Intracardiac transplantation of a mixed population of bone marrow cells improves both regional systolic contractility and diastolic relaxation.

BACKGROUND: Pre-clinical and clinical studies suggest that transplantation of bone marrow-derived stem cells can improve global cardiac function. However, no quantitative assessment of regional systolic contraction and correlation with phenotype has been made. Therefore, we used our model of cryoinfarcted rabbit myocardium for intracardiac transplantation of a mixed population of bone marrow-derived cells and assessed both regional function and myogenic conversion of the cells. METHODS: Nineteen New Zealand white rabbits underwent cryoinjury of the left ventricle. Autologous bone marrow (BM) cells were expanded in vitro. After 2 weeks, either 1 x 10(8) mixed BM-derived progenitor cells (BM group, n = 11) or vehicle (control group, n = 8) were injected into the cryoinjured region. Regional systolic function was measured using micromanometry and sonomicrometry before and 4 weeks after cell injection; cell phenotype was evaluated histologically. RESULTS: All animals in the BM group significantly improved both systolic shortening (0.11 +/- 0.7 vs -0.05 +/- 0.05 mm in the control group, p < 0.05) and regional stroke work when compared with control (9.6 +/- 2.4 vs -1.2 +/- 1.2 mm . mm Hg, p < 0.003). In addition, the BM group had improved global diastolic function, as measured by minimum dP/dt and end-diastolic pressure. On histologic assessment, BM cells differentiated toward a myogenic phenotype. CONCLUSIONS: Transplanting a mixed population of marrow-derived cells that can adopt a myogenic phenotype improves regional contractility and diastolic relaxation after myocardial infarction.

Pain in or symptomatic destruction of any number of the joints of the hindfoot can have several causes. As well as primary osteoarthritis, these include secondary arthrotic changes following fractures (especially of the talus and/or calcaneus), muscular and/or tenoligamentous instability (e.g., rupture of the posterior tibialis tendon, poliomyelitis, Charcot-Marie-Tooth disease), spastic and flaccid paresis of the lower leg and foot, late stages of various inflammatory arthritides (rheumatoid arthritis, seronegative spondarthritis, collagenoses), and neuropathic diseases with impaired depth perception (Charcot disease of the foot in diabetes mellitis, for example). As a general policy, only joints that are causing symptoms, are clearly arthritic, or are involved in a fixed foot deformity should be fused, so that as much of the residual hindfoot motion as possible is preserved. This helps to restrict future osteoarthritis in adjacent joints. The use of screws rather than staples seems to increase the fusion rates because this technique allows compression on the fusion.     

刺鼠信号蛋白对自体移植皮片中酪氨酸酶的活性影响

目的 检测刺鼠信号蛋白对自体移植皮片中酪氨酸酶的活性影响,进一步认识自体移植皮片过度色素沉着的原因。方法 建立过度色素沉着的自体移植皮片的动物模型；利用免疫组织化学方法分别检测刺鼠信号蛋白作用前后自体移植皮片中酪氨酸酶的表达,并与对照治疗组及正常皮肤相比较。结果 酪氨酸酶的表达定位于表皮基底部黑色素细胞的胞浆,在大部分组织中呈阳性表达,经刺鼠信号蛋白作用后酪氨酸酶在自体移植皮片中的表达明显减少,与在其他各对照组中的表达差异有极显著意义（P＜0.01）。结论 刺鼠信号蛋白在自体移植皮片中能竞争性拮抗α-MSH的黑色素合成,使皮片着色能力降低,从而证明皮片移植后表皮细胞中α-MSH的表达上调是皮片呈过度色素沉着的重要原因。     

Pretransplant bilateral hand-assisted laparoscopic nephrectomy in adult patients with polycystic kidney disease.

Laparoscopic procedures continue to gain popularity over traditional open procedures for a number of abdominal and pelvic surgeries. With increasing experience, the application of this technique is rising because it provides an alternative, less invasive, approach to various surgical procedures. Herein, we report our experience with adult patients with polycystic kidney disease, requiring bilateral laparoscopic nephrectomy before renal transplantation.     

中西医结合治疗小儿急性肾小球肾炎临床观察

急性肾小球肾炎为临床常见病，多发于少年儿童，男性多见。西医除对症处理外，无特效药物，中药治疗本病可明显缩短病程。笔者自2003年1月至2006年1月用清热利湿解毒汤治疗急性肾小球肾炎60例取得确切疗效，现报道如下。     

腹腔镜肝切除术患者的护理

笔者报道腹腔镜肝切除术患者的护理。术前进行心理护理和呼吸训练；术后注意早期活动，加强腹腔镜CO2气腹后的观察、护理，做好皮下气肿、胆漏、出血、高碳酸血症和下肢静脉血栓等术后并发症的观察及护理。认为周到细致的护理是手术成功的重要保证。
