IntroductionThere is a scarcity of data on the impact of behavioral habits, such as exercise, on physical health in patients with bladder cancer. We investigated the association of exercise on self-reported physical health status and examined the prevalence of bladder cancer patients with sedentary lifestyle.MethodsWe examined cross-sectional data of participants diagnosed with bladder cancer within the Behavioral Risk Factor Surveillance System (BRFSS) from 2016–2020. Patient health status was surveyed using self-reported measures, such as the total days per month when their “physical health is not good.” The primary outcome was patient-reported poor physical health for more than 14 days within a one-month period.ResultsOut of 2 193 981 survey participants, we identified 936 with a history of bladder cancer. Nearly one in three bladder cancer patients reported being sedentary within the last month, as a total of 307 (32.8%) patients reported no exercise within the last 30 days. The remaining 628 (67.2%) reported exercising for at least one day within the last month. In multivariable logistic regression model analysis, we found that exercise is protective for self-reported poor physical health status (odds ratio 0.37, 95% confidence interval 0.25–0.56, p<0.001). Patients that exercised were less likely to report bad physical health.ConclusionsApproximately one in three bladder cancer patients report no exercise within 30 days, suggesting a sedentary lifestyle. Patients that are active are less likely to self-report poor physical health status. Implementation of exercise programs for bladder cancer patients could be promising in improving health status.

IntroductionLipopolysaccharide (LPS) is widely used to induce experimental animals. However, its effects on cardiac contraction is controversial. Although LPS probably induces its influence in vivo both directly and indirectly, we focused on the direct effects of LPS in this report.Material and methodsIsolated ventricular myocytes mounted on a Langendorff apparatus were perfused with LPS. The changes in cultured H9c2 cells incubated with LPS over a 3-h exposure were compared with the changes after a 24-h incubation. Apoptosis was identified using flow cytometry and Western blotting. The mRNA levels were also determined.ResultsLPS directly stimulated cardiac contractility at low doses, although it produced inhibition at higher doses. The TLR4-coupled JAK2/STAT3 pathway was identified in H9c2 cells after LPS treatment, with an increase in intracellular calcium levels. LPS dose-dependently activated hypertrophic signals in H9c2 cells and induced apoptosis at the high dose. However, apoptosis was observed in H9c2 cells after a 24-h exposure to LPS, even at low doses. This observation appears to be associated with the level of paracrine cytokines. Changes in H9c2 cells by LPS were diminished by NPS2390, an inhibitor of the calcium-sensing receptor (CaSR). LPS also promoted CaSR mRNA expression in H9c2 cells, which may be unrelated to the changes in cytokine expression influenced by an inflammasome inhibitor.ConclusionsIn contrast to the isolated hearts, LPS activated hypertrophic signals prior to apoptotic signals in cardiac cells. Thus, LPS injury appears to be associated with CaSR, which was not markedly influenced by an inflammasome inhibitor.  相似文献

Associations between breastfeeding intention, duration and post‐natal depression (PND) have been shown in pre‐COVID‐19 studies. However, studies during COVID‐19 have not examined the associations between breastfeeding intention, breastfeeding practices, and PND in an international sample of post‐natal women, taking into consideration COVID‐19 related factors. This is the first study to address this gap as both PND and breastfeeding may be affected by COVID‐19, and have important long‐term effects on women''s and infant''s health. A cross‐sectional internet‐based survey was conducted with 3253 post‐natal women from five countries: Brazil, South Korea, Taiwan, Thailand, and the United Kingdom from July to November 2021. The results showed that women who intended to breastfeed during pregnancy had lower odds of having PND than women who did not intend to. Women who had no breastfeeding intention but actually breastfed had greater odds (AOR 1.75) of having PND than women who intended to breastfeed and actually breastfed. While there was no statistical significance in expressed breast milk feeding in multivariable logistic regression models, women who had shorter duration of breastfeeding directly on breast than they planned had greater odds (AOR 1.58) of having PND than those who breastfed longer than they planned even after adjusting for covariates including COVID‐19‐related variables. These findings suggested the importance of working with women on their breastfeeding intention. Tailored support is required to ensure women''s breastfeeding needs are met and at the same time care for maternal mental health during and beyond the pandemic.  相似文献

