RESPIRATORY FUNCTIONAL CHANGES AFTER CALTDAL BLOCK IN PEDIATRIC SURGERY

Alterations in blood-gas parameters before and after caudal block at different levels in 30 children undergoing operation were studied. No significant changes were noted after low caudal block. In high caudal block, hypoxemia may occur but can be sue- cessfully corrected by oxygen inhalation. It is sug- gested that routine use of oxygen inhalation is im- perative in caudal anesthesia and that high level block should be avoided in children with poor ventilation.     

RESEARCH ON LATE STAGE SCHISTOSOMIASIS JAPONICA IMMUNOLOGY

The immune rea.ction of late stage schistoso- miasis patients is studied. Some nonspecific cell-mediated immune reaction tests such as SK-SD skin test and rosette formation test in- dicate that. the cell-media.ted immune reaction in these patients is evidently lower than in nor- mal persons. IgG and IgM are markedly in- creased in humoral immune function. In some of the patients auto-antibody and anti-comple- ment are positive and complement 3 (CJ) con- tent decreased, suggesting that the mechanism of late stage schistosomiasis is not only asso- ciated with the delayed type hypersensitivity rea.ction of the fourth type, but also with the antigen and antibody complex diseases of the third type. In this case, it is imperative to adjust the organi.sm immune function. According to this viewpoint the authors succeeded in treating late stage schistosomiasis with traditional Chine;se medicine and specific transfer factor for adopt- ed immunity.     

APPENDECTOMY AND CANCER AN EPIDEMIOLOGICAL EVALUATION

1,119 inpatients (cancer group) suffering from :gastric cancer (507), cancer of the colon and rectum (297), breast cancer (315), and 400 patients with cerebral hemorrhage (control group) treated from January 1958 to June 1983 were studied epidemio- logically for their previous appendectomy rates. ACUTE TRAUMATIC DIAPHRAGMATIC HERNIA A REPORT OF 8 CASES

ight cases of acute traumatic diaphragmatic hernia (ATDH) were caused by penetrating injury in 2 and blunt trauma in 6. Among the latter group, 4 sustained pelvic fractures, 3 fractures of the lower extrernities and only 2 had multiple rib fractures. This denotes that crush injury to the lower abdomen may lead to an abrupt increase of intraabdominal pressure and cause ATDH. We suggest that in patients with chest symptoms after extensive crush trauma below the umbilicus, ATDH should be highly suspected and that routine plain chest X-ray is in dicated for ATDH diagnosis.     

ECTOPIC PREGNANCY TREATED WITH TRICHOSANTHIN CLINICAL ANALYSIS OF 71 PATIENTS

Pathological studies on the aborted materials after the administration of trichosanthin at the end of mid-term gestation have revealed that there is an extensive coagulative necrosis of the trophoblastic tissue of the placental villi. In 1971, we first used trichosanthin successfully to treat ectopic pregnancy instead of salpingectomy. Trichosanthin was given to the patients with ectopic pregnancy who were clinically stable and had no signs of acute distress or large quantity of intraperitoneal hemorrhage. From 1971 to 1986, a total of 71 patients were treated with trichosanthin with a success rate of 85.9% and a subsequent fertility rate of 82.6%.     

JNVESTIGATIONS OF SOME HEREDITARY EYE DISEASES CAUSING BLINDNESS

Among 121 students at the T'ianjin School for the Blind, 82 (67.8To) had congenital eye di- diseases and 39 (32.2'/o) had postnatal eye di- seases such as keratomalacia and optic atrophy. Most were aged 11 t0 20 years. 33 were first or second-born, and 49 were third or later-born. 26 students' mothers had suffered from malnutri- tion., common cold, or other ailments during pregnancy. In 6 families, the parents were con- sanguineous. Hereditary eye diseases a.re now an im.por- tant challenge in China. Preventive measures are therefore proposed.     

CYCLIC OCULOMOTOR SPASM AND RELAXATION PHENOMENON REPORT OF 2 CASES

Two cases with cyclic oculomotor spasm and relaxation phenomenon are presented with a brief discussion of the nomenclature, clinical manifesta- tion, etiology and treatment.     

THROMBOSIS OF THE GALENIC SYSTEM VEINS IN THE ADULT

A case of thrombosis of the Galenic system veins in an adult is reported. Psychiatric symptoms were the initial manifestation, and the diagnosis was made at necropsy. We believe that this condition is not as rare as it seems and that more careful clinical scrutiny and more complete examination at necropsy would disclose more cases. We suggest that the neurologists and neuropathologists consider the possibility of thrombosis of the Galenic system veins, whenever they encounter a case of thrombosis of the cerebral venous sinuses in which psychiatric symptoms are the initial manifestation.     

SOME ASPECTS OF RESEARCH ON CHRONIC BRONCHITIS IN CHINA*

During the past 9 years, much research ha.s been carried out in our country on chronic bronchitis. Ma.ssive antibronchitis campaigns have seen Chinese traditional medicines investigated, synthesized and popularized, vaccines are used when necessary and the majority of cases im- proved symptomatically. The contradiction be- tween clinical improvement and deterioration of objective parameters is to be studied.     

CYST DECAPITATING DECOMPRESSION OPERATION IN POLYCYSTIC KIDNEY PRELIMINARY REPORT OF 52 CASES

Decapitating decompression of polycystic kidney relieves local distension aching pain and renal hypertension satisfactorily, all 52 patients in this series being relieved of pain and the great majority with complicating hypertension having their blood pressure lowered. Some with bila- teral decompression had blood pressure con- trolled at normal for 7 t0 12 years. We believe that if the patients are properly selected, this operation relieves symptoms, delays progression of the polycystic condition and pro- longs life.
