急性重症胆管炎的介入治疗   总被引:9,自引:2,他引:7  
目的讨论经皮经肝穿刺放置引流管(简称PTCD)减压引流治疗急性重症胆管炎(简称AOSC)的可行性及疗效.方法 1996年3月~2001年4月行PTCD治疗AOSC病人15例,男10例,女5例,胆石症术后8例,胆管癌2例,胰腺癌3例,胆道畸形2例,其中4例放置了胆道内支架.结果 15例病人技术成功率100%,无1例出现穿刺置管所致的严重并发症.病情缓解后针对病因有8例行手术治疗痊愈,4例不能手术的肿瘤患者植入了胆道内支架,1例于置管后3d死于多脏器功能衰竭,2例置管引流病情稳定后因各种原因放弃继续治疗,保留引流管自动出院.结论 PTCD可有效降低胆管压力,缓解病情,降低AOSC的死亡率,为进一步的病因治疗创造条件.  相似文献

目的:观察云克 (99Tc- MDP)治疗类风湿关节炎 (RA)的疗效。 方法:对 80例类风湿关节炎 (RA)患者应用云克进行静脉治疗 2 1d,并对治疗前后的相关指标进行对照分析。结果 :治疗前与治疗后相比 ,患者晨僵持续时间、握力、ESR、CRP、RF有统计学意义 (P <0 .0 5 ) ,患者关节压痛数及关节肿胀数差异无统计学意义 (P >0 .0 5 )。 结论:云克治疗类风湿关节炎能改善临床症状及实验室指标 ,远期疗效好 ,而且毒副作用小。  相似文献

目的:探讨骨科手术患者术中自体血回输的临床效果,以期能减少异体血的用量。方法:将骨科择期手术患者,术中随机分为自体血回输组,及异体血输血组,比较两组术后24h伤口引流量、血常规等情况。结果:两组患者手术顺利,自体血回输组术中均未用异体血,但少数患者术后发生低凝状态,输入异体血。结论:术中自体血回输能减少异体血输入量,但须适当补充新鲜血浆和凝血因子。  相似文献

Very little information is available concerning the pharmacology of type 2 astroglia. During the past decade it has become apparent that two distinct lineages of astroglial cells can be defined in vitro. These two lineages are commonly referred to as type 1 and type 2 and are distinguished from each other on the basis of their morphological features and antigenic phenotypes. In contrast to type 1 astroglia, very little is known about the pharmacology of type 2 astroglia. The lack of information concerning the responsiveness of these cells stems primarily from difficulties encountered in isolating large numbers of type 2 astroglia free of other cell types. In the present study video- and photometer-based imaging systems were used to monitor the influence of a series of neuroligands on the intracellular calcium levels of individual cerebral type 2 astroglia in order to assess their expression of calcium-mobilizing receptors. The responses of 85 immunocytochemically identified cerebral type 2 astroglia to bradykinin (BK), norepinephrine (NE), histamine (HIST), carbachol (CARB), 2-methyl-thio ATP (2MT-ATP), glutamate (GLUT), and serotonin (5-HT) were analyzed. Approximately 50% of cerebral type 2 astroglia responded to BK, NE, HIST, CARB, and 2MT-ATP whereas only 16% and 9% of the cells responded to GLUT and 5-HT, respectively. The number of neuroligands that increased calcium in individual cells ranged from 0 to 6. These responses are quite similar to those previously demonstrated in cultured cerebral type 1 astroglia. Interaction with adenosine A1 receptors is a possible contributory mechanism to the anticonvulsant effects of carbamazepine (CBZ) and the dihydropyridine calcium antagonists. We measured the binding of [3H]cyclohexyladenosine to adenosine A1 receptors in mouse brain stem, cerebellum, and cortex after oral administration of nifedipine, nimodipine (NMD), and CBZ for 7 days and compared the results with binding in control mice. Equilibrium dissociation constant (Kd) and receptor numbers (Bmax) were calculated using Scatchard and saturation isotherm analyses. Mean Kds (SEM) in control brain stem, cerebellum, and cortex were 2.09 (0.31), 2.39 (0.2), and 3.12 (0.28) nM, respectively. Results of Bmax for the same areas were 188 (26), 280 (24), and 449 (54) fmol/mg protein. Nifedipine (p less than 0.005) and NMD (p less than 0.02) raised the Kd of A1 receptors only in the cerebellum, and CBZ increased cerebellar Bmax (p less than 0.05). These minor effects on A1 receptors in CF1 mice, when given in doses previously shown to have anticonvulsant properties in these animals, do not suggest that alteration in A1 receptor activity is an important mechanism for the anticonvulsant effects of these drugs.  相似文献