BackgroundToll-like receptors (TLRs) are important components of the innate and adaptive immune systems, and abnormal TLR expression has been linked to a variety of cancers. However, there was a lack of clarity on the association of TLR stimulation with the carcinogenesis of cancer. The study''s goal was to analyse the clinical importance of TLRs expression at the mRNA level in pan-cancer datasets, as well as the link between TLR expression and carcinogenesis, progression, and clinical prognosis.MethodsThe expression profile of TLRs derived from UCSC pan-cancer data was analysed in multiple dimensions, including clinical analysis, immunological subtype analysis, tumour microenvironment (TME) analysis, tumour stem cell correlation analysis, and drug sensitivity analysis. Additionally, we analyse protein-protein interactions, functional enrichment, and chromatin accessibility, as well as TLR expression in single-cell sequencing data.ResultsOur multi-omics analysis results imply that TLRs may operate as a biological marker for carcinogenesis and progression, a potential target for anti-tumour therapy, and a prognostic biomarker, laying the theoretical groundwork for future translational medicine research.ConclusionTLRs are involved in the formation of malignancies and can be explored in further detail as potential prognostic indicators.

目的:探讨山茱萸多糖对乳腺癌MCF-7细胞增殖、凋亡、迁移、侵袭的影响,并通过观察山茱萸多糖对腺苷酸活化蛋白激酶/沉默信息调节因子1( AMPK/SIRT1)通路的调控作用探索其可能的作用机制。方法:体外培养MCF-7细胞,随机分为对照组(正常培养基培养)和山茱萸多糖低、中、高剂量组(分别予12.5、25.0、50.0 mg/mL山茱萸多糖培养)以及山茱萸多糖高剂量+AMPK激活剂( AICAR)组(予50.0 mg/mL山茱萸多糖+2.0 mmol/L AICAR培养),采用平板克隆形成试验检测MCF-7细胞增殖情况,流式细胞术检测细胞凋亡情况,划痕试验检测细胞迁移情况,Transwell法检测细胞侵袭情况,蛋白免疫印迹法( Western blot法)检测各组细胞AMPK、磷酸化AMPK( p-AMPK)、SIRT1蛋白相对表达量。选择BALB/c裸鼠,建立荷瘤裸鼠模型,随机分为对照组(灌胃生理盐水)与山茱萸多糖组(每日灌胃800 mg/kg山茱萸多糖溶液),每组6只,连续灌胃28 d。每7 d测量2组裸鼠肿瘤体积并做组间比较,灌胃结束后处死各组裸鼠取移植瘤,采用免疫组化法检测各组裸鼠移植瘤组织中AMPK、p-AMPK、SIRT1蛋白表达。结果:体外细胞学实验结果表明,山茱萸多糖低、中、高剂量组MCF-7细胞集落形成数明显低于对照组( P< 0.05),细胞凋亡率明显高于对照组( P< 0.05),细胞划痕愈合率、细胞侵袭数明显低于对照组( P< 0.05),细胞中p-AMPK、SIRT1蛋白表达明显低于对照组( P< 0.05) ;山茱萸多糖高剂量+AICAR组MCF-7细胞集落形成数明显高于山茱萸多糖高剂量组( P< 0.05),细胞凋亡率明显低于山茱萸多糖高剂量组( P< 0.05),细胞划痕愈合率、细胞侵袭数明显高于山茱萸多糖高剂量组( P< 0.05),细胞中p-AMPK、SIRT1蛋白表达明显高于山茱萸多糖高剂量组( P< 0.05) ;山茱萸多糖各剂量对上述指标的影响表现出显著的剂量依赖性( P< 0.05)。荷瘤裸鼠实验结果表明,移植第14、21、28、35 d山茱萸多糖组裸鼠移植瘤体积显著小于对照组( P< 0.05) ;干预结束后,山茱萸多糖组裸鼠移植瘤质量与瘤体组织p-AMPK、SIRT1蛋白阳性表达率均明显低于对照组( P< 0.05),AMPK蛋白阳性表达率与对照组比较差异无统计学意义( P> 0.05)。结论:山茱萸多糖对乳腺癌MCF-7细胞增殖、迁移、侵袭有一定的抑制作用,并可促进MCF-7细胞凋亡,能抑制裸鼠移植瘤生长,其可能的机制为抑制AMPK/SIRT1通路激活。  相似文献

