目的 探讨双因素理论在血液净化中心护士夜班管理中的作用。方法 调查血液净化中心37位护理人员的职称、学历、年龄、工龄、工作能力及科研水平等状况,综合考虑后制定新的管理制度应用于护士夜班管理中,观察新制度实施前后成效。结果 新的夜班管理制度实施以来,全区护士的满意度、学习热情、继续教育、竞争意识、服务意识、工作态度、管理水平及科研水平等较前差异有统计学意义(P〈0.05)。结论 科学、合理的夜班护理管理制度,并结全适当的激励制度有利于实现护理管理目标。 相似文献

目的:评价基于中医"治未病"理论的健康管理在非酒精性脂肪肝中的应用效果。方法:选取2018年5月~2019年4月收治于我院的258例非酒精性脂肪肝患者作为研究对象,采用随机数字表法将258例患者随机分为观察组(129例)和对照组(129例)。对照组采用常规治疗方案,观察组在对照组基础上采取基于中医"治未病"理论的健康管理,比较两组治疗6个月后的总有效率、治疗前后单项症状积分、个体参数(体质量指数、腰臀比、收缩压、舒张压)、血脂指标(甘油三酯、总胆固醇),以及肝功能指标(谷丙转氨酶、谷草转氨酶)。结果:经治疗后,观察组患者的总有效率明显高于对照组,差异有统计学意义(P<0.05);且观察组单项症状积分均明显低于对照组(P<0.05),个体参数均低于对照组(P<0.05),血脂和肝功能指标水平均低于对照组(P<0.05)。结论:基于中医"治未病"理论的健康管理在脂肪肝中的应用可提高临床治疗效果,降低患者血脂,改善患者临床症状和肝功能,具有较好的应用效果。 相似文献

目的 评价直接置入雷帕霉素药物洗脱支架(CYPHER^TM,codis治疗急性冠脉综合征(ACS)罪犯病变的安全性、可行性。方法 32例接受CYPHERTM支架直接置入的患者(直接支架组)和一般情况匹配的34例球囊扩张后行冠脉支架术的患者(常规支架组),比较两组的一般情况,冠脉造影及介入治疗即刻和临床随访结果。结果 直接支架组和常规支架组的成功率分别为97%和100%,直接支架组1例支架无法通过病变,球囊预扩张后成功置入原支架。两组无一例发生介入治疗相关的的严重心脏事件。随访期间两组严重心脏不良事件发生率无明显统计学差异。结论 CYPHER^TM支架直接置入术治疗ACS罪犯病变安全可行,成功率高。 相似文献

Obesity, the metabolic syndrome, and type 2 diabetes mellitus (T2DM) are major global health problems. Insulin resistance is frequently present in these disorders, but the causes and effects of such resistance are unknown. Here, we generated mice with muscle-specific knockout of the major murine atypical PKC (aPKC), PKC-lambda, a postulated mediator for insulin-stimulated glucose transport. Glucose transport and translocation of glucose transporter 4 (GLUT4) to the plasma membrane were diminished in muscles of both homozygous and heterozygous PKC-lambda knockout mice and were accompanied by systemic insulin resistance; impaired glucose tolerance or diabetes; islet beta cell hyperplasia; abdominal adiposity; hepatosteatosis; elevated serum triglycerides, FFAs, and LDL-cholesterol; and diminished HDL-cholesterol. In contrast to the defective activation of muscle aPKC, insulin signaling and actions were intact in muscle, liver, and adipocytes. These findings demonstrate the importance of aPKC in insulin-stimulated glucose transport in muscles of intact mice and show that insulin resistance and resultant hyperinsulinemia owing to a specific defect in muscle aPKC is sufficient to induce abdominal obesity and other lipid abnormalities of the metabolic syndrome and T2DM. These findings are particularly relevant because humans who have obesity, impaired glucose tolerance, and T2DM reportedly have defective activation and/or diminished levels of muscle aPKC. 相似文献

