Depressive disorder is a common consequence of interferon α treatment. An understanding of the aetiological processes involved is evolving. HPA axis abnormalities are clearly described in community depressive disorder and represent vulnerability to depression development. We explored whether pre-treatment HPA axis abnormalities influence depression emergence during interferon α treatment. We examined waking HPA axis response via salivary cortisol sampling in 44 non-depressed, chronic hepatitis C infected patients due to commence standard interferon α treatment. Hamilton depression scales and the structured clinical interview for DSM-IV major depressive disorder status were administered monthly during treatment. Major depressive disorder developed in 26 of 44 subjects during interferon-α treatment. The pre-treatment waking cortisol response over 1 h was significantly greater in the subsequent switch to depression group (F=4.23, p=0.046). The waking cortisol response pre-treatment with interferon α appears greater in those subsequently switching to depressive disorder during treatment. This waking response may join other vulnerability factors for depression emergence in this group. This model could prove a valuable tool in understanding non-iatrogenic depressive disorder in the general population and notably the role of cytokines.  相似文献

As many as 29% of women suffer from migraine headache, yet it remains a poorly understood phenomenon. Our purpose in conducting this pilot study was to determine the relationships among migraine pain, disability, depressive symptomatology, and coping in women. A convenience sample of 34 women was recruited from university and workplace populations. Nineteen women met the International Headache Society criteria for migraine, while 15 women served as a nonmigraine comparison group. Participants completed eight instruments measuring migraine pain, disability, depressive symptomatology, and coping. The two groups of women were not significantly different on demographic variables. Migraineurs scored significantly higher for pain characteristics, disability, depressive symptomatology, and total coping scores.  相似文献

Three assumptions guiding research and clinical intervention strategies for people coping with sudden, traumatic loss are that (a) people confronting such losses inevitably search for meaning, (b) over time most are able to find meaning and put the issue aside, and (c) finding meaning is critical for adjustment or healing. We review existing empirical research that addresses these assumptions and present evidence from a study of 124 parents coping with the death of their infant and a study of 93 adults coping with the loss of their spouse or child to a motor vehicle accident. Results of these studies indicate that (a) a significant subset of individuals do not search for meaning and yet appear relatively well-adjusted to their loss; (b) less than half of the respondents in each of these samples report finding any meaning in their loss, even more than a year after the event; and (c) those who find meaning, although better adjusted than those who search but are unable to find meaning, do not put the issue of meaning aside and move on. Rather, they continue to pursue the issue of meaning as fervently as those who search but do not find meaning. Implications for both research and clinical intervention are discussed.  相似文献

Background

Antibiotic delivery to patients with fever and neutropenia (F&N) in <60 min is an increasingly important quality measure for oncology centers, but several published reports indicate that a time to antibiotic delivery (TTA) of <60 min is quite difficult to achieve. Here we report a quality improvement (QI) effort that sought to decrease TTA and assess associated clinical outcomes in pediatric patients with cancer and F&N.

Procedure

We used Lean‐Methodology and a Plan‐Do‐Study‐Act approach to direct QI efforts and prospectively tracked TTA measures and associated clinical outcomes (length of stay, duration of fever, use of imaging studies to search for occult infection, bacteremia, intensive care unit (ICU) consultation or admission, and mortality). We then performed statistical analysis to determine the impact of our QI interventions on total TTA, sub‐process times, and clinical outcomes.

Results

Our QI interventions significantly improved TTA such that we are now able to deliver antibiotics in <60 min nearly 100% of the time. All TTA sub‐process times also improved. Moreover, achieving TTA <60 min significantly reduced the need for ICU consultation or admission (P = 0.003) in this population.

Conclusion

Here we describe our QI effort along with a detailed assessment of several associated clinical outcomes. These data indicate that decreasing TTA to <60 min is achievable and associated with improved outcomes in pediatric patients with cancer and F&N. Objectives. We assessed the effects of a worksite multiple-component intervention addressing diet and physical activity on employees’ mean body mass index (BMI) and the percentage of employees who were overweight or obese.Methods. This group-randomized trial (n = 3799) was conducted at 10 worksites in the northeastern United States. Worksites were paired and allocated into intervention and control conditions. Within- and between-groups changes in mean BMIs and in the percentage of overweight or obese employees were examined in a volunteer sample.Results. Within-group mean BMIs decreased by 0.54 kilograms per meter squared (P = .02) and 0.12 kilograms per meter squared (P = .73) at the intervention and control worksites, respectively, resulting in a difference in differences (DID) decrease of 0.42 kilograms per meter squared (P = .33). The within-group percentage of overweight or obese employees decreased by 3.7% (P = .07) at the intervention worksites and increased by 4.9% (P = .1) at the control worksites, resulting in a DID decline of 8.6% (P = .02).Conclusions. Our findings support a worksite population strategy that might eventually reduce the prevalence of overweight and obesity by minimizing environmental exposures to calorically dense foods and increasing exposures to opportunities for energy expenditure within worksite settings.Sixty-eight percent of adults residing in the United States are overweight or obese,1 and these conditions affect more than 1.4 billion adults worldwide.2 Traditional obesity control strategies, which have focused on changing diet and physical activity (PA) behaviors, provide significant individual benefits3 but are considered insufficient to reduce population disease burdens,4,5 for which broad, population-based approaches are needed.6 In addition to individual biology and behaviors, the physical, social, and cultural environment appears to contribute to the upward trend in population estimates of overweight and obesity7,8 by facilitating high-energy, low-nutrient diets and reducing the need to be physically active to perform activities of daily life.9Worksites are feasible self-contained environments with established communication systems in which interventions manipulating the food and PA environment and the social marketing of lifestyle changes can be implemented. Given that 58.4% of the US population aged 16 years or older is employed,10 worksite interventions have the potential to reach large number of adults11 and can foster the participation of employees in project development and sustainability.12–14 Moreover, participatory worksite interventions address workers’ needs, priorities, and interests and allow strategies to be adapted to the realities of individual sites.13 There is also a business case for weight control programs. In comparison with their nonobese counterparts, overweight and obese employees have higher absenteeism rates, have more work limitations, and are less productive.15–18With these issues in mind, the National Heart, Lung, and Blood Institute developed the Obesity Prevention in the Worksite initiative, a population-based approach to promoting behavioral change through environmental interventions that address prevention and control of weight gain.19 Prior to this initiative, worksite trials were either limited scope interventions, targeting a few aspects of the food or PA environment,9,20–23 or broader scope efforts simultaneously targeting risk factors for cardiovascular disease and cancer (e.g., smoking, diet).24–28 In addition, few studies addressed environmental influences related to excessive weight gain.Here we report the results of Images of a Healthy Worksite, one of the studies that is part of the Obesity Prevention in the Worksite initiative; this comprehensive nutrition and PA intervention was designed to promote healthy lifestyles and to stop the shift to the right of the population body mass index (BMI) curve. In this study, worksites were designated to receive an environmental intervention, and employees participated in intervention design. We hypothesized that mean BMIs among employees at the intervention worksites and the percentages of employees who were overweight or obese would not increase over a 2-year period or would increase less than at control worksites.  相似文献

La Crosse virus (LACV), a leading cause of arboviral encephalitis in children in the United States, is emerging in Appalachia. For local arboviral surveillance, mosquitoes were tested. LACV RNA was detected and isolated from Aedes japonicus mosquitoes. These invasive mosquitoes may significantly affect LACV range expansion and dynamics.  相似文献
