Intravesical bacillus Calmette–Guérin (BCG) is the standard of care for bladder carcinoma in situ (CIS). The response to BCG therapy against CIS is generally assessed by random bladder biopsy (RBB). In this study, we examined the necessity of routine RBB after BCG therapy.
Methods
We retrospectively identified 102 patients who were initially diagnosed with CIS with or without papillary tumor and received subsequent 6–8-week BCG therapy. Thereafter, all patients underwent voiding cytology analysis, cystoscopy, and RBB to evaluate the effects of BCG therapy. We evaluated the association between clinical parameters (voiding cytology and cystoscopy findings) and the final pathological results by RBB specimens.
Results
According to the pathological results of RBB, 30 (29%) patients had BCG-unresponsive disease (remaining urothelial carcinoma was confirmed pathologically) and 20 were diagnosed with CIS. Positive/suspicious voiding cytology and positive cystoscopy findings were well observed in patients who had BCG-unresponsive disease compared with their counterparts (p?=?0.116, and p?<?0.001, respectively). The sensitivity (Sen.), specificity (Spe.), positive predictive value (PPV), and negative predictive value (NPV) of voiding cytology were 50%, 68%, 39%, and 77%, respectively. The values for cystoscopy findings were as follows: Sen.: 87%, Spe.: 57%, PPV: 46%, and NPV: 91%. The values for their combination (having either of them) were as follows: Sen.: 100%, Spe.: 44%, PPV: 43%, and NPV: 100%.
Conclusion
RBB after BCG therapy for patients with negative voiding cytology and negative cystoscopy may be omitted because their risk of BCG-unresponsive disease is significantly low (NPV: 100%).
An 18-year-old male was admitted to our Emergency Department with a traumatic abdominal wall hernia (TAWH) of the left lower
quadrant (LLQ) after suffering hypogastric blunt injury and urogenital lacerations in a motorcycle accident. Upright chest
X-ray showed a small amount of right infradiaphragmatic free air, and a computed tomographic (CT) scan demonstrated an abdominal
wall hernia. At surgery, no impairment was found in the digestive tract, and an abdominal herniorrhaphy was performed. It
is suggested that the free air had passed through a connection between the scrotal laceration and the contralateral abdominal
defect via the subcutaneous space and was palpated as emphysema. This is a new type of TAWH, which suggests that blunt abdominal
trauma may result in negative pressure in the subcutaneous and peritoneal cavity, and this could reflect the pathophysiology

The outcomes of three methods of intensity-modulated radiation therapy (IMRT) for localized prostate cancer were evaluated. Between 2010 and 2018, 308 D’Amico intermediate- or high-risk patients were treated with 2.2 Gy daily fractions to a total dose of 74.8 Gy in combination with hormonal therapy. Overall, 165 patients were treated with 5-field IMRT using a sliding window technique, 66 were then treated with helical tomotherapy and 77 were treated with volumetric modulated arc therapy (VMAT). The median age of patients was 71 years. The median follow-up period was 75 months. Five-year overall survival (OS) and biochemical or clinical failure-free survival (FFS) rates were 95.5 and 91.6% in the 5-field IMRT group, 95.1 and 90.3% in the tomotherapy group and 93.0 and 88.6% in the VMAT group, respectively, with no significant differences among the three groups. The 5-year cumulative incidence of late grade ≥2 genitourinary and gastrointestinal toxicities were 7.3 and 6.2%, respectively, for all patients. Late grade ≥2 gastrointestinal toxicities were less frequent in patients undergoing VMAT (0%) than in patients undergoing 5-field IMRT (7.3%) and those undergoing tomotherapy (11%) (P = 0.025), and this finding appeared to be correlated with the better rectal DVH parameters in patients undergoing VMAT. Other toxicities did not differ significantly among the three groups, although bladder dose-volume parameters were slightly worse in the tomotherapy group than in the other groups. Despite differences in the IMRT delivery methods, X-ray energies and daily registration methods, all modalities may be used as IMRT for localized prostate cancer. 相似文献
What is known and Objective: Pilocarpine, a muscarinic receptor agonist, has been used for the treatment of dry mouth. Salivary glands are supplied with nerve fibres that contain neuropeptides, such as substance P, calcitonin gene‐related peptide (CGRP) and vasoactive intestinal polypeptide (VIP), which are important modulators of salivation. It is known that measurement of salivary and plasma levels of neuropeptides is useful for assessing the dose–pharmacological effect relationship of drugs. The relationship between the action of pilocarpine and neuropeptides in humans has not been studied. Moreover, studies evaluate the usefulness of drug salivary levels in the pharmacological evaluation of drugs are scarce. The aim of this study was to examine the effects of pilocarpine on the levels of substance P‐, CGRP‐ and VIP‐like immunoreactive substances (IS) in saliva and plasma taken in healthy humans. Methods: Five healthy male subjects participated in this study. Pilocarpine tablet (10 mg) or placebo tablet was orally administered with 100 mL of water. Each subject was administered placebo and drug with an interval of 4 weeks in between. Saliva was sampled before and at 20, 40, 60, 90, 120, 180 and 240 min after administration of the test substances. Venous blood samples (10 mL) were also taken from a forearm vein at each time interval. The samples were then enzyme immunoassayed using a highly sensitive system for substance P‐, CGRP‐ and VIP‐IS. The amount of saliva was measured by the Saxon test. Results: A single oral administration of pilocarpine increased the release of salivary substance P‐IS (the area under the concentration‐time curve: AUC0→240 min) compared with the placebo. Pilocarpine also significantly increased the release of salivary CGRP‐IS (AUC0→240 min). Pilocarpine significantly increased the release of plasma CGRP‐IS. The salivary volume correlated with the salivary level of substance P and CGRP‐IS (r = 0·84, P <0·05 and r = 0·59, P <0·05, respectively). AUC0→240 min for substance P‐IS in saliva correlated with that for plasma (r = 0·78, P <0·05). What is new and Conclusion: Pilocarpine increases the release of salivary substance P and CGRP‐IS. This suggests that one mechanism by which pilocarpine improves dry mouth is by local stimulation of neuropeptidergic nerves. Moreover, saliva levels of substance P showed good correlation with the plasma levels. The substance P levels in saliva and plasma may be good indicators of the effects of drugs used in dry mouth/xerostomic patients. 相似文献
Background/Purpose Endoscopic drainage of pancreatic pseudocysts using transpapillary and transmural approaches has been reported. In this study,
endoscopic nasopancreatic drainage (ENPD) and pancreatic stenting were performed in patients with pseudocyst and abscess associated
with acute pancreatitis, and the usefulness and problems of the procedures were investigated.
