Background: It is estimated that approximately 50% of individuals who incur right‐hemisphere damage (RHD) have subsequent communication disorders. Lexical‐semantic, discourse, prosodic, and pragmatic deficits have been reported following RHD, but the co‐occurrence of these deficits within the same individual has not yet been systematically investigated. Therefore clinical profiles of communication impairments in individuals with RHD still have to be identified and described in order to appreciate their communication impairment and provide strategies for rehabilitation.

Aims: The goal of the present study was to explore the clinical profiles of communication impairments subsequent to a right hemisphere lesion.

Methods and Procedures: A total of 28 French‐speaking individuals with a right‐hemisphere lesion were evaluated using the Protocole MEC (Joanette, Ska, & Côté, 2004 Joanette, Y., Ska, B. and Côté, H. 2004. Protocole Montréal d'évaluation de la communication (MEC), Isbergues: Ortho Édition.  [Google Scholar]), a normalised battery allowing the assessment of communication deficits after RHD. A hierarchical cluster analysis was used to group participants according to similarities in their results on the 14 tasks.

Outcomes and Results: Four subgroups of RHD individuals were identified on the basis of the overall similarities of performance on the 14 tasks of the Protocole MEC. Participants in the first cluster showed impairments in all four language components evaluated, whereas the second cluster of participants was also impaired in prosodic, lexical‐semantic, and pragmatic abilities, but was characterised by a relative preservation of discourse abilities. The third cluster of participants did not show any abnormal results. Finally, two individuals were mainly characterised by some lexical‐semantic deficits.

Conclusions: The Protocole MEC used in conjunction with a cluster analysis provided a first step towards the identification of communication impairment profiles among the population of individuals with RHD. In the present study it was not possible to clearly identify the relationship between a given profile and factors such as lesion site, age, or education. Incidence of communication impairments was estimated to be higher in a rehabilitation centre setting than the generally accepted 50% in the literature.  相似文献

Background: Sentence production impairment in aphasia has been attributed to several possible sources that are not mutually exclusive. Linguistic accounts often attribute the difficulty to the complexity of a verb's syntactic and/or semantic argument structure. Cognitive processing accounts emphasise the reduced processing capacity observed in agrammatic aphasia, which in turn has been attributed to reduced semantic short-term memory (STM) or slowed processing.

Aims: In this study we used verb particles and prepositions to investigate effects of differences in syntactic and semantic argument structure on sentence repetition in aphasia. We predicted that verb particles and sentences containing verb-particle constructions would be easier to repeat than prepositions and prepositional transitive sentences, as the former have a less-complex semantic and syntactic argument structure than the latter. Also, semantic and phonological spans were assessed to determine if a reduction in either capacity correlates with repetition ability.

Methods & Procedures: Participants were eight individuals with chronic aphasia. The experimental task was repetition of transitive sentences balanced for length and lexical content containing either verb particles or prepositional object structures. Accuracy of sentence repetition and repetition of verb particles and prepositions within sentences was examined. We calculated the effect of structural complexity on the sentence repetition task as the difference between proportion correct of verb-particle constructions and prepositional transitives. Semantic and phonological STM spans and word spans were also assessed and correlated with this measure of the structural complexity effect on sentence repetition.

Outcomes & Results: Verb-particle sentences were repeated correctly significantly more often than prepositional transitive sentences, and within those sentences verbal particles were repeated correctly significantly more often than prepositions. The effect was strongly associated with fluency scores: it was present in participants with low fluency scores, but not in those with high fluency scores. The phonological, but not the semantic, STM probe span measure correlated with both the difference in accurate repetition of verb-particle and prepositional transitive sentences and the particles and prepositions within those sentences.

Conclusions: Results indicate that differences in argument structure of particle and preposition constructions influence sentence repetition in agrammatic aphasia. The finding that lower fluency scores are associated with poorer performance on more complex structures suggests that this effect is associated with agrammatism. The impact of these structural distinctions between particles and prepositions should be taken into account during development of treatment stimuli for those with agrammatism.  相似文献

The specificity of an immunohistochemical reaction is guaranteed by two sets of controls. Positive controls verify the specificity of the primary antibody and demonstrate that it binds only to the protein which was used as an immunogen. Negative controls ensure that the labelling technique is specific and that the primary antibody is responsible for generation of the immunostaining. In fact, the production of a labelling may also be related to cross reactivity or to non-specific physical or chemical interactions. This paper reviews the characteristics of various epitopes and antibodies, describes different strategies which prove the specificity of the immunohistochemical reaction in research or diagnostic pathology and point towards the essential information which should be reported in a paper.  相似文献

BACKGROUND: Delineating asthma subphenotypes is of interest to understand the cause of the disease. Few studies have addressed the interrelationships of quantitative asthma-related traits. OBJECTIVE: We sought to study the interrelationships of allergy markers and FEV(1) in relation to asthma and sex in children and adults. METHODS: Total IgE levels, skin prick test (SPT) positivity, eosinophil counts, and FEV(1) were assessed in 299 asthmatic cases (children and adults) recruited in chest clinics and 309 nonasthmatic population-based control subjects in the French Epidemiological Study on the Genetics and Environment of Asthma, Bronchial Hyperresponsiveness, and Atopy. RESULTS: Allergy parameters were significantly higher in asthmatic cases than in control subjects for children and adults and for both sexes. Sex and age modified the pattern of concordance of high IgE levels, SPT positivity, and eosinophilia among asthmatic cases, with the greatest overlap in male children (64%) and the lowest in male adults (18%). Patterns of change over the lifespan of IgE levels, eosinophil counts, and FEV(1)/height(2) varied, with the acceleration of FEV(1) decrease being particularly evident in asthmatic adults. In adult cases and control subjects, SPT positivity (particularly to indoor allergens) was significantly related to IgE levels but not to eosinophil counts. The association of eosinophil counts with IgE levels was evident only in children. Environmental factors (smoking, pets, and country living) did not alter the patterns observed. CONCLUSIONS: Each allergy-related phenotype showed a distinct relation with asthma, with the role for eosinophils being different than that for IgE levels and SPT responses. CLINICAL IMPLICATIONS: Taking age and sex into account is essential for understanding the interrelationships of the various allergy-related phenotypes to asthma status.  相似文献

