Twenty patients, aged 4 months to 58 years, were evaluated for liver transplantation by duplex sonography, and 15 transplantations were completed; 42 postoperative examinations were performed. Sonographic findings were correlated with seven preoperative and five postoperative angiographic evaluations. Preoperative duplex US findings included tumors, portal vein occlusion, varices, biliary obstruction, and variant vascular anatomy. Postoperative findings included hepatic artery occlusion, portal vein occlusions (one with cavernous transformation), portal vein stenosis, biliary obstruction, intrahepatic and extrahepatic fluid collections, and air in the portal vein due to ischemic bowel. Use of angiography allowed confirmation of the vascular abnormalities and demonstrated evidence of rejection in patients with normal Doppler waveforms. Duplex sonography is a valuable portable technique for evaluating these patients and can be used in triage of patients requiring angiography. 相似文献

Thirteen patients were prospectively studied with arthrography to determine whether oblique head positions were helpful in diagnosing internal derangement of the temporomandibular joint (TMJ). After injection of contrast material, inclined transcranial lateral spot films were obtained with the patient's head in upward oblique, downward oblique, and neutral positions. The spot films were analyzed for internal derangement by assessing the size of the mass effect on the arthrogram. Twelve of the 13 arthrograms showed evidence of internal derangement of the TMJ. However, the apparent size of the mass effect varied in different head positions. Each of the three views failed to demonstrate the mass effect in at least one instance (eight different patients). No head position was better than another in demonstrating the mass effect. Thus, oblique head positions are useful adjuncts for diagnosing internal derangement of the TMJ. 相似文献

Tone recognition is partially subserved by neural activity in the right
frontal and primary auditory cortices. First we determined the brain areas
associated with tone perception and recognition. This study then examined
how regional cerebral blood flow (rCBF) in these and other brain regions
correlates with the behavioral characteristics of a difficult tone
recognition task. rCBF changes were assessed using H2(15)O positron
emission tomography. Subtraction procedures were used to localize
significant change regions and correlational analyses were applied to
determine how response times (RT) predicted rCBF patterns. Twelve trained
normal volunteers were studied in three conditions: REST, sensory motor
control (SMC) and decision (DEC). The SMC-REST contrast revealed bilateral
activation of primary auditory cortices, cerebellum and bilateral inferior
frontal gyri. DEC-SMC produced significant clusters in the right middle and
inferior frontal gyri, insula and claustrum; the anterior cingulate gyrus
and supplementary motor area; the left insula/claustrum; and the left
cerebellum. Correlational analyses, RT versus rCBF from DEC scans, showed a
positive correlation in right inferior and middle frontal cortex; rCBF in
bilateral auditory cortices and cerebellum exhibited significant negative
correlations with RT These changes suggest that neural activity in the
right frontal, superior temporal and cerebellar regions shifts back and
forth in magnitude depending on whether tone recognition RT is relatively
fast or slow, during a difficult, accurate assessment.

BACKGROUND: Cromer blood group antigens are located on decay- accelerating factor (DAF, CD55), which contains four short consensus repeats (SCRs). Cromer system antibodies may be of clinical significance in blood transfusion. STUDY DESIGN AND METHODS: Soluble recombinant DAF (srDAF) constructs, consisting of all four SCRs or of only two SCRs, were expressed in the yeast Pichia pastoris. They are used in hemagglutination-inhibition tests with Cromer system antibodies and with DAF-specific monoclonal antibodies. RESULTS: The srDAF inhibited hemagglutination by all Cromer system alloantibodies in undiluted serum. Antibodies to antigens of other blood group systems were not inhibited by the srDAF. Hemagglutination-inhibition tests with domain-deleted srDAF showed that UMC is on SCR-4 and confirmed that Tca, TcaTcb, and WESb are on SCR-1; Dra is on SCR-3; and Cra is on SCR- 4. CONCLUSIONS: Hemagglutination inhibition with srDAF is useful in the recognition of antibodies that belong to the Cromer blood group system and facilitates pretransfusion testing. This use of domain-deleted srDAF provides an easy method of determining epitope location on DAF and is an aid to more precise identification of Cromer system antibodies. 相似文献