Methods After endoscopic retrograde pancreatography was done, ENPD and/or pancreatic stenting were performed in 13 patients with pancreatitis
and pseudocyst or abscess that communicated with the main pancreatic duct.
Results ENPD was performed in seven patients, and was effective in all five patients with cysts: the cysts disappeared or shrank.
However, the condition in the two patients with abscess was unchanged, and percutaneous drainage was performed. Stenting was
carried out in six patients, and the cyst disappeared or pancreatitis was improved in all six. The stent was removed from
two patients, but no recurrence has been noted so far.
Conclusions ENPD and stenting are effective therapeutic choices for acute and chronic pancreatitis and pseudocysts, and they are superior
to percutaneous drainage to avoid pancreatic fistula, but they may not be effective for pancreatic abscess. Selection of therapeutic
methods corresponding to individual cases is important. 相似文献
Background Radiofrequency (RF) ablation for the treatment of the section line prior to liver resection has been proposed as a way to
reduce blood loss during hepatectomy. Our group compared hepatectomy with and without RF ablation to determine whether this
technique actually reduces blood loss during liver resection and whether it affects the perioperative outcome.
Method Of 151 patients who underwent a hepatectomy between January 2002 and October 2005 at the Division of Gastrointestinal Surgery
in the Department of Surgery of Saitama Medical University, 48 who had a partial hepatectomy or resection of a portion of
liver smaller than a single Couinaud segment were included in the study. Twenty patients who had RF-assisted hepatectomy [RF
(+) group] and 28 patients who had hepatectomy without ablation [RF (-) group] were studied to compare the rates of intraoperative
blood loss and the effects of RF ablation on the perioperative outcome.
Results Intraoperative blood loss was significantly reduced in the RF (+) group. In contrast, the alanine aminotransferase activity
in the RF (+) group was significantly elevated immediately after the operation. There was no significant difference in the
incidence of postoperative complications between the groups, although bile leakage did occur in three RF (+) patients.
Conclusions Our results demonstrate that the RF ablation technique can be a useful way to reduce surgical blood loss. In view of its association
with severe postoperative liver damage, the technique must be applied with caution. The danger may be especially relevant
to patients with chronic liver disease and decreased liver reserve. 相似文献
Purpose Total pelvic exenteration (TPE) is the standard procedure for locally advanced rectal cancer involving the prostate and seminal
vesicles. We evaluated the feasibility of bladder-sparing surgery as an alternative to TPE.
Methods Eleven patients with advanced primary or recurrent rectal cancer involving the prostate or seminal vesicles, or both, underwent
bladder-sparing extended colorectal resection with radical prostatectomy. The procedures performed were abdominoperineal resection
(APR) with prostatectomy (n = 6), colorectal resection using intersphincteric resection combined with prostatectomy (n = 4), and abdominoperineal tumor resection with prostatectomy (n = 1). Local control and urinary and anal function were evaluated postoperatively.
Results Cysto-urethral anastomosis (CUA) was performed in seven patients and catheter-cystostomy was performed in four patients. Coloanal
or colo-anal canal anastomosis was also performed in four patients. There was no mortality, and the morbidity rate was 38%.
All patients underwent complete resection with negative surgical margins. After a median follow-up period of 26 months there
was no sign of local recurrence, and ten patients were alive without disease, although distant metastases were found in three
patients. Five patients had satisfactory voiding function after CUA, and three had satisfactory evacuation after intersphincteric
resection (ISR).
Conclusion These bladder-sparing procedures allow conservative surgery to be performed in selected patients with advanced rectal cancer
involving the prostate or seminal vesicles, without compromising local control. 相似文献
PURPOSE: Laparoscopic choledochotomy on patients indicated for common bile duct exploration was carried out according to an algorithm for managing choledocholithiasis. This study describes retrospectively our method and evaluates a new cystic duct biliary decompression cannula (J-tube) as an alternative to the T-tube. METHODS: Patients with confirmed choledocholithiasis (n=46) underwent laparoscopic choledochotomy. The T-tube was inserted in cases with suspected retained stones after common bile duct clearance, and the J-tube (950-mm long, 4 Fr) with a tapered and J-shaped segment at the distal end was inserted in other cases. RESULTS: Only 1 case was converted to open surgery (success rate, 97.8%); the J-tube was inserted in 30 patients and the T-tube in 15. The median operation time, hospital stay, and the interval until removal of the tube were significantly shorter with J-tube than with T-tube cases. Bile leakage after surgery occurred in 4 J-tube and 2 T-tube cases with one residual stone in each case. CONCLUSIONS: The transcystic decompression tube is easily and safely inserted with the J-kit. Among several strategies currently available for the management of choledocholithiasis, laparoscopic choledochotomy with the use of the J-tube is one of the safest and most feasible methods. 相似文献
