For patients who lost one or more teeth, oral osseointegrating implants can offer a natural and solid solution. In order to rehabilitate (partially) edentulous patients with oral implants, a surgical intervention is inevitable. It is therefore of utmost importance that both esthetical and biomechanical demands are met in as few interventions and as little time as possible, which makes a thorough preoperative examination of the edentulous region mandatory. Radiographs of different kinds offer the possibility to evaluate bone volume and--to a lesser extent--quality of the area to be rehabilitated. It should however be kept in mind that radiography necessarily brings about a certain amount of possible detrimental radiation burden. Selection of the appropriate radiographic technique should therefore be based on a well thought-out balance between diagnostic value and risk level of the intervention on the one hand and radiation burden on the other.  相似文献

Background. The necessity of nerve anastomosis in an attempt to regain cutaneous sensitivity following flap transfer has been the basis of many discussions. In our study, we investigated the degree of sensory recovery with emphasis on the different nerval qualities on the radial forearm and the latissimus dorsi flap. Collective. Sixty-two patients with 66 latissimus dorsi flaps and 19 patients with a radial forearm free flap were examined. Results. Except for five latissimus dorsi and two radial forearm flaps, all flaps investigated showed clinical signs of sensory recovery, whereby the degree and quality of cutaneous reinnervation varied. In comparison, neural reconstruction did not lead to any clinical improvement. Conclusion. Based on our clinical results, we consider nerve reconstruction during radial forearm and latissimus dorsi free flap transfer unnecessary. To what extent this can be said for other flaps requires further investigation.  相似文献

Porphyromonas gingivalis produces virulence factors which can modify the molecular and cellular components of the host immune response. In the present work we investigated the role of specific virulence factors from P. gingivalis in the induction of apoptosis in Jurkat T cells. P. gingivalis culture supernatants mimicked the effect of butyric acid on T‐cell apoptosis and this effect was associated with an increase in histone H4 acetylation. A role for proteases was excluded in experiments which demonstrated that neither protease inhibitors nor use of P. gingivalis mutants defective in protease synthesis had any effect on the stimulation of T‐cell apoptosis in this system.  相似文献

The emerging field of orofacial pain was considered by the American Dental Association for full status as a new dental specialty. While the recognition of orofacial pain as a specialty was denied, the American Academy of Orofacial Pain plans to continue its efforts. Many recent advances in the neuroscience of orofacial pain have led to treatments that provide significant relief for patients with chronic orofacial pain disorders. However, access to this care has been limited, leaving many patients to suffer. Dentists are generally supportive of the efforts to develop oral pain treatment into a specialty because the field will provide benefits for both dentists and their patients. A recent survey of 805 individuals who reported having a persistent pain disorder revealed that more than four out of 10 people have yet to find adequate relief, saying their pain is out of control--despite having the pain for more than five years and switching doctors at least once. "This survey suggests that there are millions of people living with severe uncontrolled pain," says Russell Portenoy, MD, president of the American Pain Society. "This is a great tragedy. Although not everyone can be helped, it is likely that most of these patients could benefit if provided with state-of-the-art therapies and improved access to pain specialists when needed." Development of the field of orofacial pain into a dental specialty has been moved primarily by the fact that historically, patients with complex chronic orofacial pain disorders have not been treated well by any discipline of healthcare. Recent studies of chronic orofacial pain patients have found that these patients have a higher number of previous clinicians and have endured many years with pain prior to seeing an orofacial pain dentist (see Figure 1). Complex pain patients and the clinicians who see them are often confused about who they should consult for relief of the pain. Treatment for those patients within the existing structure of dental or medical specialties has been inadequate, with millions of patients left suffering. Insurers are also confused with regard to reimbursement and may make decisions to exclude treatment for orofacial pain disorders under both dental and medical policies. However, dentistry has taken a leading role in healthcare to address the national problem of developing the field of orofacial pain into a dental specialty. A study of dentists and dental specialists has shown that there is a recognized need and broad support for developing this field into a specialty.  相似文献

Periodontal infection and preterm birth: results of a prospective study   总被引:22,自引:0,他引:22  
BACKGROUND: Previous studies have suggested that chronic periodontal infection may be associated with preterm births. The authors conducted a prospective study to test for this association. METHODS: A total of 1,313 pregnant women were recruited from the Perinatal Emphasis Research Center at the University of Alabama at Birmingham. Complete periodontal, medical and behavioral assessments were made between 21 and 24 weeks gestation. After delivery, medical records were consulted to determine each infant's gestational age at birth. From these data, the authors calculated relationships between periodontal disease and preterm birth, while adjusting for smoking, parity (the state or fact of having born offspring), race and maternal age. Results were expressed as odds ratios and 95 percent confidence intervals, or CIs. RESULTS: Patients with severe or generalized periodontal disease had adjusted odds ratios (95 percent CI) of 4.45 (2.16-9.18) for preterm delivery (that is, before 37 weeks gestational age). The adjusted odds ratio increased with increasing prematurity to 5.28 (2.05-13.60) before 35 weeks' gestational age and to 7.07 (1.70-27.4) before 32 weeks' gestational age. CONCLUSIONS: The authors' data show an association between the presence of periodontitis at 21 to 24 weeks' gestation and subsequent preterm birth. Further studies are needed to determine whether periodontitis is the cause. CLINICAL IMPLICATIONS: While this large prospective study has shown a significant association between preterm birth and periodontitis at 21 to 24 weeks' gestation, neither it nor other studies to date were designed to determine whether treatment of periodontitis will reduce the risk of preterm birth. Pending an answer to this important question, it remains appropriate to advise expectant mothers about the importance of good oral health.  相似文献

