Paraesophageal hernia in children is relatively rare entity. These children usually present with recurrent respiratory tract infection or vague Gl symptoms. An 11 year female presented with episodic vomiting, heartburn and features of hypocalcemic tetany. Investigations revealed right paraesophageal hernia. Reduction of the contents and repair of the defect with fundoplication was done. Patient is asymptomatic on follow-up.  相似文献

Of the various manifestations of ocular chemical burns (OCBs), ischemia of the limbus and the peri-limbal sclera indicates poor prognosis and in severe cases threaten the integrity of the globe. Tenonplasty is a surgical procedure which involves advancing the Tenon’s capsule over the ischemic areas to provide a vascular supply and to enable migration of the conjunctival epithelium. This review aims to provide an overview of the diagnosis of limbal ischemia and its management with Tenonplasty. A literature review was conducted using the keywords “Tenonplasty,” “Tenon’s capsule,” “ocular chemical injury,” “ocular thermal injury,” “Tenon advancement,” “scleral ischemia,” and “limbal ischemia,” and outcomes were studied from seven selected articles. In addition to clinical evaluation, in vivo imaging techniques such as anterior segment optical coherence tomography angiography can provide an objective method of measuring and monitoring the ischemia and re-perfusion of the peri-limbal vasculature. Tenonplasty can be performed in eyes with acute OCBs with scleral or limbal ischemia by dissecting the Tenon’s layer from the orbit and securing it to the limbus. The indications, mechanism of action, peri-operative considerations, surgical technique, and post-operative care of Tenonplasty are discussed in detail. The average time for post-operative re-epithelization ranges from 1 to 6 months with the formation of a symblepharon being the most common complication. In conclusion, Tenonplasty is a globe-salvaging procedure in cases with severe limbal and scleral ischemia because of OCBs and has good anatomical outcomes priming the globe for subsequent re-constructive and vision-restoring surgeries.  相似文献

Immunoreactive inhibin-like material (ILM) was measured by radioimmunoassay (RIA) in serum and gastric juice samples of 23 fasting normal men, 27 men with chronic superficial gastritis (CSG), and 21 men with carcinoma of stomach (5 for gastric analysis). Serum ILM levels in carcinoma of stomach patients (367 +/- 55.5 ng ml-1) were significantly higher than in normal men (15.4 +/- 2.6 ng ml-1; P less than 0.01) and in patients with CSG (109.8 +/- 17.7 ng ml-1; P less than 0.05). Sixty two per cent and 86% of patients with carcinoma of stomach showed elevated ILM levels which were higher than the highest noted in patients with CSG and normal men respectively.  相似文献

Histomorphological changes and steroidogenic potential of the gonads during sexual differentiation and development were studied in Rana curtipes from tadpole stage 25 (Gosner) until maturity. In stage 25 tadpoles of smaller snout-vent length (SVL; 4-5 mm) the gonads were indifferent, containing a few somatic and germ cells, whereas in larger tadpoles (SVL > 7 mm) gonads were differentiated into ovaries with a central lumen. Onset of meiosis was seen in these ovaries. At stage 26, diplotene and first growth phase oocytes were found. With advancement in developmental stage and after metamorphosis the ovaries progressively enlarged due to increase in number and size of oocytes. Vitellogenesis began in the ovary of 4-month-old frogs. Females attained maturity 6 months after metamorphosis. The frogs showed amplexus and one frog spawned. Onset of testicular formation seen at stage 31 was associated with the degeneration of oocytes and infiltration of darkly stained somatic cells in the central region. By stage 35 all oocytes degenerated, leaving behind a large number of somatic and germ cells interspersed with each other. At stage 38, formation of seminiferous tubules enclosing spermatogonia and pre-Sertoli cells was seen. Initiation of meiosis was evident at metamorphic climax. Cysts of elongated spermatids associated with Sertoli cells were seen in 45-day-old frogs. Histochemically, delta(5)-3 beta-hydroxysteroid dehydrogenase activity was localized in the ooplasm, follicular cells, and interstitium of the ovary from stage 28 onward. The enzyme activity in the testis appeared in 45-day-old froglets. In R. curtipes gonadal differentiation is a semidifferentiated type since gonads initially differentiate into ovaries, and later, in the prospective males, the ovaries degenerate and transform into testes. The males attain maturity much earlier than the females. In R. curtipes gonadal sex differentiation precedes the onset of gonadal steroidogenesis.  相似文献

Objectives:

To study the antiovulatory and abortifacient effects of ethanolic extract of Areca catechu in female rats.

Materials and Methods:

For antiovulatory effect, ethanolic extract of A. catechu at 100 and 300 mg/kg doses was administered orally for 15 days. Vaginal smears were examined daily microscopically for estrus cycle. Rats were sacrificed on 16th day. Ovarian weight, cholesterol estimation, and histopathological studies were done. Abortifacient activity was studied in rats at 100 and 300 mg/kg doses administered orally from 6th to 15th day of pregnancy. Rats were laparotomised on 19th day. The number of implantation sites and live fetuses were observed in both horns of the uterus.

Results:

The extract of A. catechu showed a significant decrease in the duration of estrus at 100 mg/kg (P = 0.015) and 300 mg/kg doses (P = 0.002) as compared with control. Metestrus phase was also significantly reduced at 100 mg/kg (P = 0.024) and 300 mg/kg doses (P = 0.002). There was a significant increase in proestrus (P < 0.001) phase. However, diestrus phase was unchanged. Histopathological study of the ovaries showed mainly primordial, primary, and secondary follicles in the test groups as compared to control. There was also a significant (P = 0.002) decrease in ovarian weight and a significant (P = 0.021) increase in ovarian cholesterol level at 100 mg/kg dose. In the study to evaluate abortifacient effect, the mean percentage of abortion with 100 and 300 mg/kg doses were 75.5% and 72.22%, respectively, which was significantly (P = 0.008 and P = 0.006, respectively) increased when compared with control.

Conclusion:

The ethanolic extract of A. catechu at doses of 100 and 300 mg/kg has antiovulatory and abortifacient effects.  相似文献
