In vitro effects of detergent sclerosants on coagulation, platelets and microparticles.

OBJECTIVES: To investigate the in vitro effects of Sodium Tetradecyl Sulphate (STS) and Polidocanol (POL) on clotting tests, clotting factors, platelets and microparticles. MATERIALS AND METHODS: Platelet rich (PRP) and platelet poor (PPP) plasmas were incubated with varying concentrations of STS and POL. Clotting tests, platelet/plasma turbidity, and microparticle studies were performed. Specimens were mixed with individual factor deficient plasmas and clotting factor activities were studied. RESULTS: STS at high concentrations (>0.3%) destroyed platelets, microparticles and the clotting factors V, VII and X. It prolonged all clotting tests including prothrombin time (PT), activated partial thromboplastin time (APTT), non-activated partial thromboplastin time (NAPTT), thrombin time (TT), factor Xa clotting time (XACT) and surface activated clotting time (SACT). Higher concentrations of POL were required to achieve some anticoagulant activity. Low sclerosant concentrations shortened XACT and SACT, and induced release of procoagulant platelet derived microparticles. Increased exposure time resulted in increased procoagulant activity. STS at concentrations higher than 0.5% precipitated a complex containing apolipoprotein b and fibrinogen. CONCLUSIONS: Detergent sclerosants affect the clotting mechanism by interfering with clotting factor activities, procoagulant phospholipids and platelet derived microparticles. STS has more anticoagulant activity than POL in high concentrations. Low concentration sclerosants demonstrate procoagulant activity.

后腹腔镜治疗肾上腺肿瘤35例临床分析

目的总结后腹腔镜技术治疗肾上腺肿瘤的临床经验.方法采用后腹腔镜切除肾上腺肿瘤35例,男12例,女23例.年龄25岁～72岁,平均49.8岁.左侧15例,右侧20例.其中嗜铬细胞瘤5例,原发性醛固酮增多症10,柯兴氏病2例,神经节细胞瘤2例,无功能性腺瘤16例.腰部取3个套管针穿刺入路,用自制的水囊扩张器扩张后腹腔,用超声刀将肿瘤切除.结果 35例手术均成功,无中转开放手术病例.术中出血量10～500 ml,平均74 ml.病人均未输血.手术时间45～200 min,平均90 min.结论后腹腔镜手术切除肾上腺肿瘤手术效果确切,手术时间短,术中出血少,损伤小,术后恢复快,可以替代绝大多数肾上腺肿瘤开放手术.     

瘢痕成纤维细胞中TGF-β受体与整合素相互作用的实验研究

目的:了解TGF-β受体和整合素在瘢痕增生和挛缩过程中的作用.方法:通过荧光定量PCR法(FQ-PCR)测定经TGF-β受体、整合素和粘着斑激酶(FAK)抗体阻断后培养的瘢痕成纤维细胞TGF-β受体以及整合素表达量的变化.结果:经不同抗体阻断后培养的瘢痕成纤维细胞其TGF-β RI和整合素β 1的基因拷贝数均较阴性对照组有不同程度的下降(P＜0.05).结论:TGF-β受体和整合素介导的信号传导途径间可能存在着正反馈的效应,共同促进瘢痕的增生和挛缩.FAK是两条信号传导途径的交汇点和作用的中心环节.     

Closure of left ventricle perforation with the use of muscular VSD occluder.

Growing experience in interventional cardiology leads to the use of large diameter of vascular equipment. In some instances, the so-called hybrid procedures are performed. After performing the interventional procedure, the opening in ventricular wall is closed surgically. Our intention was to check if the MVSDO can be used to close the perforation in the heart after the interventional cardiology procedure performed through the left ventricular (LV) free wall. In three pigs under general anesthesia, the heart was exposed through a small substernal incision. The LV was punctured and an 18F sheath was introduced into the LV. A 14 mm MVSDO was inserted through the 10F Delivery System. Using both the echocardiographic and angiographic guidance, the MVSDO was placed on the LV wall to close the opening in the LV. Time and volume of bleeding was recorded. In all cases the occluder was successfully placed closing the opening, bleeding observed after deployment of occluder lasted for approximately 2 min. We think MVSD occluder can be used to close the LV free wall perforation after hybrid interventional cardiac procedure. Early bleeding through MVSDO might be resolved by the manufacturing of new occluder with better sealing properties.     

上海市某区警察心理健康状况和生活质量的时点调查

目的调查警察心理健康状况和生命质量的基本流行学资料。方法采用梅奥心理服务问卷,主要内容为心理健康状况和生命质量测定等。在上海市某公安分局内进行截断面时点调查。结果1共调查警员1611人,男性91.5%,女性8.5%,年龄28.8±17.5岁。2GHQ的平均得分:1.5±2.31,存在可能心理障碍倾向者比例15.3%,可能存在重度心理障碍者比例2.9%。3SCL-90总分大于160分者比例17.2%;单项分大于2分比例最高的是强迫症状因子(19.5%),睡眠饮食因子次之(12.8%),躯体化症状因子(12.6%)第3位,敌对因子(12.4%)处于第4位。4健康变化、总体健康感、心理健康和生命活力得分是相对最低的维度。结论1在警察群体中存在心理障碍者比例远高于一般市民,生活质量一些维度相对较差;2建立长效机制是维护和保证警察心理健康和生活质量的有效手段。     

HPLC法测定盐酸美沙酮含量

本文采用高效液相色谱法，以盐酸布比卡因为内标，检测波长220nm，在Symmetry-C18柱上，以己腈-磷酸二氢钾（pH2．5）（3862，V／V）为流动相，测定盐酸美沙酮及其口服液的含量。方法平均回收率为100．17％，日内及日间的RSD均＜1％。方法简便、快速、准确。