Extramarital involvement (EMI) occurs with high prevalence among couples in clinical and community settings, frequently resulting in considerable distress both to participants and their spouses. The field lacks a synthesized review of this literature. Without such a synthesis, it has been difficult for researchers and clinicians to have an understanding of what is and is not known about EMI. This article reviews the large and scattered EMI literature using a framework that encompasses multiple source domains across the temporal process of engaging in and responding to EMI. In addition, this review delineates conceptual and methodological limitations to previous work in this area and articulates directions for further research.  相似文献

Objective To elucidate the effect of FasL gene expression on the proliferation and apoptosis of hypoxic rectal carcinoma cells. Methods The normoxic expression level of FasL in HR-8348 subtype cells (HR-8348B, HR-8348L, HR-8348F and HR-8348As) with different invasive power were verified by Western blot. Hypoxia models for HR-8348B, HR-8348L, HR-8348F and HR-8348As were constructed with chemical modeling, then the FasL levels in all groups at 12 h after hypoxia were quantitated by Western blot. Distribution of different cell life cycles was determined with flow cytometry. Cell reproductive activities were detected with MTT method, and cell apoptesis was assessed with TUNEL. Results FasL protein was pigmentized at the position of 40 000 by Western blot, and the expression level of FasL was significantly higher in HR-8348F cells than those in HR-8348B, HR-8348L and HR-8348As cells(F=361.149, P<0.01) in normoxia. At 12 h after hypoxia, the FasL level was also significantly higher in HR-8348F cells than those in other groups (F=278.766, P<0.01), but was not markedly different as compared to themselves in normoxia (t=1.762, P>0.05). The proliferation index was significantly higher in HR-8348F (60.43±3.72) than those in HR-8348B (40.01±3.30), HR-8348L (41.30±4.06) and HR-8348As cells (35.87±4.39), respectively (F=39.477, P<0.01). However, both inhibition rate of proliferation and apoptotic index were remarkably lower in HR-8348F (17.30±1.98 and 13.10±1.04) than those in HR-834B (33.70±4.33 and 21.60±1.31), HR-8348L (34.20±3.92 and 20.10±1.15), and HR-8348As (38.00±4.55 and 23.90±1.23), respectively (F=28.811 and 76.462, respectively, P<0.01). Conclusion The expression enhancement of intracellular FasL in rectal carcinoma in hypoxia can lead to accelerated proliferation and reduced apeptosis of cells, which will promote tumor cells to adapt microenvironmental hypoxia.  相似文献

平阳霉素加地塞米松局部注射治疗婴幼儿血管瘤   总被引:6,自引:0,他引:6  
目的探讨平阳霉素+地塞米松局部注射治疗婴幼儿血管瘤的疗效。方法将平阳霉素8 mg+地塞米松5 mg+2%利多卡因1~2 ml+生理盐水3~8 ml稀释后,行血管瘤内注射至肿胀发白为限,1次平阳霉素用量不超过8 mg。观察2周,视瘤体颜色及硬度决定是否继续用药,直至瘤体开始变硬萎缩为止,平阳霉素注射总量不超过40 mg。近两年来,用该方法治疗1~8个月龄婴幼儿体表增生期血管瘤36例,其中颜面部24例,胸部3例,背部2例,上肢5例,足2例;草莓状血管瘤30例,混合型血管瘤6例。血管瘤最小面积0.6 cm×1.2 cm,最大面积23.0 cm×12.0 cm。结果1次注射治愈2例,2~3次注射治愈25例,4~5次治愈9例,其中局部坏死1例(为足背足底大面积草莓状血管瘤),经换药及手术愈合。随访6~19个月,未见复发,除1例遗留瘢痕外,其余全部无瘢痕愈合。有效率达100%。结论平阳霉素+地塞米松局部注射治疗婴幼儿血管瘤,具有疗效显著、简便易行、安全可靠等优点,是治疗增生期血管瘤首选方法之一,值得推广。  相似文献
