Three-dimensional topography of the optic nerve head can be objectively described by means of quantitative parameter values. Owing to interindividual variability in the disc configuration, new structural variables, such as the relative height of the peripapillary retinal nerve fiber layer in stereo-videography and height variations in the contour line along the disc border in laser scanning tomography, have been shown to differentiate normal optic discs and optic nerve heads with pathological alterations better than standard volumetric data. Using laser scanning tomography, the effect of age on the parameter values of disc area, rim area and the ratio of the cup-to-disc area was evaluated in 61 healthy subjects (age 14 to 77) who had normal visual fields on computerized static perimetry. The rim area decreased with age (rs = -0.26, P < 0.05); the ratio of the cup area to disc area increased (rs = 0.25; P < 0.05). No relationship was found between patient age and disc area (Spearman correlation). Although the coefficients of correlation are rather small, they may reflect the physiological age-related decline in axonal count. The high resolution of strictly confocal laser scanning microscopy allows for visualization of nerve fiber bundle defects underneath a surface of regular reflectivity. The existence of such hidden nerve fiber bundle defects has been postulated before on the basis of experimental studies and may be an early morphological sign of glaucomatous damage.  相似文献

Intraocular pressure following intravitreal silicone oil injection   总被引:5,自引:0,他引:5  
The clinical features of 100 consecutive cases and the histopathology of six enucleated eyes, all having undergone silicone oil injection for complicated retinal detachments, were studied with regard to the incidence and mechanisms of postoperative intraocular pressure changes. Preoperatively, five patients had glaucoma and 35 had hypotony (7 mm Hg or less). Immediately after surgery, 43 patients had glaucoma and 14 had hypotony, while seven had glaucoma and 39 had hypotony in the late postoperative period (average follow-up of 1 year). In most cases, the mechanism of intraocular pressure change did not appear to be directly related to the presence of silicone, but rather to the preoperative status of the eye and other aspects of the vitreoretinal surgery. When silicone was a factor, the resulting intraocular pressure appeared to represent a balance between mechanisms obstructing aqueous outflow (pupillary block and the reaction to silicone bubbles in the anterior chamber angle) and mechanisms reducing aqueous production (cyclitic membranes and retinal detachment).  相似文献

The objective of this study was to determine the association of plasma levels of uric acid, an endogenous antioxidant, in women with cervical intraepithelial neoplasia (CIN), while controlling for the confounding effects of human papillomavirus (HPV) infection, age, smoking, and use of oral contraception. Plasma-reduced and oxidized uric acid levels were determined in 650 women by high-performance liquid chromatography, employing electrochemical technique. The findings demonstrated that 1) plasma-reduced uric acid (PRUA) levels in women with CIN (n = 311) were significantly lower (P < 0.05) compared with women in a control group (n = 339); 2) according to multiple logistic regression analysis, PRUA levels were negatively (P = 0.0113) and HPV infection were positively associated (P < 0.0001) with CIN, after controlling for the confounding effects of the studied factors; 3) according to multiple regression analysis, there was a 31% decrease in CIN risk for each incremental increase of 1mg/dl of PRUA; and 4) according to polychotomous logistic regression analysis, independent of HPV infection, PRUA level was inversely associated with the histopathological graded severity of CIN. We have previously reported decreased plasma levels of exogenous antioxidants, for example, vitamins C and E, in women with CIN independent of HPV infection. The data suggest that plasma deficiencies of several antioxidants in HPV-infected uterine cervical tissue may create an oxidative environment that renders the tissue susceptible to free radical damage. It may be speculated that chronic free radical-induced tissue damage in the context of persistent HPV infection may be involved in the pathogenesis of CIN.  相似文献

OBJECTIVE: To investigate the prevalence of human papillomavirus (HPV) infection in normal oral mucosa, and to observe the natural history in the oral cavity in oral swab samples collected from healthy volunteers on Miyako Island, Okinawa, Japan. STUDY DESIGN: The prevalence of HPV infection in oral buccal mucosa cell scrapes collected between 2000 and 2002 from a cohort of 668 healthy volunteers was determined. HPV DNA was detected by consensus polymerase chain reaction (PCR) using MY09/MY11 primers followed by direct cycle sequencing. Just over 2 years later the HPV-positive participants were reevaluated. RESULTS: Of the 668 subjects, 662 samples were analyzed for HPV. HPV DNA was detected in 4 (0.6%) specimens. HPV type 16 (HPV16), HPV53, and HPV71, mucosal types, and HPV12, a cutaneous type, were all identified by direct sequencing. In the follow-up survey, the HPV71- and HPV12-positive participants again tested positive, while HPV DNA was not detected in the HPV16- and HPV53-positive participants. CONCLUSION: The results of this study among healthy individuals from Miyako Island suggest that oral HPV infection is uncommon. In this cohort, HPV71 and HPV12 were persistent, while HPV16 and HPV53 were transient in normal oral mucosa.  相似文献

Brain function and selenium concentration are well maintained in rodents under conditions of selenium deficiency. Recently, however, targeted deletion of the selenoprotein P gene (Sepp) has been associated with a decrease in brain selenium concentration and with neurological dysfunction. Studies were conducted with Sepp(-/-) and Sepp(+/+) mice to characterize the neurological dysfunction and to correlate it with dietary selenium level. When weanling Sepp(-/-) mice were fed the basal diet (<0.01 mg/kg selenium) supplemented with 0, 0.05 or 0.10 mg selenium/kg, they developed spasticity that progressed and required euthanasia. Supplementing the diet with > or =0.25 mg selenium/kg prevented the neurological dysfunction. To determine whether neurological dysfunction would occur in more mature Sepp(-/-) mice deprived of selenium, Sepp(-/-) mice that had been fed the basal diet supplemented with 1.0 mg selenium/kg for 4 wk were switched to a selenium-deficient diet. Within 3 wk they had developed neurological dysfunction and weight loss. At 3 wk, the 1.0 mg selenium/kg diet was reinstituted. Neurological function stabilized but did not return to normal. Brain selenium concentration did not increase. Weight gain resumed. This study shows that neurological dysfunction occurs when selenium supply to the brain is curtailed and that the dysfunction is not readily reversible. Both the absence of selenoprotein P and a low dietary selenium supply are necessary for the dysfunction to occur, indicating that selenoprotein P and at least one other form of selenium supply the element to the brain.  相似文献

Effects of common monoterpenoid alcohols and ketones were investigated on recombinant human gamma-aminobutyric acid A (GABAA; alpha1beta2gamma2s) and glycine (alpha1 homomers) receptors expressed in Xenopus oocytes. GABA currents were enhanced by coapplications of 10-300 microM: (+)-menthol>(-)-menthol>(-)-borneol>(-)-menthone=camphor enantiomers>carvone enantiomers, with menthol acting stereoselectively. By contrast, thujone diastereomers inhibited GABAA receptor currents while glycine currents were only markedly potentiated by menthol. Positive modulation by (+)-menthol was explored given its pronounced effects (e.g., at 100 microM, GABA and glycine EC20 responses increased by 496+/-113% and 135+/-56%, respectively). (+)-Menthol, 100 microM, reduced EC50 values for GABA and glycine from 82.8+/-9.9 to 25.0+/-1.8 microM, and from 98.7+/-8.6 to 75.7+/-9.4 microM respectively, with negligible effects on maximal currents. This study reveals a novel neuroactive role for menthol as a stereoselective modulator of inhibitory ligand-gated channels.  相似文献