OBJECTIVE: The purpose of this study was to evaluate micro flow imaging (MFI) in depicting the vascular architecture of hepatocellular carcinoma (HCC) and the correlation between pathologic differentiation and the intratumoral vascular architecture pattern. METHODS: Micro flow imaging and contrast harmonic imaging (CHI) were performed in 37 patients with HCC. A sulfur hexafluoride-filled microbubble contrast agent was used. The enhancement level and intratumoral vessels were evaluated on CHI. The vascular architecture of each tumor was evaluated on MFI. Pathologic differentiation of the tumors was classified according to the Edmondson grading system. RESULTS: All 37 HCCs showed hyperenhancement in the arterial phase and hypoenhancement in the portal and late phases on CHI. Intratumoral vessels in the arterial phase were visualized in 20 (54.1%) HCCs. On MFI, the vascular architecture in all lesions was clearly delineated and categorized into 3 patterns: cotton, shrubbery, and deadwood, identified in 12 (32.4%), 22 (59.5%), and 3 (8.1%) of the tumors evaluated, respectively. A chi(2) test showed that pathologic differentiation significantly correlated to the vascular pattern (P = .006). Three (75%) of 4 Edmondson grade I HCCs showed the cotton pattern; 18 (75.0%) of 24 Edmondson grade II HCCs showed the shrubbery pattern; and the deadwood pattern was shown only in Edmondson grade III and IV HCCs. CONCLUSIONS: The MFI technique is more effective in depicting the intratumoral vascular architecture. The vascular architecture pattern correlates with pathologic differentiation of HCC. 相似文献

Cerebral microbleeds (CMBs) are known to be associated with cognitive impairments in the elderly and in patients with various diseases; however, the nature of this association has not yet been evaluated in Parkinson’s disease (PD). In the present study, we analyzed the incidence of CMBs in PD according to cognitive status, and the impact of CMBs on cognitive performance was also evaluated. The CMBs in PD with dementia (n = 36), mild cognitive impairment (MCI, n = 46), or cognitively normal (n = 41) were analyzed using conventional T2*-weighted gradient-recalled echo images. Additionally, the relationship between the presence of CMBs and cognitive performance on individual tests of cognitive subdomains was analyzed using a detailed neuropsychological test. CMBs occurred more frequently in PD patients with dementia (36.1 %) compared to those with MCI (15.2 %), those who are cognitively normal (14.6 %), and normal controls (12.2 %, p = 0.025). However, the significant association of CMBs with PD dementia disappeared after adjusting white matter hyperintensities (WMHs) as a covariate. The frequencies of deep, lobar, and infratentorial CMBs did not differ among the four groups. After adjusting for age, sex, years of education, and WMHs, PD patients with CMBs had poorer performance in attention domain compared with those without CMBs (34.9 vs 42.6, p = 0.018). The present data demonstrate that even though CMBs were inseparably associated with the presence of WMHs, CMBs occur more commonly in PD patients with dementia than in those without dementia. Nesfatin-1, a newly discovered satiety molecule which reduces feeding behavior, has been recognized as a unique regulatory neuropeptide with its multiple roles, both central and peripheral. However, whether it had neuronal modulation effect on dopaminergic neurons is largely unknown. In the present study, using whole-cell patch clamp under current-clamp mode, we investigate the effects of nesfatin-1 on the electrical activity of rat nigral dopaminergic neurons. Nesfatin-1 could produce a resting membrane potential hyperpolarization on the majority of dopaminergic neurons tested. The spike frequency decreased by 23.13?±?5.93 and 43.20?±?5.56 % in 5-nM and 10-nM nesfatin-1 groups, respectively. These effects persisted in the presence of ionotropic glutamate and GABA receptor antagonists. Our study suggests that nesfatin-1 postsynaptically inhibits the electrical activity of nigral dopaminergic neurons. 相似文献