Background:The aim of this study was to investigate the clinical effect and safety of accelerated-strategy initiation of renal replacement therapy (RRT) in critically ill patients.Methods:PubMed, Embase, OVID, EBSCO, and the Cochrane Library databases were searched for relevant articles from inception to December 30, 2020. Only RCTs that compared the clinical efficacy and safety between accelerated-strategy RRT and standard-strategy RRT among critically ill adult patients with acute kidney injury (AKI) were included. The primary outcome was 28-day mortality.Results:A total of 5279 patients in 12 RCTs were included in this meta-analysis. The 28-day mortality rates of patients treated with accelerated and standard RRT were 37.3% (969/2596) and 37.9% (976/2573), respectively. No significant difference was observed between the groups (OR, 0.92; 95% CI, 0.70–1.12; I2 = 60%). The recovery rates of renal function were 54.5% and 52.5% in the accelerated- and standard-RRT groups, respectively, with no significant difference (OR, 1.03; 95% CI, 0.89–1.19; I2 = 56%). The rate of RRT dependency was similar in the accelerated- and standard-RRT strategies (6.7% vs 5.0%; OR, 1.11; 95% CI, 0.71–1.72; I2 = 20%). The accelerated-RRT group displayed higher risks of hypotension, catheter-related infection, and hypophosphatemia than the standard-RRT group (hypotension: OR, 1.26; 95% CI, 1.10–1.45; I2 = 36%; catheter-related infection: OR, 1.90; 95% CI, 1.17–3.09; I2 = 0%; hypophosphatemia: OR, 2.11; 95% CI, 1.43–3.15; I2 = 67%).Conclusions:Accelerated RRT does not reduce the risk of death and does not improve the recovery of kidney function among critically ill patients with AKI. In contrast, an increased risk of adverse events was observed in patients receiving accelerated RRT. However, these findings were based on low quality of evidence. 目的 比较非肥胖型与肥胖型非酒精性脂肪性肝病(NAFLD)患者代谢特征和肝脂肪变程度差异。方法 2015年1月~2021年6月我院诊治NAFLD患者274例,根据体质指数(BMI)=25 kg/m2,将其分为非肥胖型66例和肥胖型208例。计算胰岛素抵抗指数(HOMA-IR),行磁共振仪检查,应用仪器自带软件计算肝脏脂肪含量(LFC)和腹部皮下脂肪厚度(ASFT),使用FibroTouch测量肝脏受控衰减参数(CAP),使用多普勒超声诊断仪检测颈动脉内膜中层厚度(CIMT),将肝脂肪变分为Ⅰ级、Ⅱ级和Ⅲ级。结果 非肥胖型NAFLD患者BMI为(23.4±1.4)kg/m2、血清丙氨酸氨基转移酶水平为24.2(19.6,35.8)U/L,天冬氨酸氨基转移水平为25.0(18.7,32.3)U/L,甘油三酯为(1.9±0.4)mmol/L,高密度脂蛋白为(1.3±0.4)mmol/L,空腹血糖为(5.0±0.7)mmol/L,空腹胰岛素为7.0(4.8,9.4)μU/mL,HOMA-IR为1.6(1.1,2.3),尿酸为376.5(294.3,4...  相似文献
