The distally based sural fasciomusculocutaneous flap for foot reconstruction.

Finding appropriate soft-tissue to cover a wound located over the middle or distal portion of the foot can be challenging. A distally based sural fasciomusculocutaneous flap including the sural nerve and a midline cuff of the gastrocnemius muscle can be used for this purpose. This flap is designed on the proximal third of the posterior calf and is nourished in a retrograde manner by the lower peroneal septocutaneous perforators, through the vascular axis of the sural nerve to the musculocutaneous perforators of the gastrocnemius muscle. Between October of 2002 and January of 2004, this flap was applied in nine individuals, including four diabetic patients. The skin defects all resulted from trauma, osteomyelitis or chronic ulcer, and combined with bone or tendon exposure. One flap developed distal necrosis. The other flaps survived fully and provided good contour. In our series, diabetes mellitus seemed not to compromise the vascularity of the flap. The distally based sural fasciomusculocutaneous flap is very useful for lower limb reconstruction, particularly for the foot, because of its long vascular pedicle and the availability of the skin portion of the proximal calf based on direct branches between the musculocutaneous perforators and the neurovascular axis of the sural nerve. This is an important variant of the sural neurocutaneous flap and it appears to be a good alternative to free flaps for resurfacing the foot.     

胃肠道间质瘤的预后

随着对胃肠道间质瘤（gastrointestinal stromal tumor，GIST）分子生物学的认识和激酶抑制剂类靶向治疗药物的诞生，GIST的预后也发生了明显改善，疾病缓解率和手术切除率显著提高，生存期延长。不同的临床病理因素和治疗方法对GIST病人生存期具有根本影响。     

三磷酸腺苷结合盒运转体A1与颈动脉粥样硬化斑块的关系

目的 探讨三磷酸腺苷结合盒运转体A1(ATP binding cassette transporter A1,ABCA1)在人颈动脉粥样硬化斑块中的表达变化及作用机制.方法 收集24例人颈动脉粥样硬化斑块标本和10例肠系膜动脉标本(对照组),采用RT-PCR测定ABCA1 mRNA和视黄酸X受体α(RXRα)mRNA表达水平,并采用Western Blot检测ABCA1及RXRα的蛋白表达水平.24例人颈动脉粥样硬化斑块标本按病理分级,比较病理组织为Ⅲ级和Ⅰ级动脉粥样硬化组织间ABCA1 mRNA、RXRαmRNA表达水平及蛋白表达水平.结果 颈动脉粥样硬化斑块组的ABCA1 mRNA(0.79±0.04)和RXRα mRNA(0.73±0.04)表达与对照组相比上调,差异有统计学意义(P＜0.05);ABCA1 mRNA与RXRα mRNA增加水平相关(P＜0.05);颈动脉粥样硬化斑块的ABCA1蛋白表达(0.22±0.03)下调水平与对照组(0.53±0.03)相比差异有统计学意义(P＜0.05);Ⅲ级和Ⅰ级动脉硬化斑块ABCA1mRNA、RXRα mRNA及蛋白表达水平差异有统计学意义(P＜0.05).结论 ABCA1及RXRα蛋白表达水平下调可能是进展性动脉粥样硬化损害的关键因素.     

肠系膜缺血性疾病的临床诊断

早期诊断和及时治疗，对降低肠系膜缺血性疾病的并发症发生率和病死率至关重要，但由于肠系膜缺血性疾病缺乏特异的临床表现，鉴别诊断也比较困难。因此，基于临床表现的早期诊断一直是非常困难的课题。     

Observations on the efficacy of combined acupuncture and medicine in Treating 50 cases of pseudobulbar paralysis

目的：观察针药结合治疗假性延髓麻痹的疗效。方法：采用完全随机抽样法分为针刺组50例，对照组48例。针刺组在西医治疗的同时，应用针刺进行综合治疗；对照组单纯接受西医药物治疗。结果：针刺组有效率为92．0％，对照组为62．5％。两组疗效差异具有非常显著性意义（P〈0．01）。结论：针药结合治疗假性延髓麻痹效果明显优于单纯西药治疗。     

The modified outside-in suture: vertical repair of the anterior horn of the meniscus after decompression of a large meniscal cyst

This article describes a modified suture technique designed for the vertical repair of the anterior horn of the meniscus after arthroscopic decompression of a large meniscal cyst. This procedure comprises of three steps: first, the meniscus was pierced vertically using a suture hook and a No. 0 PDS suture. Second, both ends of the No. 0 PDS on the femoral and tibial surfaces of the meniscus were pulled to the outside of the joint capsule using a spinal needle preloaded with suture material. Finally, a skin incision was made adjacent to the suture materials, and both ends were tied. We recommend this technique not only for the vertical repair of the anterior horn of the meniscus after decompression of large meniscal cyst, but also to repair a longitudinal tear of the meniscus.     

负压进针在心血管内科病人中的应用

[目的]探讨负压进针在心血管内科病人的静脉输注中是否优于常规进针法.[方法]将2004年1月-2005年6月我科收治且静脉穿刺难度大的病人随机分为实验组和对照组,实验组150例应用负压进针法,对照组150例应用常规进针法,比较两组的穿刺成功率和回血率.[结果]负压进针法比常规进针法的穿刺成功率提高了12%,回血率提高了13%,两组间穿刺成功率和回血率的差异有统计学意义(P＜0.01).[结论]负压进针大大提高了穿刺成功率和回血率,减少了病人的痛苦,提高护理人员的工作效率和住院病人满意度.     

初中生信件咨询常见问题分析

目的通过分析初中生心理发展的特点，找出信件咨询常见问题的原因，提出问题解决的对策。方法通过对初中生心理咨询信件的整理分析。结果信件咨询作为学校心理咨询的一种非主流形式，由于其自身的种种缺陷并没有受到过多的重视。介从初中生的来询信件中确实反映了一些问题。结论对初中生进行心理健康教育有十分重要的意义。     

静脉用免疫球蛋白的治疗作用机制及其不良反应

静脉滴注免疫球蛋白用于多种自身免疫性疾病和炎症性疾病的治疗,其作用机制包括对自身抗体的作用,抑制补体结合,阻止膜攻击复合物形成,调节巨噬细胞Fc受体,抑制致病性细胞因子和其他免疫调节分子等。该药不反应反应较少,主要有皮肤过敏反应、无菌性脑膜炎、血浆黏度增加和血栓形成以及肾功能损害等。     

天文潮汐对心脏病患者心力衰竭发生率影响的研究

目的　研究天文潮汐对心脏病患者发生心衰的影响。方法　分析 118例心脏病患者发生心衰在各潮汐期的分布。结果　在强天文潮汐期内心脏病患者发生心衰显著多于其余时期 ,而且在朔、望期发生的人数多于上、下弦期。结论　护理人员要了解天文潮汐的变化规律 ,教育心脏病患者在强天文潮汐期内要保持情绪稳定 ,必要时在强天文潮汐期内可给予心脏病患者适量的镇静剂和安眠药。
