Management of potentially malignant disorders: evidence and critique

At a workshop coordinated by the WHO Collaborating Centre of Oral Cancer and Precancer in the United Kingdom, issues related to management of patients affected by oral leukoplakia were discussed by an expert group. The consensus views of the working group are presented here. Although removal of a lesion still seems to be the predominant method of treatment by the majority of relevant health care professionals, no randomized controlled trials have been undertaken to test the hypothesis that excision either by scalpel or laser greatly influences the potential for later malignant transformation within the oral mucosa of an affected individual. Results of observational studies indicate that, although surgery may have a beneficial effect, this is not likely to reduce the risk of later recurrence nor malignant transformation at the same or another site. Medical measures that lessen the size, extent or histopathological features of dysplasia within leukoplakia likewise presently do not seem to be of particular promise, as relapse or later malignant transformation can occur, and there is a risk of adverse effects, particularly with systemic agents (which themselves may be contra-indicated in some individuals). While the risk of malignant transformation, and the development of further potentially malignant disease may theoretically be reduced by cessation of risk activities, such as tobacco usage and alcohol consumption, there remain no good studies that demonstrate that such measures significantly reduce such events.     

Effect of mouthrinsing after toothbrushing with a fluoride dentifrice on human salivary fluoride levels

In a recent clinical trial of sodium monofluorophosphate dentifrices, oral rinsing habits were found to influence dental caries. Thus an oral fluoride clearance study has been undertaken which was designed to test a possible mechanism for the observed effects. Eight subjects brushed with one of the trial dentifrices and then rinsed using 1 of 8 procedures of varying thoroughness. The salivary fluoride concentration measured 5 min after dentifrice application decreased significantly with increasing rinse volume, rinse duration, and rinse frequency (p less than 0.01, analysis of variance). The area under the clearance curve determined over a further 3 h was significantly higher (300%; p less than 0.01) following use of the least thorough rinsing procedure (5 ml x 2 s once) as compared with the corresponding area under the clearance curve following the most thorough procedure (20 ml x 10 s twice). These findings indicate that rinsing habits may play an important role in the oral retention of fluoride from dentifrices which may, in turn, affect their clinical efficacy.     

Overview of the clinical evidence for the use of NovaMin in providing relief from the pain of dentin hypersensitivity

Dentin hypersensitivity is a common condition that affects the adult population worldwide. NovaMin is technically described as amorphous sodium calcium phosphosilicate, and has been shown in laboratory studies to rapidly occlude dentin tubules and form a protective hydroxyapatite-like layer on the dentin surface. A number of clinical studies investigating the efficacy of NovaMin for the relief of pain from dentin hypersensitivity have been conducted in the United States, Ireland, China, and India. This article reviews the available literature and finds support based on randomized controlled clinical trials for the use of NovaMin in anhydrous toothpaste formulations in providing relief of pain from dentin hypersensitivity.     

Management of hemangiomas and pediatric vascular malformations

Pediatric vascular anomalies can be difficult to diagnose and complex to treat. One must be able to distinguish hemangiomas from various vascular malformations, as well as appreciate their dynamic course with time. Thorough understandings of the clinical and diagnostic techniques used to evaluate these lesions are paramount for the treating surgeon. In addition, knowledge of current treatments from watchful waiting to radical extirpation is mandatory. This must all be done in the setting of a developing child. We present a current review of the literature regarding the comprehensive care of pediatric vascular lesions.
