The authors' purpose of this study was to analyze the current system of healthcare manager's education in the Slovak Republic. The study was essentially qualitative and used a triangular approach. This included an analysis of the range of healthcare management education providers, an assessment of needs that was based on questionnaires distributed to a sample of top managers in hospitals, and a focus group discussion. The findings revealed a gap between the estimated needs and the current capacities for management education. The results serve as a baseline for a discussion of future management training and education for healtcare providers in the Slovak Republic.     

Mechanisms of Sublingual Immunotherapy

Allergen-specific sublingual immunotherapy is now recognized to be an efficacious and well-tolerated treatment for allergic rhinitis. Emerging treatment strategies are also aimed at the primary treatment of allergic asthma, particularly allergy to house dust mites. Knowledge of the exact mechanisms of action of sublingual immunotherapy is at a basic level, although there appear to be similarities to the immunological changes seen in subcutaneous immunotherapy. An improved understanding should allow the development of more effective treatment programs and widen the potential use of this form of immunotherapy. This review discusses the possible mechanism of action of sublingual immunotherapy, including data from animal and clinical studies, while comparing this with the current understanding of subcutaneous immunotherapy.     

Mechanical Effects of Rear-Wheel Camber on Wheelchairs

Giving the rear wheels of manually propelled wheelchairs negative camber (by angling them such that the bottoms of the wheels are farther apart than the tops) has become increasingly popular, especially among active users. This study's purpose was to evaluate how changing camber affects a wheelchair's mechanics and the extent of these changes. Using a representative wheelchair occupied by an anthropomorphic dummy, we altered the camber from +15° to ?15° and measured the effects on nine selected variables. In addition to increasing the wheelchair's track width, negative camber increases the wheelchair's wheelbase and decreases the height of the frame. With negative camber, the center of gravity (CG) of the occupied wheelchair moves backwards. Negative camber causes a backwards tilt of the frame which, in turn, causes toe-out of the rear wheels, a positive caster-inclination angle and an increase in the caster-trail distance. It is apparent that a change in one component of a wheelchair can affect many others, which should be taken into consideration and, where appropriate, compensated for when changing the camber of the wheelchair.
