Inflammation and sleep in healthy individuals   总被引:3,自引:0,他引:3  
STUDY OBJECTIVES: Inflammation is relatively common in individuals with a sleep disorder and is associated with quality of sleep. The purpose of this study was to examine whether inflammation is associated with quality of sleep in healthy individuals. DESIGN & SETTING: Observational study in a General Clinical Research Center. PARTICIPANTS: This study characterized inflammation and polysomno-graphically verified sleep in 124 African American and Caucasian American women and men without a sleep disorder. MEASUREMENTS AND RESULTS: Circulating levels of 3 markers and/or mediators of inflammation known to be elevated in sleep disorders and in cardiovascular disease were determined (interleukin-6 [IL-6] endothelin-1 [ET-1], soluble intercellular adhesion molecule-1 [sICAM-1]). Sleep was characterized by polysomnography. Multiple linear regression analyses showed that increasing age, male sex, and African American ethnicity were independently associated with poorer sleep. After controlling for these variables, higher levels of ET-1 were independently associated with greater sleep latency (P < or = 0.01), greater rapid eye movement (REM) latency (P < or = 0.01), more slow wave sleep (P < or = 0.05), and less stage 1 sleep (P < or = 0.01). Higher IL-6 levels were independently associated with greater REM latency (P < or = 0.05). CONCLUSIONS: The findings suggest that, in individuals without a known sleep disorder, ET-1, a potent vasoconstrictor and mediator of inflammation, is associated with more deep sleep, whereas both ET-1 and IL-6 are associated with increased latency of sleep and of REM. The findings underscore the complex relationships between peripheral markers of inflammation and sleep.  相似文献

Purpose:The aim of this study is to study the association between Nocturnal Intraocular Pressure (IOP) related Peak recorded by a Contact Lens Sensor (CLS) and glaucoma progression in treated glaucomatous eyes.Methods:Institutional study in which forty glaucoma patients were recruited from glaucoma clinic. A total of 19 patients were labeled as progressors on current anti-glaucoma treatment despite controlled day time IOP whereas twenty one patients were clinically stable showing no progression. Worse eye of each patient was selected for placement of CLS. The timing of the highest signal (IOP related peak) was noted in 24 hour CLS graph and if it fell within the time frame of 11 pm to 5 am, it was labeled as ''nocturnal IOP related peak''.Results:Progressors were found to be significantly more prone to night spike than Non Progressors (χ2 = 6.812; n = 40; P = 0.009), thus, showing a definite association between the two. Association between Nocturnal IOP related peak and various other variables like age, gender, mean daytime IOP and systemic illness was studied. A positive correlation was established between female gender and Nocturnal IOP related spike with a significantly higher proportion of females showing night spike than their male counterparts (χ2 = 5.763; n = 40; P = 0.016). Other parameters did not show any significant relationship with Nocturnal IOP related spike.Conclusion:Dynamic 24 hour recording by CLS is beneficial in detecting nocturnal IOP-related peak, and thus, can potentially improve the clinical care of glaucoma patients, especially those showing progression.  相似文献

Abetalipoproteinemia is an uncommon cause of ataxia and retinitis pigmentosa (RP). Most of the neurological and ocular manifestations occur secondary to deficiency syndromes that is consequent to fat malabsorption from the small intestine. In this report, we have described the phenotype of a young adult female who manifested with recurrent diarrheal illness in her first decade, followed by anemia, RP, and neurological involvement with progressive deafness, cerebellar and sensory ataxia, and subclinical neuropathy in her second decade of life. While RP and sensory ataxia due to vitamin E deficiency are well-recognized features of abetalipoproteinemia, deafness is rarely described. In addition, we have highlighted the abnormal posterior column signal changes in the cervical cord in this patient. Early recognition avoids unnecessary investigations and has a potential to retard the disease progression by replacing some of the deficient vitamins.  相似文献

PURPOSE

The Canadian Primary Care Sentinel Surveillance Network (CPCSSN) is Canada’s first national chronic disease surveillance system based on electronic health record (EHR) data. The purpose of this study was to develop and validate case definitions and case-finding algorithms used to identify 8 common chronic conditions in primary care: chronic obstructive pulmonary disease (COPD), dementia, depression, diabetes, hypertension, osteoarthritis, parkinsonism, and epilepsy.

METHODS

Using a cross-sectional data validation study design, regional and local CPCSSN networks from British Columbia, Alberta (2), Ontario, Nova Scotia, and Newfoundland participated in validating EHR case-finding algorithms. A random sample of EHR charts were reviewed, oversampling for patients older than 60 years and for those with epilepsy or parkinsonism. Charts were reviewed by trained research assistants and residents who were blinded to the algorithmic diagnosis. Sensitivity, specificity, and positive and negative predictive values (PPVs, NPVs) were calculated.

RESULTS

We obtained data from 1,920 charts from 4 different EHR systems (Wolf, Med Access, Nightingale, and PS Suite). For the total sample, sensitivity ranged from 78% (osteoarthritis) to more than 95% (diabetes, epilepsy, and parkinsonism); specificity was greater than 94% for all diseases; PPV ranged from 72% (dementia) to 93% (hypertension); NPV ranged from 86% (hypertension) to greater than 99% (diabetes, dementia, epilepsy, and parkinsonism).

CONCLUSIONS

The CPCSSN diagnostic algorithms showed excellent sensitivity and specificity for hypertension, diabetes, epilepsy, and parkinsonism and acceptable values for the other conditions. CPCSSN data are appropriate for use in public health surveillance, primary care, and health services research, as well as to inform policy for these diseases.  相似文献