BACKGROUND: Although a growing body of evidence indicates that oral irrigation with water has therapeutic benefits in periodontitis, the mechanisms of action have not been elucidated. The aims of this study were: (1) to analyze the effects of oral irrigation (Water Pik Oral Irrigator) on the clinical signs of adult periodontitis (AP) and on the levels of interleukin-1 beta (IL-beta), prostaglandin-E2 (PGE2), interleukin-10 (IL-10) and interferon-gamma (IFN-gamma) in GCF, and (2) to analyze the influence of the periodontitis-related IL-1 genotype (IL-1GT) on these variables. METHOD: A single-center, blinded study in otherwise healthy humans (n= 52) with localized mild to moderate AP was carried out, using the following groups: group A (n= 12), no oral hygiene for 14 days; group B (n=20), routine oral hygiene (ROH) for 14 days; group C (n=20), supra-gingival oral irrigation plus ROH for 14 days. Group A patients were crossed-over to group C for 14 days (=day 28) after a professional prophylaxis. Group assignment was randomized by a coin toss, with the exception of group A subjects, who were self-selected as per recommendations of the internal review board for human subjects. GCF was sampled from 3 study teeth per patient and analyzed for IL-1 beta, PGE2, IL-10 and IFN gamma by ELISA on days 0, 7, 14 and 28. Probing pocket depths (PPD), clinical attachment levels (CAL), bleeding on probing (BOP), gingival index (GI) and plaque index (PI) were measured by a calibrated examiner (TWS) on days 0, 14 and 28. Analysis of covariance was performed using SAS 6.12 and Proc Mixed with group and IL-1GT as the factors and the baseline levels as the covariate, with output being least squares means and least significant difference (LSD). Significant differences were declared if the p-value for the F-statistic was < or =0.05. RESULTS: Oral irrigation plus ROH resulted in a significant reduction in PPD, BOP, GI and PI, as well as IL-beta levels by 7 days and PGE2 levels by 14 days, relative to ROH or no oral hygiene. Interestingly, decreased IL-1 beta levels in patients using oral irrigation plus ROH was accompanied by a trend for increased levels of the "anti-inflammatory" cytokine IL-10. ROH reduced GI, BOP and PI, and PGE2 levels by 14 days, but had no effect on IL-1 beta or IL-10 levels relative to no oral hygiene. The effects of no oral hygiene were reversed by a prophy followed by oral irrigation plus ROH for 14 days. No clinical differences were evident between IL-1 GT (+) patients (n= 1) and GT (-) patients (n=40), but the former had significantly elevated levels of GCF IL-10 and borderline increases in IL-1 beta (p=0.07). CONCLUSIONS: Oral irrigation with water for 14 days had an improved therapeutic benefit for AP over that of routine oral hygiene alone and this improvement was accompanied by a down-modulation of the pro-inflammatory cytokine profile in GCF.  相似文献

BACKGROUND: The recently introduced Er:YAG laser radiation appears to be a promising alternative in treating dental hard tissue due to its thermo-mechanical ablation properties and the lack of thermal side effects. The present in vitro study attempted to evaluate the use of Er:YAG laser radiation in combination with a specially developed delivery system in removing calculus from root surfaces. METHODS: Fifty extracted anterior teeth, premolars and molars, were divided into 2 groups of 25 each with (group A) and without (group B) subgingival calculus. Source of radiation was an Er:YAG laser device with a wavelength of 2.94 microm, in the infrared optical spectrum, a pulse duration of 250 ns, and a pulse repetition rate of 15 pps. In each group, 6 teeth were irradiated with 300 laser pulses either at 60 mJ, 80 mJ, 100 mJ, or 150 mJ. The samples were continually moved linearly using a computer numeric controlled device. The volumetric evaluation of root substance removal was performed with a 3-dimensional laser scanning system (100,000 surface points per sample, accuracy: 5 microm) and special image analyzing software. A scanning electron microscopic (SEM) observation was performed to assess the laser induced ultrastructural changes on the root surfaces. Statistical analysis was carried out with ANOVA followed by the Scheff*e test and with regression analysis according to Pearson-Bravais at a level of significance of 5% (P <0.05). RESULTS: The linear measurement of substance removal on calculus samples (group A) revealed average depths between 174.38 (+/-16.13) microm and 501.85 (+/-111.01) microm. Defect depths on the teeth without calculus (group B) ranged from 37.78 (+/-14.03) microm to 484.44 (+/-80.63) microm. The SEM observation of laser-treated root surfaces revealed no signs of thermal damage; e.g., melting, fusion, or cracking. CONCLUSIONS: The results of the present study showed that a substance removal with Er:YAG laser radiation at lower energy densities is comparable, in effect, to that after conventional root surface instrumentation with curets. The results seem to indicate that calculus removal can be selectively done using lower radiation energies. Considering the favorable results of the SEM investigation, the use of the Er:YAG laser in periodontal therapy may be possible in the future.  相似文献

The aim of this study was to investigate the leakage along the apical portion of warm gutta-percha obturated curved canals. Human mandibular premolars with single, curved (21 degrees-40 degrees) canals were prepared using the Lightspeed technique. Two groups of prepared canals, matched according to curvature and prevalence of apical transportation, were obturated by two techniques. Coronal gutta-percha was removed immediately after root obturation was completed to simulate the procedure for post space preparation. Leakage along the apical 3 mm of root filling was measured with a fluid transport device. Vertical condensation of warm gutta-percha and Pulp Canal Sealer provided less leakage than Thermafil plastic obturators and AH26 sealer (P = 0.002).  相似文献