Objectives

There is increasing attention for dietary patterns as a potential strategy to prevent cognitive decline. We examined the association between adherence to a recently developed Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) diet with cognitive function and cognitive decline, taking into account the interaction between the apolipoprotein E ε4 genotype and the MIND diet.

Design

Population-based prospective cohort study.

Participants

A total of 16,058 older women aged 70 and over from the Nurses’ Health Study.

Measurements

Dietary intake was assessed five times between 1984 and 1998 with a 116-item Food Frequency Questionnaire. The MIND score includes ten brain-healthy foods and five unhealthy foods. Cognition was assessed four times by telephone from 1995 to 2001 (baseline) with the Telephone Interview for Cognitive Status (TICS) and by calculating composite scores of verbal memory and global cognition. Linear regression modelling and linear mixed modelling were used to examine the associations of adherence to the MIND diet with average cognitive function and cognitive change over six years, respectively.

Results

Greater long-term adherence to the MIND diet was associated with a better verbal memory score (multivariable-adjusted mean differences between extreme MIND quintiles=0.04 (95%CI 0.01-0.07), p-trend=0.006), but not with cognitive decline over 6 years in global cognition, verbal memory or TICS.

Conclusion

Long-term adherence to the MIND diet was moderately associated with better verbal memory in later life. Future studies should address this association within populations at greater risk of cognitive decline.

Although type 1 diabetes historically has been more common in patients eight to 19 years of age, type 2 diabetes is emerging as an important disease in this group. Type 2 diabetes accounts for 8 to 45 percent of new childhood diabetes. This article is an update from the National Diabetes Education Program on the management of type 2 diabetes in youth. High-risk youths older than 10 years have a body mass index greater than the 85th percentile for age and sex plus two additional risk factors (i.e., family history, high-risk ethnicity, acanthosis nigricans, polycystic ovary syndrome, hypertension, or dyslipidemia). Reducing overweight and impaired glucose tolerance with increased physical activity and healthier eating habits may help prevent or delay the development of type 2 diabetes in high-risk youths. The American Academy of Pediatrics does not recommend population-based screening of high-risk youths; however, physicians should closely monitor these patients because early diagnosis may be beneficial. The American Diabetes Association recommends screening high-risk youths every two years with a fasting plasma glucose test. Patients diagnosed with diabetes should receive self-management education, behavior interventions to promote healthy eating and physical activity, appropriate therapy for hyperglycemia (usually metformin and insulin), and treatment of comorbidities.  相似文献

The role of retinal microglial cells (MCs) in age-related macular degeneration (AMD) is unclear. Here we demonstrated that all retinal MCs express CX3C chemokine receptor 1 (CX3CR1) and that homozygosity for the CX3CR1 M280 allele, which is associated with impaired cell migration, increases the risk of AMD. In humans with AMD, MCs accumulated in the subretinal space at sites of retinal degeneration and choroidal neovascularization (CNV). In CX3CR1-deficient mice, MCs accumulated subretinally with age and albino background and after laser impact preceding retinal degeneration. Raising the albino mice in the dark prevented both events. The appearance of lipid-bloated subretinal MCs was drusen-like on funduscopy of senescent mice, and CX3CR1-dependent MC accumulation was associated with an exacerbation of experimental CNV. These results show that CX3CR1-dependent accumulation of subretinal MCs evokes cardinal features of AMD. These findings reveal what we believe to be a novel pathogenic process with important implications for the development of new therapies for AMD.  相似文献

PurposeThe article identifies the aspects of health and outcomes that are considered important from the perspective of ambulatory children with cerebral palsy (CP) and their parents regarding lower limb orthopaedic surgery and explores how they experience surgical interventions.MethodsFour databases (Embase, MEDLINE (Ovid), CINAHL and PsycINFO) were searched from inception to 11 April 2020. Studies were included if they: 1) they involved children or young adults diagnosed with ambulant CP or their family, 2) participants had experience with lower limb orthopaedic surgery and 3) studies employed qualitative research methods. The Critical Appraisal Skills Programme was used to appraise identified studies. The ‘Best-fit framework’ synthesis approach was used by applying the International Classification of Functioning-Children and Youth (ICF–CY) linking rules and thematic synthesis. The review process was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines.ResultsSix studies were included. Four themes were generated which were linked to the ICF–CY framework: Body function and structure, Activity and participation, Environmental factors, Personal factors, as well as non-ICF–CY themes including Emotional well-being and Goal setting. Important surgical outcomes identified were pain, fatigue, movement-related function, mobility, walking ability, community life, emotional well-being, and adequate provision of public and health services.ConclusionThese findings are important for understanding patient-centred outcomes in lower limb ortho-paedics surgery and providing focus for future interventional studies aimed at improving outcomes of importance to children with CP. These findings highlight the importance of long-term support to help people negotiate the challenge of surgical regimes and to achieve good outcomes after orthopaedic surgery.
