Laparoscopic appendicectomy — A successful operation in adults and children

Laparoscopic appendicectomy has been the subject of several encouraging reports, but has not as yet gained widespread acceptance. We present a series of 159 consecutive laparoscopic appendicectomies performed, over a 4 yr period, in both adults and children. We find the procedure as safe as its open counterpart, with patients fit to leave hospital within the same time period. Perforated appendices were amenable to this procedure, and the location of the appendix did not alter the outcome. Children responded as well as adults post-operatively. Obesity may be an indication for this form of treatment. Removal of displaced faecoliths associated with perforated appendicitis is a difficult technical problem in less than 5 per cent of patients.

采用高渗盐液，抗休克裤及两者联用纠正大鼠早期失血性休克。结果显示，单独使用ＡＳＴ，仅见动脉血压短时间回升，而单用ＨＴＳ，血压则明显回升。若先用ＡＳＴ，再注入ＨＴＳ，也能取得明显效果。失血休克大鼠血浆纤维连接蛋白水平显著降低，各组之间均无差异血浆脂质过氧化物－丙二醛的含量明显增加，且在ＨＴＳ组，ＨＴＳ＋ＡＳＴ组与ＮＳ对照组之间有显著差异。     

五味子酚对大鼠嗜中性白细胞功能与形态的作用(英文)

目的:探讨五味子酚(Sal)对大鼠嗜中性白细胞(Neu)功能的调节作用.结果:Sal 1,10,100μmol·L~(-1)剂量依赖性抑制Neu功能,Sal 100μmol·L~(-1)使Neu表面伪足和皱褶消失,并可降低细胞内钙离子浓度、升高细胞内cAMP水平.结论:Sal可通过影响细胞内钙离子浓度、cAMP水平及细胞表面形态抑制Neu的功能.     

Phase I and pharmacologie study of liposomal daunorubicin (DaunoXome)

Summary We have completed a phase I and pharmacology study of liposomally-encapsulated daunorubicin (DaunoXome). Of 32 patients entered, 30 were evaluable. No toxicity was encountered at the initial doseescalation steps from 10 to 60 mg/m². At 80 mg/m², two patients manifested grade 2 neutropenia. At least grade 3 neutropenia occurred in all patients receiving 120 mg/m². Alopecia and subjective intolerance were mild. Cardiotoxicity was not observed except for an episode of arrhythmia in a patient with lung cancer and prior radiation. Only one minor objective response was observed in this population of refractory solid tumors. Pharmacokinetics differed from those of the free drug with no detection of daunorubicinol. We recommend future phase II studies with a dose of 100 mg/m² in previously treated and 120 mg/m² of DaunoXome in previously untreated patients with solid tumors.EDW is supported in part by ACS award 92-14-1     

消化性溃疡的皮质醇分泌异常

采用放射免疫测定法研究了70例非应激性消化性溃疡患者的皮质醇分泌状态;并比较了患者与84例正常人的血清皮质醇浓度及其昼夜分泌节律。发现非应激性消化性溃疡血清皮质醇浓度明显高于健康对照组(P<0.01);然昼夜分泌节律相仿。结果提示,非应激性消化性溃疡有皮质醇分泌的明显异常。     

A review of incisional hernia repairs: preoperative weight loss and selective use of the mesh repair

At the Shouldice Clinic pre-operative weight loss is used prior to incisional hernia repairs. Mesh repair is selectively used, based on specific hernia characteristics. A series of 236 patients were reviewed and followed up for 36 months. Data were available on 188 patients (80%). There were 15 recurrences (8%). The number of obese patients was reduced from 67 (35.6%) to 25 (13.3%) through the weight loss program. The hernia diameter, gastrointestinal complications, and surgical site infection were significantly related to recurrence but not the type of repair, obesity, location, or previous recurrences. The risk factors of incisional hernias include size, intestinal complications and infections. A selective use has a comparable result to the exclusive use of mesh repair. Weight reduction has yet to be shown to affect the rate of recurrence, and further prospective studies are required.
