite being constantly cited as a critical intervening variable in the recovery from alcohol problems, there is a paucity of literature on client motivation. This paper reviews the current literature which impacts on motivation and its importance in treatment and develops in a stepwise manner the revised expectancy/motivation hypothesis, tentatively explaining both motivation and denial as a natural process in behavioural change. Because of the revised expectancy/motivation hypothesis' distinctly defined stages which closely relate to the process of nursing and the qualitative and quantitative measurement it entails which the nursing process demands, it offers a particularly appropriate model for treatment within nurse practice.     

Loneliness among elderly people living in Stockholm: a population study

The purpose was to investigate experienced loneliness among the elderly. The material included 1725 people, aged 75 and over. The study describes relationships between loneliness, social network, cognitive function and health. Thirty-five per cent experienced loneliness, and a higher percentage was found among women. A gradual increase in loneliness was found up to the age of 90, after which a levelling was found. Elderly persons living together with a partner experienced less loneliness. There were no significant differences between those with and without children. Ten per cent reported not having any friends and, of these, one out of two experienced loneliness. A high frequency of experienced loneliness was found among elderly people with reduced cognitive function. Subjectively experienced bad health and loneliness were strongly related to each other, i.e. a person who experienced loneliness did usually not feel completely healthy.     

蛇毒溶栓素对实验动物血小板功能和环腺苷酸含量的影响

本文报道蛇毒溶栓素对实验动物血小板功能和血浆环腺苷酸含量的影响.(1)对血小板数的影响:给家犬以4 u/kg量静注1小时后,循环血的血小板数由药前130.44×10~9/L±50.02×10~9/L下降至60.20×10~9/L±20.50×10~9/L(P<0.05).(2)对血小板聚集功能的影响:猕猴和家兔分别按4 u/kg、8 u/kg量静注1小时后,对ADP诱导的血小板聚集率分别山药前的62.32±22.05%和43.66±10.60%下降至1.32±0.49%和4.95±1.96%;抑制率分别为97.50±1.75%和88.34±4.31%,与药前组相比均有非常显著的差异(P<0.001).(3)对血小板cAMP含量的影响:猕猴和家兔以蛋白竞争结合法测定血小板内cAMP的含量,药后1小时的含量65.43±31.35pmole/mg,32.25±15.43pmo1e/mg蛋白质,与药前12.02±0.95pmole/mg,4.25±3.13pmole/mg蛋白质相比有明显升高(P<0.001).     

新型无机絮凝剂POLY A-F处理印染废水的研究

本文采用新型无机絮凝剂FOLY A-F联合木炭柱处理针织厂印染废水取得较为满意的效果。废水CODCr含量的波动范围为161.8～483.6ppm;固体悬浮物95.5～442ppm;pH8.4～11.6。经本方法处理后,溶液基本上无色透明,固体悬浮物近于0,CODCr含量少于100ppm,CODCr去除率为77.9%。以上四项检测指标均达到国家规定的排放标准。适用于处理中、小型针织厂的废水。     

钩端螺旋体核糖体提取物免疫保护作用的研究

作者对核糖体现有的制备方法进行了部分改进,从黄疸出血群赖型017株钩体中成功地制备了核糖体提取物。该提取物经SPA-ELISA 证实具有免疫原性,对40只受试豚鼠的保护试验结果表明,核糖体提取物对同型毒株的攻击具有明显的保护作用;结果还证明核糖体提取物能诱导机体的体液免疫反应。     

Perineal examination as a predictor of underlying external anal sphincter damage

Objective To assess the relation between perineal inspection and sphincter integrity in parous women.
Design Prospective observational study.
Setting District general hospital.
Population Fifty-seven consecutive parous women attending a gynaecology clinic for problems unrelated to the pelvic floor.
Methods A detailed history of bowel function and mode of delivery obtained; the perineum inspected to determine the presence and position of scarring, and anal endosonography performed.
Results In 19 women with an intact perineum on inspection, endosonography showed perineal scarring in five, with both perineal and sphincter scarring in three. Four had urge faecal incontinence. Three patients had a perineal tear only on inspection, but this group was too small for analysis and was discounted. Nine had an episiotomy scar only. Endosonography demonstrated perineal scarring in four, and combined perineal and sphincter scarring in two; one woman in this group had urge faecal incontinence. Twenty-six women had episiotomy and perineal tears on inspection. Endosonography revealed underlying perineal scarring in five women, with combined perineal and sphincter scarring in 14; six women in this group had urge faecal incontinence and one passive incontinence for flatus. Sonographically the scarring was anterior and circumferential rather than radial, and mostly left-sided, whereas on inspection episiotomy and perineal scarring were right sided.
Conclusions A normal perineum on clinical examination does not exclude underlying sphincter damage. The incidence of sphincter damage increases significantly when an episiotomy scar is associated with a perineal tear.     

血清TPS浓度在消化道肿瘤中价值的研究

共选择３０例消化道肿瘤病人，其中食管癌，胃癌，大肠癌各１０例，设健康献血者３０例，对两组血清特异型组织多肽抗原浓度进行了检测，结果肿瘤组血清ＴＰＳ浓度明显高于健康人。消化道肿瘤阳性检出经达７０％，其中食管癌８０％，胃癌６０％，大肠癌７０％。消化道肿瘤合并淋巴转移阳性检出率达７６．９％，其中食管癌１００％，胃癌６６．７％，大肠癌７０．０％。随着病程发展，血清ＴＰＳ浓度呈现升高趋势。     

Systemic necrotizing vasculitis associated with childhood sarcoidosis

Childhood sarcoidosis is a rare disorder with protean manifestations. The case of a child with prolonged fever, hepatosplenomegaly, pancytopenia, and systemic necrotizing vasculitis manifesting as fever, rash and skin infarctions, digital pregangrene, and foot drop is reported. This is the first case of systemic necrotizing vasculitis reported in sarcoidosis. The fulminant course of the disease required treatment with intravenous pulsed cyclophosphamide and high doses of corticosteroids. The spectrum of vasculitis in childhood and adult sarcoidosis is reviewed.     

锰超氧化物岐化酶模型配合物的研究

为探索天然超氧化物岐化酶（ＳＯＤ）岐化超氧离子的机理，及用于临床的可能性，我们合成了乳酸、柠檬酸和酒石酸的锰（Ⅱ）配合物作为Ｍｎ－ＳＯＤ的模拟物，用光照法测定了它们的活性，得到了有意义的结果：锰配合物作为Ｍｎ－ＳＯＤ的模拟物，有肯定的活性，其中以乳酸锰配合物活性最高。
